I love the people who've been here, and Im only leaving to be closer to my daughter in the other facility. Some of the people here have really touched my heart and I'm going to miss them. The staff here are so great (the food could have been better) but the experience has been top of the line and I hope my next facility is just as good. I have nothing bad to say about it here. Being able to see my mother so happy and content made me realize I made the right decision! It was a pleasure working with Kyle throughout the admissions process. Despite the fact that he is my saint from above, he has been extremely helpful when it comes to applying for Medicaid benefits and is well-versed in explaining all things for admissions application, services, and amenities. When mom moved in, Nate in Guest Service greeted us, and walked us through everything we needed to get settled. Michael from housekeeping and Erick maintenance guy ensured the room was in well working order. The staff has earned my full trust and I know mom is in good hands. One of the best aspects, I think, this nursing home is that they have tenured staff and the same nurses and same CNAs taking care of mom daily. I would recommend Avantara whenever someone asks for a quality nursing home recommendation. Very glad we found this amazing place to call her home. To all the dedicated staff: from the bottom of my heart, thank you much for being caring, supportive, and loving.

My Dad came here for approximately two weeks to regain his strength after suffering two falls and being hospitalized. He improved a lot during that time, and had a very good experience with all of the therapists including physical, speech and occupational. In fact, all of the staff was wonderful, making us and him feel supported and important. His only complaint was the taste of the food but to be fair, he had to be on a pureed diet due to esophagus issues. Overall a very good experience and would recommend to others without hesitation.

Avantara in Lake Zurich is a hidden precious gem. My sons did lengthy research on placement for me and we found Avantara because due to proximity and central location. I've always love Lake Zurich as a community and I'm blessed to have had the opportunity to complete my physician and occupational therapy here. From the moment of checking in, I was warmly welcomed by Kyle (guest services director) who throughly answered all my questions and what to expect. I never had such a dedicated team of caregivers. Melanie, Taylor, Maddie, and Kent in therapy were super! Nurse Patti and CNA Lexi we're terrific and tucked my in at nights. I would choose Avantara if I have to come back. They earned my highest recommendations.

Avantara will always be my first choice in skilled nursing rehab physical and occupational therapy. After searching many options in the Lake Zurich area, it is close to home and easy on my family to come visit me. I'm a returning resident and have been at Avantara four times and for good reason. I LOVE it and my experience has got a lot better since my first visit back when I had my knee repaired. The facility is first-rate in all areas such as nursing care, dietary and therapy. They customize my food to my liking and I enjoy choosing my menus on what I want to eat. I like the the chefs salad, chicken salad, and they give large portion sizes based on my preference. The food is least bit like the hospital food. I say that the Nursing team is top notch! RNs Patti R., and Mari Rose; CNAs Lexi, Kimmi, and Chabinga; PT/OT Katya M., Ken B., Taylor C., and Margaret. The facility is private, beautiful, clean, and has ongoing variety of high quality services and activities for residents. The staff cares about each individual and provides both the expert nursing and aide interaction that is personal and caring. Lastly, most definitely Pat W. who gave me a surprise baked potato from kitchen and Kyle R. guest services, who is a sweetheart guy, highly professional, personable, and a delight. He takes care of any concerns I have and answers all my questions. Avantara earned my highest recommendation and I hope you will choose them too!

As a bariatric sized patient, my options for facilities was limited. Under the advice of other medical professionals in the area, Avantara in Lake Zurich was recommended to me highly! Upon arrival, there were a couple of small hiccups that were quickly remedied, and that means a lot to me! The staff here is very attentive, and there are quite a few that have stood out during my 3 week stay. The administrative staff including Kyle R (Director of Guest Services) & Christina S (Social Services) were both very helpful in getting me settled and in. Alvin M (Registered Dietician) made sure my allergies were avoided but he also took the time to know what foods I liked or disliked. I was pleased to be given a list of alternate food items to choose from when I did not like what was on the menu of the day. The kitchen staff and CNA team did a great job of making sure my food came to my room hot and was very presentable. While at Avantara, I received both occupational therapy and physical therapy. I will call out a couple that I worked closely with, but the entire team is well oiled and their manager Jerome A (Director of Rehab) is quick to jump in and assist his team and patients as needed. Melanie W (PTA) pushed me to my limits in a good way and helped me regain strength that I didn't know was possible. I feel more steady than I have in years. Taylor C (OT) and Indu S (OT) both worked with me to strengthen my non-injured hand, and also to do the minimal Therapy I was able to do with my injured hand. They also provided me with exercises to continue at home. The entire rehab department is amazing! It is seriously one of the best I have seen as an outsider or an insider in any care facility! Now for the nursing staff! This team works hard every day to make sure all patients are cared for and respected. I will say my favorite nurse was Patti R (RN) and David J (LPN) was a very close 2nd! Nadia G (Wound nurse -LPN) was quick to respond when I had a skin concern. She was able to inform the doctor and get medication needed to quickly treat it before it became a bigger problem. My favorite CNA was Jennifer S (CNA). Jennifer was the most attentive to my needs. She was proactive, always smiling and diligent about all of her patients. Chibinga H (CNA) was also very good at caring for me. I was a little unsure of having a male CNA at first, but his professionalism was outstanding and quickly erased my fears. Lynda B (CNA) took great care of me on the night shift. I'm a night owl so that was not always an easy task. Liz (CNA) was so patient on one of my toughest days. Earl V (CNA) was cheerful and encouraging! When you are placed into a care facility, you don't realize how many people work behind the scenes for your care. When I first arrived maintenance was quick to make sure I had a wheelchair that would fit someone of my size. Erik B (Maintenance Director) solved my problem, and did it with care! Also, behind the scenes is the housekeeping team! Every single person on this team was amazing! Maria S would always enter my room with a huge smile on her face and make cleaning my room feel like her favorite task of the day. Although my Spanish is very broken, because I last used it in high school, we laughed about my Spanish and her English. Alejandra R was also always smiling as she entered my room and took extra care to put my things away neatly. Mercedes B (Laundry Aide) always came to my room and made sure all my dirty laundry was in the bag before she took it to launder it. My clothing always came back! Zero pieces of laundry were lost! All of my laundry was either put on hangers or folded, extremely neatly, and with care. She always smiled and wished me a good day! I would also like to say that the staff was very flexible with my visitors...no one ever felt rushed to leave and were treated amazing. If you or a loved one are in need of a skilled nursing facility, I highly recommend Avantara!

