Mom is leaving the facility on Saturday, she's happy to be going home but a little sad to leave as she has enjoyed the camaraderie with all the staff. Although there was one issue when she first got here it was immediately taken care of and the director called me personally to make sure things were taken care of and that mom was comfortable and happy. The director is also going to help us with an issue we have outside of here. She is going above and beyond and it is greatly appreciated. This place is wonderful and caring.thank you to everyone

Shelby at the front desk, went the extra mile to help save my bacon today after I forgot forms for a patient to sign. My office is 40 miles away and her willingness to help not only ensured my patient completed necessary healthcare forms. It saved me from a severe panic attack. My 91 yo mom was taken to Scripps Encinitas ER due to inability to drink water from sedation ordered by hospice.Mom has a strong body but now has severe dementia.The hospital hydrated her and she was transferred to Aviara right across the interstate 5 which is close to where we live.The staff are excellent in caregiving and my mom is being kept safely,her diaper is changed frequently,the food is good especially the mashed sweet potatoes,and she has not fallen down.My mom is thriving now and I am happy for her and myself.

The staff at Aviara are exceptional from the Admission dept. Sandra to Jordan and Angela in case management. All go above and beyond. Came to visit a longtime friend, and was very impressed with your lobby. A scent of fresh air literally. I was also impressed with your check in procedure which allowed me a name badge. As I walked in, I couldn't help but see how friendly the staff were. I didn't stay long but for the time I was there I'd give your place a 5 star rating.

Every day at Aviara I felt I was living in encinitas, which is truly a beautiful city, Aviara truly turned my outlook of facilities upside down it is so nice, friendly and clean. Despite never leaving their facility my entire stay due to injuries and severe pains, The courtyards, large windows with terrific views, and their really cute backyard made me feel like I was still in Encinitas. Residing at Aviara was one of the best chapters of my life in fact. People there are in good spirits, and if it’s not the residents, the staff are the ones who brighten not only my day, but the residents. I witnessed their great activities such as live music, bingo, even painting. Everyday, literally on a daily basis, I would see the cleaning staff cleaning facility including our bedrooms and the floors keeping everything sanitary and clean which was really important to me. The staff was amazing. They really care about the residents who live there. They are so delightful, funny, and caring. With that, the administrative staff, nurses, cnas, kitchen staff, etc are all extremely organized and do a great job delegating and deliberating tasks. With everything, I know exactly which staff to go to if I ever needed anything. I am truly thankful for my time at Aviara.

My father has been here just a few days. But I can certainly say that he is being very well taken care of. The staff is friendly, responsive, and follows up on requests. My father is comfortable. Hi, this place is amazing, very helpful from the first day felt the kindness and helpfulness from different staff and knowing that they're around the clock so quick to ask if mom needs anything always have patience to listen, so glad to see the change in her recovering days here and a blessing to be here and that she is healing each day. I wouldn't think of anywhere to be but here is a blessing for her to be here at Aviara. Blessed the staff and the great work that they put in each day. Aviara has put our mind at ease when our father had to come in after open heart surgery. We feel he is well cared for here. Staff and been attentive and friendly and satisfy any requests we have. PT, OT, respiratory therapy, three square meals a day, also the staff comes in and checks on him regularly. We have a garden view window we can open for fresh air. We had a couple of needs right when we got here. We Needed a power outlet, the blinds wouldn’t open. They came in and replaced the blinds and added outlets to the wall immediately. When I became concerned about some of his symptoms I had three different doctors or nurses come in to tend to my father They also have daily activities for those patients that want to be social

