They went above and beyond taking care of my grandma! I live out of state and it was amazing having peace of mind with her being there. The staff is so caring and friendly. My grandma always raved about the food!

Nurse [name removed] needs to brush up on her bedside manner. She's a prime example of why nursing homes get their reputation of patients being verbally abused. And, what gives her the right to make patients do more than what their physical therapists allow them to do? I don't care that it's the rehab to home unit. It is not Nurse [name removed] decision to make until the therapist releases the orders. There's going to be a day when life bites [name removed] in the butt just like her patients. I really doubt if she'd appreciate the same kind of care that she gives out. I have nothing but respect for the rest of the nurses and CNA's in that unit. They have all been very sweet and respectful despite of the challenges they face. Kudos to them. May I suggest you compensate your employees better so the good ones will stay?

This place is amazing. I came here for three weeks of hip replacement rehab and knew the minute I walked through the door I was n the right place. The rehab staff is awesome -- very knowledgable, very professional and love to have fun while doing it. The food is great because the cooks are great. The care staff is very caring and always on top of things. Lots of empathy going on around here. Everybody chips in. At meal times, it's all hands on deck - administrators, nurses, maintenance people and the cooks themselves are serving food to the residents. It's a team effort from the word go. I've been coming back as an unofficial volunteer; now I'm back for in-patient rehab. It's the best place to be in Wabash - hands down.

Wellbrook of Wabash is where my neighbor chose to go for recovery. The people there took to her instantly. The environment always seems to be clean even with all the people and residents that are in, out, and around the building every day! I'm only at day 7 but it's been a pretty great 7 days!!

Wellbrook served our family at a crucial time. Their staff was competent and very kind, going above and beyond on their end to admit her and get her the care she needed. We had no way of knowing it would be her last days, and we could not have asked for a better staff or more warm atmosphere to care for she and our family at such a crucial time. I cannot find words to express how thankful I am the Wellbrooke. This facility has gone above and beyond before my mom was even a patient. After getting discharged from the hospital we were sent to Waters Of Wabash West. The facility was NOT what was advertised online. It looked like it hadn’t been updated in decades and I honestly don’t know how they have passed inspections. I feel so terrible for the people that live/have lived there. While I’m their care mom declined rapidly. She was supposed to be there for therapy but was there 5 days before even getting her evaluation. While there she lost weight, was bathed occasionally, and almost never left her room. Her depression worsened and she became reclusive. I reached out to Cassie at Wellbrooke on the 5th day she was there when I realized how bad my moms situation was. Cassie in admissions immediately made it her mission to help get my mom out and to her facility. She worked with me and my moms insurance case manager. We were immunizations almost every day, if not every other day. Only 12 days after my mom was at the Waters of Wabash West, they called asking if I had heard from my mom that day. No other context was given and I could tell by the ladies voice she was concerned/nervous. I asked why and ahe hesitantly said mom was a bit more painful than normal and had vomited. She wasn’t constipated so that’s not the problem. They were giving her another pain pill and nausea medication. I immediately tried calling my mom but she wasn’t able to answer. After sending family there (at the time I was in a different state) they said she looked terrible and was in severe pain. I could hear my mom crying out in pain. I immediately called the nurse at the waters of wabash back and demanded they take her to the hospital. They replied with yea, that sounds like a good idea. I’ll print the papers and call the ambulance. I called my family back to tell them if the ambulance isn’t there in 10 mins to call themselves. Mom ended up having to be transferred to a trauma center for emergency surgery and was in critical condition. While at the hospital, the physical therapy team was appalled by how much mom had declined in her PT/OT abilities since leaving the hospital only 12 days prior. They were beyond happy to know mom was not going back to the waters of wabash. The whole time mom was in the hospital (11 days) the waters of wabash NEVER ONCE called to check in on her. Meanwhile, Wellbrooke (again she wasn’t even officially a patient with them yet) called the second they learned mom was in the hospital to see how she was doing. Cassie has been a life saver and I cannot express how thankful I am for her. Sean even came up from Wabash to her hospital (1.5hr drive) to see mom and make sure they had everything they needed to get her to Wellbrooke at discharge. Needless to say, never take your loved ones the the waters of wabash, but I am feeling much better and have 100% confidence that my mom will be cared for at Wellbrooke.

I can’t say enough about how amazing my experience was at this place! I had the privilege of being assisted by Kristiana, who was unbelievably exceptional and was extremely kind and professional. She was so helpful towards me and my family and she really made our experience there even more enjoyable. Overall I had such a great experience and I will definitely recommend this place to my family and friends as well as recommend Kristiana for any service or help they need!

Met with the incredibly kind representatives from Wellbrooke and could not be more impressed. They not only answered all our questions, but took us (I went with my grandma) on a very informative tour, introduced us to some lovely residents and sent us home with a warm and delicious loaf of freshly baked bread. Every person we met was so inviting and the facility is beautiful. With physical therapy on site and long term care, as well as multiple levels of assisted living, it wasn't a difficult decision. My grandma has already decided when the time is right this is where she wants to live.

My Father, Bob Hettmansperger, has been in for care at WELLBROOKE for several weeks last Summer, 2021 and again early this Spring and presently July, August and September, 2022. There is one Nurse that I want to publicly bring attention to. . . she has become our very favorite and Dad even invited her and her 2 sons to Bachelor Creek Church of Christ with him. She has opted to bring her boys and come to our Church in Roann, WALK BY FAITH. She has the biggest & most caring heart & goes above & beyond her duties there. Dad's stay was made extra special & blessed by the care of: ’’ Kristiana Swetman ’’ Anyone else that has had Kristiana watching over them, during their stay at WELLBROOKE, receives the same warmth & extra special care. . . I have no doubt!!! SHE IS THE VERY BEST. . . WITH SUCH A GOOD HEART!!! She has made our memories of WELLBROOKE warm & fuzzy, reassuring us that Dad was in excellent hands & care . . . ’’ We are blessed to continue our friendship with Kristiana beyond WELLBROOKE!!! ’ There are several other Nurses, that I could name, but Kristiana definitely stands out!!! ’’ Thank you to all of you at WELLBROOKE for the wonderful care of our Dad!! You have become "family" to us!!! ’’ I love how the Nurses and Residents warmly welcome and embrace my pups when I take one in. . . I just got home from taking COPPER in to see Dad this afternoon. . . and got detained sitting with some Female Residents at the Front around the Fireplace with her. This is the most beautiful senior living facility that I have ever seen. The Trilogy staff are well trained, friendly, and very helpful. They all have a servants heart and all are so passionate about their jobs. You can tell that the residents like living here by the smiles on their faces when you pass them in the hallways.
