I had a tooth abscess that caused an infection in my throat and trachea that caused me to go septic. I was sent to the ER by my dentist and had to be put into an induced coma. I was transferred to St. Joe's where I remained in a coma for 2 months and unfortunately also had a stroke. While I was in my coma I had a tracheostomy performed and a feeding tube placed in my stomach to sustain my life. When I woke from my coma, I was so weak and could not move my legs. I was transferred to Montecito for rehab in April. I remember feeling extremely anxious, was still on the ventilator, and could not move. I immediately started PT, OT, and ST. Brittney (COTA) and Brandon (PTA) were fantastic and truly exceptional people. We developed a tremendous relationship over the 5 months of therapy. They were so thoughtful and caring. They pushed me every day and I am so thankful for everything they have done for me. On day one, I could not move out of bed, stand, walk, or take care of myself. I am now independently doing everything but just a little slower. Jamie and Annika were my speech therapists that helped me get my swallowing strength back after being on the ventilator for so long and being unable to eat. I am very pleased with Montecito as it was a lovely facility. My CNAs were so kind, caring, helpful, and encouraging. Rashida was one of my favorite nurses. Eileen and John were my favorite respiratory therapists. All of them were so caring, warm, and truly cared about helping me. Montecito did not seem like a medical facility. It seemed more like a home with a big family. It was always very clean and just very inviting. I would highly recommend Montecito to anybody needing therapy.

On October 3, 2023, I was riding my motorcycle when I was hit by a car. I don't remember much from the accident but when I woke up in the hospital, I had a left below knee amputation. I was lucky to be alive due to the blood loss and trauma I experienced. I also broke my spine and pelvis in multiple places. The doctors in the hospital told me I would likely never walk again. I was transferred to Montecito on December 21, 2023. I remember trying to be patient and strong because I knew I had a long journey ahead of me. The therapists were very supportive and motivating. They helped me get a prosthetic and helped me defeat the odds of being told I would be wheelchair-bound for the rest of my life. I am now able to walk with a walker and am taking care of myself thanks to all my PT and OTs I have worked with. After almost 8 months of therapy, I am discharging back home to my wife! I am so happy that I chose Montecito and am forever thankful to the staff for taking care of me.

I cannot recommend outpatient Montecito Post Acute Care & Rehabilitation Physical Therapy Dept enough. The therapists specially Chris and Kyle are incredibly knowledgeable and skilled in helping patients recover from injuries or surgeries. They create personalized treatment plans based on individual needs and goals, and are great at explaining exercises and techniques in a way that is easy to understand. The staff is friendly and welcoming, creating a supportive and encouraging atmosphere that makes the rehabilitation process much easier. They truly care about their patients' well-being and are dedicated to helping them reach their full potential. I have seen significant improvement in my mobility and strength since starting physical therapy with Chris at Montecito Post Acute Care & Rehabilitation, and I am so grateful for the expertise and guidance of all the therapists. I like to start by expressing my sincerest gratitude to Kyle, DeNell, and Branden. They have been my greatest advocates during my recovery. I've had a few challenges during my time in hospitals and the Montecito. Regardless they were always right there ready to help. They even as far as to tell when I had appointments, doctors I needed to speak with, helping get the right medical equipment that I will need. When I had to go to the ER and leave the Montecito, I was gone for a week before I returned. When I did on a late Friday night DeNell checked on me the following morning. It all means a lot to me. Especially their patience, knowledge, and enthusiasm. It made feel welcomed and comfortable. I could go on and on about how great they are.

Kyle and Brendan at Montecito Post Acute Care and Rehabilitation 'Out Patient Services' have been instrumental in the in the recovery process of my husband, Ernie's, foot amputation surgery. Kyle has done an excellent job in helping us identify the goals needed to help Ernie achieve pre surgery activities in the home and outside of the home. Brendan has been instrumental in identifying the exercises Ernie needed in order to improve his strengthen to allow him to go from his wheelchair, to walker to his cane. Kyle and Brendan have made this very tough journey as manageable as possible and we are extremely appreciative for their knowledge and direction. Thanks to these two Ernie is on the verge of walking without assistance and we are extremely thankful for all they have done.