Following a stroke, my mother was admitted to the Rehab center. She had very little mobility, so she required a lot of help with her daily activities. All staff are very kind and really cared about my mother's well being. Special thanks to Kyle for the excellent customer service and for helping us with the admission and coordinating all the other teams of nurses, physical therapists, nutritionist and social workers. Thank you to all staff that helped keep my mother clean and went way and beyond to feed her and keep her comfortable.

Jessica in admissions made the process so easy and smooth. The staff here is very caring and compassionate including CNA Chibinga, CNA Jennifer, and Taylor in PT. Alvin the Dietitian was very accommodating to me for my alternative meal items which I was pleased with. I had superior care which helped me to heal and get back home stronger.

My father was at avantara in lake Zurich for about a month. I can't tell you what a wonderful experience he had there! He loved the nurses there and all the staff! I give them a five star rating for all the great care they gave him, his favorite part was being called grandpa by the nurses.. He said patti was his favorite nurse that gave him a smile everyday and great care. The nurses always took time out to talk with him and ask if he needed anything. Definitely would recommend this nursing facility to anyone in need of help and great care! five thumbs up!

My mother, Aniela, chose Avantara Lake Zurich for short-term stay following 6-days in hospital. Before she went to hospital she was independent, walking and lived alone. Disease took her strength away and she wasn't able to take care of her self anymore, walking was a huge problem. She was very unsteady and unable to walk more than few steps with walker. Avantara's Three weeks of therapy program put her back on her feet. My mom does not speak English and we were blown away that this facility has POLISH SPEAKING nurses and CNAs!! Kyle from guest relations was key in providing translation cards for English/Polish, which made communication smooth and transparent. I'd like to also recognize the clinical team, CNAs Kaitlyn, Chibinga, Kimmi, and absolutely adored Nurse PATTI — she is the best nurse ever!!

My mom has been at Avantara in their "penthouse' on the third floor since the end of the summer. Kyle welcomed and helped mom start to get acclimated. Michael helped move mom's dresser in, Eric helped set the TV up (the smart TV was too smart for me!). I appreciate that they listen to me about her care, ideas for making it easier for mom to get used to the new place and new routines. I was really grateful to Maureen, during this stressful time, asking me how I was and checking on me the next time I saw her. They have so many activities Donna, Jenny, and Oma plan activities for every day and take care that everyone who wants to participate, can. I love that I can take my mom regularly to do things with me. The RNs and CNAs are caring, kind and really good at what they do. Dottie is always so helpful and asks me questions to care better for my mom. CNA Mary listens and trys my ideas, Melissa talks with me and let's me know how things are going for mom. Lito is always so fun and helpful, when I ask him so many things. Lily was really great when I came early to get mom. I really appreciate Comfort's advice and suggestions. Earl and Alexis have been so helpful in finding missing items. I watch them all care for the people who can't take care of themselves with high level of kindness and compassion. I like that I can continue taking mom to her doctor, but they also have doctors and specialists available. They are flexible with what I ask. They do a really good job with the food, it smells good, looks really appetizing, even the pureed food looks thick and yummy. I must say Danielle has been pretty amazing since day one. She is a true joy and unceasingly helpful, no matter what! I hope that she continues to help the third floor out, I see the residents really respond well to her. The front desk crew: Robin, Caroline, Patrick, and Sylvia are wonderful Sylvia is helping me practice Spanish, Robin tells me wonderful stories, Caroline has given me great local ideas, and Patrick always listens. Definitely a friendly, warm, supportive environment. My mom needed more care and this is the perfect place.

I enjoyed my week at Avantara Lake Zurich for my short-term rehab. It was very productive and I was able to regain my strength and confidence. I got great customer service and healthcare at this facility. Also, Kyle guest services director assisted me in getting my Wall Street Journal and Chicago Tribune newspapers. If that's not superb customer service I don't know what is. I can now get back to living my life and will do better now I've learned that I can walk and do better eating better.
