My husband spent 19 days in Autumn Care and the services were excellent. The staff is very caring and treat the residents with care and dignity. The facility is very clean no odors at all , even though some of the residents are incontinent. Each resident was dressed appropriately and well cared for. Therapy was great, they got him walking and independent. I highly recommend this facility.

My mom recently had a stay at this facility after having a knee replacement. Simply put, this facility was exceptional. Every interaction we had with the staff was very respectful, compassionate, and made my mom feel loved and welcomed. The nurses and aides made sure that she had everything she needed. Her pain was managed extremely well by the clinical staff. She was seen very promptly by the physician and all the questions we had were answered. The therapy did a great job of getting her up, moving and conditioned to return home. The progress they made with my mom in such a short time was amazing! The social worker spent the time with her to identify everything she needed to get home safely and had her set up with outpatient physical therapy the very next day. You can tell when you walk in to the facility that it is very well maintained. The housekeepers do an outstanding job at keeping the facility clean. The housekeeping staff also were very caring, always asking how my mom was feeling when they came to clean her room and empty the trash. I would 100% recommend this facility to anyone needing rehabilitation or needed long-term placement for themselves or a loved one. Our family sincerely thanks you and appreciates all the care you provided my mother!!!

In 2019 this place was great but it has gone down hill in a handbasket. After my husband's back surgery I asked for Autum care because of our past experience. NEVER AGAIN. On his first day there, I asked the Nurse on duty that when she "got a chance, could she please give him a pain pill" I was told and I quote "I have 24 other patients and I haven't even met him yet, I will get there when I get there" Really? What an attitude. This is NOT the place for this Nurse. This incident was reported to the care team and I was told it would be taken care of. After a few days, my husband became confused, complained of burning when urinating and sick to his stomach. I asked this same nurse to have him checked for a UTI. I was told she would put a note in the chart. This ended up there was no note and the next Nurse who was fabulous (Rosie) told the PA. I was told this PA said she looked at my husband and didn't see any signs of a UTI. (By looking, really? and talking to a confused man, Really?) After I purchased the strips from CVS I tested him twice, Positive both times. Then thank God for Rosie because the next day he was sitting in the wheelchair for rehab and she thought he was having a stroke. They called an ambulance for him and low and behold, after tests at Duke in Mooresville he not only had a UTI but an E-Coli infection. He was treated with antibiotics and after 4 days release to a new facility where he is now getting great care and extensive rehab. The food at Autumn Care is horrible and always cold, but they do offer to heat it up in a microwave. Have you ever heated up scrambled eggs in a microwave? They are like rubber. I absolutely DO NOT recommend this place. It's really a shame because it really used to be a great place. I will say one thing, It is very clean and has no smell, but that's it.

I can easily say that this is the worst medical care I have ever received. This is the first review I have left on a facility--in general, it takes a lot to upset me. I am compelled to let others know what they may face in Autumn Care of Cornelius. It was recommended by the hospital that I receive rehab after treatment for a severe break in my femur. I was given a list to choose from with star ratings. Autumn Care of Cornelius had only a 2 star rating, but a woman visited my hospital room and really encouraged me to try their facility-- that the poor rating was an old rating and they were a 4-5 star facility currently. She showed beautiful pictures and we were impressed with what she had to say. I checked other ratings online and the ratings were wide ranging. We decided to give it a go because it was close to home. As I was being transported to the place, I got a call from a neighbor warning me that many of the members of the neighborhood were "aghast" that I had opted for Autumn Care of Cornelius. I figured "What the heck--I am pretty laid back, easy to please, and it's only for a few days anyway". I am so sorry I didn't listen to them. I have been treated for years for chronic pain, due to a failed spinal fusion and peripheral neuropathy. As a psychologist, I am well aware of the dangers of addiction and opioid use, but my treatment at ACOC was nothing less than torture. My pain level from the accident was extreme, but it was never treated adequately. They would only give about half the medication I was taking for pain PRIOR to the accident. My new injury wasn't being treated at all and it was joined by pain from my prior condition. After hours of unrelenting pain, I begged them to contact my pain doctor and my husband even brought in my prescription bottles to show them my baseline pain treatment levels, but they wouldn't budge. They said since I was scheduled to leave the facility for an appointmentwith my pain doctor in 2 days, that I could just have him send recommendations back with me and they would honor them. I suffered through 7-8 level pain for 2 days, came back from my appointment with recommendations of my regular pain treatment plus a little extra that I could request for an extreme break out of pain. I was so relieved that I was going to get some relief, but very little changed in the next day, so I demanded to be released. They tried hard to convince me otherwise. One nurse tried to explain that some of the staff were misreading the dosage of the pill given (no excuse at all!) Ironically, the Physical Therapy team was outstanding, but I can't recommend you send your loved one there due to the unresponsive medical team. It is hard to give even 1 star. I encourage you to heed the warnings that I ignored.

I am a hotel manager and I just wanted to share this with any of those out there needing short term rehabilitation or skilled nursing services. During the recent ice and snow storms, Autumn Care housed the staff at our property. This action was to ensure that would be no deficiency in care to any patient. I enjoyed meeting a number of the staff members. You can tell that care about their staff and undoubtedly care about their patients.

I’ve been at this facility approximately 3 to 4 weeks now. I’ve been both here and back to the hospital. Overall, the physical plant is excellent. The overall level of service is very good. On a personal note I’ve been in three or four nursing homes in the last 14-18 months, I would definitely put this on the top of the hierarchy. People come here are fortunate. Physical therapy is quite good. They are knowledgeable and I’m moving forward with my goals that will take me home.

My wife and I both have been patient (whatever the nomenclature is for people that need care I need to be rehabilitated.) We both were here for periods of time and we both were extremely well treated. I’m very very happy with the personnel they’re friendly. They’re courteous their professional and I would if I needed a rehab again I certainly would never hesitate to come to autumn care of Cornelius. Could you extremely well and help you rehabilitate and that’s the name of their game thank you very very much I’ve made good friends and Jasmine and Jessica and Rebecca and they’ve been extremely helpful and they’ve handled my wife and myself in a very friendly and compassionate way. And in a professional manner. the only reason I’m going home is because that’s where my house is and where my previous life is. I’m going to try to recoup that life and if I can’t, I certainly will come back to autumn care of Cornelius or asked to come back for additional care

My brother ended up in the hospital with urgent/emergency needs and surgery. He then required rehab services. There were about 20 facilities on his approved insurance list, and Autumn Care was closest to us. I'll be honest....he got there late in a Thursday afternoon....and the weekend was tough.... BUT....the remaining 5 1/2 weeks that he was there....100%%%%%%%%% Alex, Cici, Jasmin, Brian, Courtney, Michelle, Theresa, Daron.... SO MANY OTHERS.... and sorry that I have forgotten some names... We are one week in to his new residence, and everyone there also claims that the entire team at Autumn Care has created a huge step forward to his continued recovery! And, his oncologist agrees as well! Thank you Autumn Care Team!