So far my brother has been there over a month and the care has been the best . He had a stroke at 53 on 12/25/2023 And also every hospital and facility have not given him the care he’s getting here . Staff is professional, caring , shows empathy toward others as well . Clean rooms , hallways etc . And no smelly facility. I noticed no employees leaning against the walls and chitchatting. My beloved 97-year-old Papa has been a guest at Aviara for the past month, and likely will remain for some time. Our family chose Aviara among several long-term care facilities in San Diego County upon serious consideration, but I hope that my testimonial will highlight Aviara’s amazing support and services to its clients. First and foremost, Aviara involves my family in Papa’s overall care; WE are part of Team Aviara! I was with Papa 24/7, and got to witness the sensitive care from Team Aviara (around the clock) that validated and accommodated his physical and mental challenges. The entire team at Aviara “ from the front desk, to the fabulous nurses and nurse assistants, to the food service personnel, and all of the therapy professionals “ really care about my Papa; and listen closely to my family when we offer suggestions to accommodate and assist him. When Papa had a medical emergency last week, the Aviara Administrator and an entire team responded to him within 30 seconds, and did everything needed to assure that he would be okay. The facility is very lovely and clean, with beautiful outdoor spaces for their clients to enjoy. Team Aviara keeps re-arranging Papa’s room to make it easier for him to call for assistance, watch TV, make phone calls, AND arrange for family visits that involve many of us. My family gives Aviara a 10+ rating. While I hate that Papa is not home, I know he is safe, sound, and well cared for at Aviara.

This is the second time that I have been in aviara rehab and I wouldn't choose another place I have had nothing but positive treatment here starting from the CNAs all the way up through the nurses and especially the rehab I have received with OT and PT if you are looking for a rehab for yourself or someone you know are a family member please consider aviara as I don't believe you will find a better place to get rehab done by caring loving people

My father was in this facility for more than a month and in the beginning there were some challenges, but once we both spoke up about what he needed, things changed for the better. All the staff was very understanding and helpful with whatever my dad needed to be comfortable at Aviara. We especially loved Magdalena, the CNA and also Hugo the RN. They work so hard to get all their patients what they need. Angela the Case Manager, was and still is very helpful with my dad’s discharge, making sure he has everything he needs in my home now. My dad said he is definitely going to miss being there because he started to like his routine and all the staff. Thank you so much for taking good care of my father!

The most anxiety ridden experience is with pain medication. It’s not well run & I’m expected to keep on top of it , instead of concentrating on resting. I’m told one thing by a nurse & the opposite happens, which leaves me suffering for hours @ a time. By then, the medication I’m given barely takes the edge off & seems worse. Which raises my blood pressure.

Always clean. Very attentive and helpful staff that speaks good English. The patient I was visiting was there for over 3 weeks. She thought the staff was sweet and helpful and she liked the food. One thing I will say is PAY ATTENTION to the physical therapy the patient is getting. Sometimes the therapist, while sweet, push the patients too hard. The patient I was seeing is 100 years old and had had a fall. The PT had the patient do leg lifts with weights. That lead to soreness that caused the patient A LOT of pain because the leg was already injured from the fall. So as a family member SHOW UP at one or two of the therapy sessions just to monitor what ia going on.

This place is the best! We have been looking for a place for my brother for the longest time and have been at several facilities throughout San Diego county. The caring and energetic staff, cleanliness of the grounds and huge windows that pull in light all day are huge amenities. The food is great as well and my brother is well fed and seems very very happy, happier than he has been in a long long time. Thank you to all of the staff at Aviara!

Nataly certified nurse assistant for my roomate n I caught me at a very frustrating moment with a recent leg amputation and she didn’t realize until I told her she literally snapped me out of a funk and the rest of the night went good as could be the remainder of the shift. She really sets the bar high and again tonight I felt so wanted here and comfortable with Natalie around. Thank you again Nataly n Aviara. Other great mentions charge nurses Cici n Britney med passing nurses Estevan Tony Gaby Marylin assistant Jesse. and in kitchen I believe his name was Jawan always smiling n contagious laugh.

