My name is Mark Johnson. I am the new Administrator for The Convalarium of Dublin. I started November 4th, 2024. I have been a licensed Nursing Home Administrator in the State of Ohio since 1984. I am thrilled to be the new Administrator of the Dublin facility, which is operated by LionStone, LLC. Along with a new Director of Nursing, Moni Hawthorn, we have several other new managers and are committed to making The Convalarium the facility we can be proud of. We offer Ventilator Care and NIV, non-invasive vent care, with 27/7 Respiratory Therapists on staff. We also traditional PT,OT,ST with our own Lionstone Therapists. The past 3 years has been hell on me since the woman up north try to poison me I'm the man that truly believes God will judge me for whatever I say or do I was doing some research and I got to make things right I want to get physical therapy five stars in the nurses five stars I made a mistake on these pills that was given to me and I thought they were codeine pills but I was wrong I didn't look at the numbers correctly on them and I forgot to add the TL all I did was keep looking at the numbers 172 what showed it was coding so I made a bad choice in a bad mistake I'm trying to make it right nobody sold me any pills one person said I said that and I did one person said that I said that and I did not I thought these pills were given to me but they turned out to be a steroid I was so upset thinking I was going through with what I went through with this woman up north thank God she's locked away but anyhow I'm trying to make a wrong right the only bad thing about this place it stinks like any other nursing home I can only hope and pray people forgive me for being so upset but to know where I've come from and what has happened to me the past 3 years I'm lucky to be alive I hate taking any pill I never took any meds at all not even for a headache cuz I figured I'll get through it but the last 3 years has been rough on me I just hope everybody can forgive me for the ugly review I put on this place I just. I just wish that some of these AIDS would not bring their problems to work with them and take it out on us we're going through enough here and there are a couple of them like that I just wish the nurse would sit down and listen to me what really happened for what's happened in a position I was put in to feel the way I do a little bit of patience and kindness goes a long ways with a person but if I don't have it for me nobody else is God bless you all.

I was admitted here for what was supposed to be two weeks of physical therapy which has turned into a long term stay. They have taken care of my needs and swarm like honey bees to a hive when any issues have occured, taking care of them promptly. I couldn't have asked for a better facility or staff to have stepped in and help me when I could no longer care for myself. They go above and beyond to provide and maintain a clean and safe facility, serve quality food with many choices, give outstanding physical therapy, provide impeccable aide care as well as knowledgeable nurses, nurse practitioners and doctor that have never stopped trying to find out what is going on with my health concerns. The activities ladies Richelle and Diane always create interactive activities between staff and residents keeping our minds and bodies moving and in good spirits. I can not say enough about the social worker Tonya who jumps leaps and bounds to make sure that I have been able to get the help and care I need in this difficult stage in my life. You will not be disappointed or worried putting a family member here to be cared for, they treat us as they would their own family here at the Convalarium of Dublin.

I have worked in this building for over two years and there are so many great things about the Convalarium! We have had changes in the last year but each one for the better care of our residents. We are finally agency free and have hired our own staff - individuals who are committed to consistent care for our residents. Our therapy team is incredibly knowledgeable and work diligently to successfully return your loved ones back home as independent as possible. We have 24 respiratory support for residents living with Ventilator needs. They are very experienced and work closely with our facility Pulmonologist. We have two physicians and a CNP in the building M-F. Our management staff is working hard to address resident and family needs, educating and supporting our staff. Our new activity director is enthusiastic and already planning enriching activities that have our residents excited again! I truly enjoy being a part of the Convalarium staff and working for our residents daily!

The staff works hard and as a team. Everybody helps in anyway that they can to help endure the care and safety for the residents and that just shows the good people that work here.

The care is bad or neglected in this facility! Bob's dirty trousers were put into a plastic bag and then thrown into a cupboard for more than a week but not washed. His eyeglasses were inside the pocket, no one could help him to find his glasses for a week until this afternoon when I collected his things for taking back home, I opened the bag and found the eyeglasses. Originally I didn't want to write this review, I didn't mean to raise this case. But just half an hour ago when Bob went to bed, took off his trousers, the scenario really surprised me! I don't know how many days he didn't take a shower. But I am sure that there were no service people to remind him to change his underwear everyday. I had asked many times to put his daily schedule and his medicine list on the wall, but no one would do it. There, Bob's gum hurt for a month, didn't get addressed and any help. 11:14 pm (07-01 11:16 pm) When I read the book of Nursing Assistant Care - the Basics, I further realize that the service of this facility is really unprofessional. (1) The overbed table was often piled up with other stuff; (2) After 12 pm, the breakfast tray was still on the table. After 3 pm, the lunch tray was still on the table; ;(3) The trash was often not emptied daily; (4) One of the directors didn't do what she said she would do.

My mom has been receiving excellent care while at the Convalarium. The nurses and aids have treated her with dignity and respect. The admissions team has been helpful and understanding during one of the most difficult times our family has experienced. We would recommend this facility for both short and long term care needs for your loved one.

My aunt was recently a resident for a short skilled stay. I wanted to share because it seems that people more often than not only share bad experiences and I want to share a good one. The staff was so friendly, they have even called to check on my aunt after her discharge. The food was good, there is no odor, friendly staff, great therapy. I think that we often forget that this is a business of people, and the people here truly care about making sure their patients get better so they can go home. The few minor concerns we had they addressed immediately. My aunt told me that if she ever had to do this again this would be the only place she would consider. Wonderful facility!! This place has such a homey feel to it, my aunt was comfortable from the minute we walked in! My family highly recommends this facility! Don't get fooled by the glitz of other facilities. You should concern yourself with how people treat you, and how they treat their patients when you take a tour.

No regrets; A couple years ago my mom fell and broke her hip. We had her do rehab at the Convalarium. During this time her dementia grew worse so we decided to see if they could accommodate her for long term nursing care and without delay they said yes. The entire staff is totally professional and caring toward her needs and making sure we are kept up to date on any changes in her health. Not only do they take care of her they have also been an enormous help assisting me in all the paperwork that is necessary during these critical times. The food is outstanding and much better than the previous assisted living facilities where she lived. If your in search for a rehab or long term care facility for a family member I would highly recommend the Convalarium.
