My mom is a resident here. She has been living here since January after a medical scare. She like it here, except she has a hard time with the food Staff and nurses are so dear and caring.

The staff was so kind and caring to Larry. He lost 20 pounds while there for a week and a half cause they couldn't meet his dairy free ( lactose) and gluten free needs. Thank you for your very kind, caring staff, CNAs, like Alex, Terry, Erin, Deanna, all the CNAs provide such dignity with their care and work so hard; plus CMAs, nurses: Marigold, Judy, all the nurses, PT, maintence (nick-name "scotch?") for promptly replacing landline , admission Lisa,Discharge Vikki and Jenn, social services, Ms. Hanley, , very hard working cleaners - housekeeping, Food Bill and Carla, so many to thank for your care and kindness and help to Larry, helping both of us. Thank you. Larry is in good hands with you and your care and kindness.

Avamere Three Fountains was an amazing experience. Their staff is amazing and are always providing great care. If your love ones need rehab I would recommend Avamere Three Fountains.

Avoid this place at all costs.the staff do not care and they give you the cheapest food they can.To make matters worse they have a nasty new York pervert CNA working there named [name removed].that guy is a real piece of healthcare trash That needs to be fired and kicked to the curb.Nurses and caregivers do this all the time, it's called boundary violations and is subject to license revocation!!

My wife and I were both recently at Avamere at Three Fountains skilled nursing facility for 2.5 weeks following a car accident. We received physical and occupational therapy while there, along with nursing care. We both thought the care was of very high quality, with everyone who tended to us being caring, compassionate, friendly, and respectful. Calls for assistance were answered promptly day and night. In addition to good care the therapy was practical and driven by our need to be functional when we left. Meals were tasty and sufficient, with menu choices at each meal and light snacks were available as well. The facility was clean and fresh and well maintained. Family who visited were all very complimentary of the facility, noting how friendly the staff was and how there was no hint of the disinfectant smell many of us associate with care facilities. Can highly recommend Avamere at Three Fountains.

My wife Gretchen came here 6 days ago from Providence and this place is 1st Class! Fantastically helpful and friendly staff, from the top to bottom. The CNAs and Nureses are superb. The office staff is extremely professional, knowledgeable, and always helpful. I eat here as a guest with my wife and the food here is really EXCELLENT! Yes, it's amazing. I hate carrot cake and tried the carrot cake here and it was unbelievably good! The seafood and fruit salad for lunch today was truely excellent. The staff takes super good Loving care of all the old folks, I come here every day and see it first hand. This is a fabulous place to take care of your loved ones. Nice big rooms, well lighted, smells great and always a comfortable place to visit where you KNOW your loved ones are being taken care of. 5 star rating! SFC Patrick Purtle U.S. Army (RET).

Just moved in my sweet wife so far. Everything is so clean and beautiful. Staff is friendly an alert. Honestly, I pitched a fit. We had about experience in Utah with a similar facility so I checked the ratings period I'm not sure why people hated this facility and I will be on watch. And will update has taught him goats so far, it's perfect.

While I don’t think the facility itself is run the best that it could be, I want to thank individual staff members who went out of their way to care for my mom and were very supportive and kind to me during the last three years. CeeCee was so sweet and kind and treated my mom with respect and kindness. She would braid my mom’s hair for her, wash her eyes gently with a damp washcloth, and speak to her and check on her often. I knew that whenever CeeCee was assigned to room 52 my mom was in the best hands. Michael and Raul also treated my mom well and joked with her which I know she enjoyed. Bev and Lexi took good care of her and both of them did a good job of keeping me updated on what was going on. Lynette and Chris are both excellent and I appreciate the kindness and tact that they both showed. Brandy is another great person and she continued to care for my mother’s welfare and show me support even when my mother wasn’t able to do physical therapy anymore and wasn’t really her responsibility. Karen took care of my mother’s diabetic needs with her toenails, among other things, and was also a great source of comfort to me. Doreen was good to us and always offered Mama ice cream and other treats to try and get her to eat. The day I went in to pick up my mother’s possessions after she passed was incredibly difficult and Vicki, Brandy, Michael, and others were wonderful to my brother and I” helping us to sort what we wanted to donate from what we kept, and providing encouragement and hugs. I apologize if I left anyone out or misspelled any names - I will be forever grateful to all of you and thankful that you took care of Mama and her physical needs when I wasn’t able to. Thank you so much.

Honestly the best care Ive seen yet! At first I was A little skeptical as there was some phone issues but every visit I’ve had the care is exceptional! And the phone issue was resolved! Very clean and awesome care so far.

My mother is now 100 and on hospice care. But in the 4 years under the care in the long term nursing unit, (sheeven surviving Covid), I have had the utmost respect for those involved in her daily care.

Mom was treated with kindness and respect by all employees. PT and OT were very kind and professional. Food was delicious; she is particular about her food being hot enough so, understandably, that standard was not always met. Avamere was strict about Covid compliance, as it should be, but when the state allowed it they readily permitted visitors by appointment. Jessica, an administrative employee, was accommodating to meet the need for communication between the facility and the family, as needed. I give Avamere at Three Fountains high marks for their care!

My mom was admitted for 2 weeks of rehabilitation after a knee replacement. She was so impressed with the care from the nurses and CNA's. Now on week 3 she is ready to come home. She's been cleared by PT, OT, and everyone else needed. Today they told us when she was admitted they assessed her and decided she needed a MINIMUM of 5 weeks. They said it's not a prison, she's welcome to leave any time she just had to sign an AMA. what they don't tell you is if you sign out against medical advice your insurance most likely WON'T pay the bill.

My dad is at Three Fountains as he has a broken wrist and AFib. They do not want him to anything his self afraid he might fall. Understandable. But it has been hard for him since he was very independent before. I am waiting to get a report from the social worker so we can bring him home. Hard not being able to visit him. My dad says they feed him well.

Pros - Some employees very nice and competent. Cons - Understaffed. Bad food. Thefts from room. Sloppy RNs (one let her hair dangle in an open wound.) Poorly trained staff. There were two occasions where it took four hours to get pains meds. I wouldn't want to stay here.