Trying to find a skilled nursing facility for a loved one is not an easy or enjoyable process. And the way that Medicare is set up makes it particularly frustrating if your family member needs cancer treatment. My sister and I have been in a stressful loop trying to help our father find a place to recover after his recent hospital stays. Aviara has come through for us. We appreciate the friendliness and hard work of Thrina, especially as she helped sort out a room readiness issue for our dad. We've also been impressed with the nurses, Brittaney, Emily, and Evelyn, as they've already helped us feel more at ease. And the food management staff member who came and spoke to our dad about his diet needs a shout out as well. Our father has a sorely limited pallet, and we appreciate any help we can get in finding solutions to get him more nutrition. There's no way to feel "good" about a situation like ours and no matter what SNF he ended up at, we knew we would need to stay involved and be present when we can to advocate for him, but that being said, Aviara has been open and inviting to us. Currently, Aviara is going through a refurbishing process. There is a wing of the building that has not yet been updated and it looks pretty rough, which is why I am so glad to know that they are changing things. Our father was in one of the older rooms the first time he came in, now he is in one of the refurbished rooms and it makes us all the more grateful that they are making this investment. That effort is more than can be said about other SNFs we looked into. It's our sincerest hope that he can recover and go home, with our continued support and the assistance of the staff at his new temporary residence. And even if he doesn't recover as we hope, the biggest help is knowing that he can receive some consistency and kindness where he is.

Great staff, social services and case manager are compassionate and resourceful and advocating for patient needs as they transition home. I came to visit a couple patients that are our clients as we are helping them find a senior living community Our client said that she really likes it here. I love my time visiting clients and knowing when they have good care.

All the staff at this facility are well trained in the professional detailed care of each specific patients needs. You definitely get round the clock 7 days a week medical attention and the physical therapy department personally for me, got my body back in as good of shape its been in about 9 years.

My mom has been here for four weeks, and our experience has been very positive thus far. All of the CNAs RNs, wound care, charge nurses and incoming registry folks have been so supportive and willing to support both the families and more importantly, my moms care. I have found them to be very compassionate and empathetic to my moms dementia needs. Special call outs for Bernadette, Evan, London, Charles, Abby, Mel for outstanding care. In addition, the activities director is excellent in making things fun and engaging for the community. I do come a lot to visit and observe the staff which I think is critical to ensure best care for your loved ones. I would definitely come back for my mom if needed. She has improved much more than the doctors expected which is a testament to her great care.

This is my second time updating the review of this facility. I initially gave 1 star based on my uncles first night there, but subsequently we were pleasantly surprised by the professionalism and compassion of some employees and increased the review to 3 stars. In particular, CNA Bianca (I hope I got her name right) was very helpful and showed real caring when helping my uncle today. Many other staff members were also super today but I do not remember their names. But since my second review we had some additional disturbing things happen which made me lower the rating to 2 stars. The main issue is that the nurses did not seem to be up on my uncles condition or why he was even there. For instance they didn't know he was there preparing for surgery. They also did not give him his medications as ordered. They tried to blame the pharmacy for not writing the order on the pill bottles, but the bottles clearly said to see the attached doctors orders (I saw the head nurse throw these away). Next, my uncle was not given a walker his first night despite obviously needing one and despite our pleas. Its simple common sense, not everything has to be thought of in terms of liability. Thank goodness he did not fall his first night. He did receive a walker the next day. Second, the lock on his window is broken and leaves his room unsecured. This is still an issue. Third, he was not given a remote control for the TV on his first night for reasons I will never understand. It seems there is a dedicated employee who hands out this precious commodity. Why can't the remote stay in the room?? Not a big deal, its just a matter of common sense if you are trying to make patients comfortable. After my uncle had his surgery we made sure he went elsewhere for rehab. He went to Bayshire in Carlsbad which was great and highly recommended.

Great care, the nurses and assistants are knowledgeable, caring and efficient, the dietitian is knowledgeable and will discuss diet preferences with you, the physical therapy with Mark and Henrika is incredible, ; the occupational therapy with Matthew and Cece is incredible, they are knowledgeable and experienced and fun to work with! The office staff, Trossi and Rosa, and Social Services Director, Bernadette are very knowledgeable and helpful! And very important, a great cleaning staff, Aviara is very very clean!

9 months ago Just was so impressed with Aviara we recently toured and then placed my mom at Aviara. The community is clean and smells amazing. The staff are so compassionate. They care about my mom and my family. The Marketing Director went way above and beyond she Walked me through every part of the admission and held our hand when we were worried and scared of the process. They love my mom as if she was their own family. The nursing director is compassionate and attentive. My family can not be happier with this community.