My friend spent 5 weeks at Montecito. She came from another SNF that was awful!! She had been in 2 others several years ago, and of these 4, Montecito was the best. Staff seemed to be on top of things from the beginning. Nurses, CNAs, case managers, NPs, even management we interacted with, dealt with our concerns quickly. The patient portal was very helpful and kept current with info. It was a great resource.

Chris was flexible with my needs. He always addressed my biggest issue but also helped me with any secondary issues I may have had. I was initially a patient in January 2023 at their inpatient after having both legs amputated below the knee on Christmas Day 2022. I discharged to a group home where I remained bed bound due to my legs still healing. I did not get my prosthetics until August. At that time, I was very weak from the prolonged bed rest. My prosthetist at Hanger referred me to Montecito's new outpatient therapy program where I met my therapists Michelle and Kyle. I was very depressed but meeting these two changed my life. They are so friendly and truly care about me. Michelle has been working on my right arm function from a stroke I had in 2019. Kyle has helped my core and leg strength more than I ever could have imagined. I would recommend this outpatient to anybody who needs it!

Montecito PAC&R outpatient services is incredible! My experience with the outpatient care I receive is phenomenal. Kyle, PT, and Michelle, OT, are extremely attentive in their care for their patients. I am a double lower-limb amputee, and credit the success of my progress to the compassion, dedication, and sincerity of commitment to Kyle and Michelle. I give them 5 stars. The gym is small, and can get a little crowded. The parking situation could use some help. I give this area 4 stars. However, one is able to still work through both and the Awesome care makes up for all of the minor things. I highly recommend Montecito PAC&R outpatient services for any rehabilitation needs! They do NOT disappoint & you will NOT be disappointed!

Nim was my Dads case manager while he was at Montecito and he was amazing. He is a kind, professional, empathetic, saint. He was updating me on a daily basis regarding my Dads progress, including on his days off. Nim truly cares about his patients and goes above and beyond to make sure they are getting the care and attention needed during this difficult rehab time. The nurses are very attentive and super sweet. I felt that my Dad was surrounded by sweet nurses without any attitudes. It was a welcomed pleasure after his hospital stay. Nothing but miscommunications. The admissions will promise to do everything. Doesn't happen. Daughter taken to respiratory unit & received no breathing treatments though they were ordered! Not fed on time or nurses did not know how?! No shower until we begged. Medications were ALL messed up! Given prednisone for something she had months ago & was not even taking now! Don't get me started about the actual physical facility — Ensign, the mother company - needs to burn it to the ground & rebuilt! 4 ppl should not share one bathroom. The space is WAY too tight & no room for your loved one. The air is stale & it smells awful! How is this environment conducive to healing? Even if they have decent Therapists, It's not worth the caregiving, which is all over the place, depending if it's a weekend or holiday. No one communicates with each other. The nurses & CNA's Even told us this... Which doesn't give you any assurance with your loved one. Would not recommend. Some people are very nice and communicate well, but The facility itself shows that the company just doesn't care about healing people or taking care of them properly. If you complain, you get excuses like... Well, that was a new nurse, can you have some understanding? Can YOU have some understanding?? That is my Daughter... Would you want your loved one here?? Interesting how there's no new SNF in the whole entire valley & all the ones we have suck! Bottom line, the company only cares about making money.

Our Family is forever grateful for Monteicto and their staff! My 90 year old dad just had a pacemaker put in and needed rehab for a few days. Dad was barely able to stand on his own when we left the hospital and after being under the care of Kyle in physical therapy and his staff he was up and around in no time. We are so grateful for Kyle he knows his stuff. People sometimes have a hard time with rehab facilities but please know you are in the best hands with this facility!! In July 2023, I was found down in the middle of an intersection due to heat stroke. I was admitted to KPC and was in a coma for several weeks. When I woke up, I had a tracheostomy and was dependent on a ventilator to breathe for me. I was diagnosed with metabolic encephalopathy, rhabdomyolysis, and renal failure requiring me to start on dialysis in the hospital which helped heal my kidneys. I transferred to Montecito in September 2023 and was thankfully off ventilator support but still had my trach and PEG. I grew to love all the employees at Montecito and feel like they are a big family to me. My nurses and respiratory therapists have been helpful from the very beginning with anything that I needed. When I first started therapy, I could not walk, was very weak, and felt like giving up. This team of PTs and OTs have been beyond supportive and motivated me to get better everyday. After 1 month of therapy and being at Montecito, I am now completely independent, got my trach and PEG removed and ready to discharge home. I want to thank everybody that has helped me to get where I am now and I will be forever thankful.

I was riding on my motorcycle to work when somebody turned in front of me resulting in me crashing into her car. I remember waking up on the curb after the accident with a bystander holding me while the paramedics arrived. The only other thing I remember was seeing my dad at the site of the accident waving down paramedics to lead them to me and also looking at him while I was being loaded into the ambulance. The crazy part of that story was my dad passed away 2 years ago. I do not recall anything else from that accident but per the police report, I landed 200 feet away from the accident and sustained a torn brachial plexus causing paralysis to my right arm, fractured both shoulders, fractured 3 spots in my neck and 3 spots in my back, right femur fracture, left compound femur fracture, both knee caps, right tibia, right ankle, L elbow/wrist, compound fracture of right collar bone, fractured pelvis, and torn colon. Thank God I was wearing my helmet as I did not sustain any head or brain injuries. I was admitted to Chandler Regional and spent 28 days in the ICU, with my first week in an induced coma and then 1 month in their step down. I went through over 20 hours of surgeries to correct all my fractures, save my limbs and fix my colon. I originally did not want to go to a skilled nursing facility or any rehab because I was scared. I was scared of being alone and being dependent on strangers to take care of me. So I decided to transfer home to my second floor apartment with around 20+ stairs to climb. The paramedics brought me up the stairs on the gurney. At this time, I still had all my skin grafts, staples/stitches, and was completely bed bound. I still had multiple appointments a week where transport was not showing up or did not have the equipment to get me out of my apartment. I ended up getting an infection in my left knee due to missing my appointments which resulted in me being readmitted to the hospital. I was at home for one month, depressed being bed bound and was not doing any therapy as I was non-weight bearing on all my limbs. I also felt like a burden on my wife and our 7 kids, ages 6 months to 14 years old. I felt bad seeing my kids and wife getting burned out taking care of me so I decided to discharge from the hospital to a skilled nursing facility for rehab and continued care. My first day at Montecito I was scared, alone, and did not know what to expect for my day to day. The first few days, I felt uncomfortable like I was living in somebody else's home. That changed quickly as I started meeting the Montecito team. Everybody from the CNAs, nurses, therapy and activity department all seemed like a big family and helped me feel more comfortable. I was quickly evaluated by therapy within the first few days of being here. I did not know what to expect but they made me feel confident that even though I had a long road ahead of me, I was going to recover. The therapy team is one massive family of over 25 therapists and the most supportive people I have ever met. Chris (PTA) and Cassie (OTA) were my main therapists during my recovery. When I first started therapy, I still could not move any of my limbs, had excruciating pain in all my joints and was still non weight bearing in all my limbs. I never believed Chris when he would tell me everyday that he would get me walking again. But after daily therapy for 3 months, I am now walking over 1000 ft with a cane, independently standing and able to take care of myself. After 3 months of therapy at Montecito, I am finally discharging back home to my wife and kids. The staff really helped me mentally, physically and emotionally through my whole recovery. I am planning on continuing outpatient therapy with Chris and the Montecito team in order to completely return to my prior level of function. I wanted to thank Montecito for everything. I am so thankful I chose Montecito to do my rehab. I have met some great people and made some lifelong friends. I recommend this therapy for anybody looking to get better!

My mom stayed her for a couple weeks after being trans from acute care. My mom says: "The care was excellent and the food was mediocre. Physical therapy were very patient." She liked it. Although we had some issues in discharging my mom, they were resolved. They helpful when we needed anything. Also my mom really liked that they did the follow up survey.

I am the MPoA of a patient who underwent rehabilitation at Montecito in Mesa, AZ. It has been close to a year since we were there and if those who were employed there then are there to this day, I would recommend this facility to anyone. The front staff were always friendly and courteous. The administration went out of there way to accommodate us with our special needs. The nursing staff is all around kind, knowledgeable, and strict when they need to be. The Physical Therapists were able to work with this patient, who could not walk when he was first admitted, until he was able to walk and perform his usual routine with assistance at home. They have in-house dialysis which was convenient for us.
