I have worked at Table Rock for a little over a year now and I have to say that our team is by far the best. I have worked in this field for 18 years and have never had worked with a Team so dedicated to their residents. We have faced our share of challenges, but our staff show up not only for each other but for our residents. They put their heart in soul into the care of your loved ones, they strive to always keep a smile on their faces and to ensure their needs are met. The bonds we share with our residents enrich our lives and theirs.

The staff at Table Rock is dedicated to the residents. It's obvious they love their jobs. The residents are attached to the staff and it's touching to see the bonds that form. This is a very nice facility and I enjoy being a part of the community as therapy staff. I was originally placed in this facility on a temporary basis, I chose to stay permanently because it's a wonderful environment. I'm saddened to see reviews left by former employees. I have not seen any of the negative issues mentioned in the negative reviews. Management is quick to address concerns. They have a high expectation for the standard of care. I would definitely not turn a blind eye if I saw problems. The staff has always been quick to get the residents what they need when I have brought up concerns. Whether it be following up with a medical issue, or helping the residents with needs or care, they take care of what needs to be done. Please consider a tour if you are considering memory care for your loved one. P.S. There is no raffle or reward for leaving a review. This is my true personal experience. I love working with the memory care population and I want the community to know this is a safe and special place for those who need it.

It was very hard giving up the care of our mom, but after checking many Memory Care options in the Rogue Valley and getting the input from my daughter (a Home Health Care Nurse, who said they over achieved!) We are very pleased to be a part of Table Rock! They keep Mom healthy, happy, and engaged! Tyler was very knowledgeable and patient, walking us through all questions and steps.

I am a consultant tasked with maintaining the community's compliance and focusing on continued education. Like every industry today, as healthcare providers, we face challenges. Frontier attempts to mitigate these challenges by offering above average, starting pay, continuing education for all staff members and morale boosting bonuses for the care team that will pick up that extra shift and offer their personal time to your loved one . Working at an 84 bed memory care presents it's on set of unique challenges. My hats off to the care staff who commit to assisting these memory care residents. The care team have the most demanding job and put their heart and soul into your loved one. They are the faces the residents see daily. They are their confidant. They are their family. They earn the residents trust and that is not an easy task. This memory care staff have made strides in the past year to bring the community into compliance and adhere to Frontier standards. Kudos to the management and care team that turned Table Rock into a safe, loving place to live. You are recognized for what you do.

They take good care of the residents in the home and the nursing staff is really good. Now the bad.... My mom died there on 2/2/22 and Table Rock pulled twice for ~$1800 totaling over $3600 for 02/01/22 month payment. On 2/8/22 after finding out we have no money (over drawn by quite a bit), I found that they double pulled for February even though mom is dead. I called them and Christina told me "Sir we owe you nothing!". After arguing with her he looked at the ledger (why wasn't she doing that from the beginning). She saw that yes they pulled twice for February, and that shouldn't have happen since mom died on the second. Fast forward over a month and talking with other people there, and I'm still heavy in the negative and accruing overdraft fees at my bank. I have not received any money and everyone we talked to there says yes we owe you and the check will be in the mail... for over a month now. Might seek legal advice shortly

I have to put you two cents in here. I have worked for numerous LTC facilities in S.O. over the years and while it's true, Table Rock is short staffed, I challenge anyone to find any care facility that is not. But even short staffed, the caregivers in this facility bust their butts to meet their residents needs up to and including pulling extra shifts on their days off or working 16 hours straight. They know their residents as individuals and treat them with love and respect. There have been those couple of caregivers who slack or don't involve themselves in giving quality care but they no longer work here. I know that as Fern Gardens this place did not do well but the new management and the administration are making a real difference and they only took over 9 months ago. It is a beautiful facility with good, caring people who's only detriment is there aren't enough caregivers to hire that are willing to work hard and be a part of this team. Those that work here are quality and I am proud to be working alongside them.

Beautiful facility. Tony is very polite and respectful. We enjoyed the tour and will keep Table Rock in mind when we decide to move mom into memory care. The care staff was very nice and caring. We loved hearing about the activities and the residents look like they are enjoying themselves.

I cannot say enough positive things about Table Rock Memory Care and the entire staff!!! To start, the intake process and home evaluation was professional and efficient. Upon my dads first day at the facility, the staff was as helpful and polite as could possibly be. Not only to my dad but my entire family. The caregivers, Med tech's and nurses were all friendly, skilled at their jobs and above all, caring toward my dad and again our entire family. We couldn't have picked a better facility! If you're looking for perfections, I don't think you will ever find it. No one can care for your loved one better than you can at home. But when the care level exceeds what you can handle at home, Table Rock Memory Care is an excellent place for your loved one to transition to. A Big Thank You to the entire staff for all of your hard work, care and love you give each and every day. Not only to my dad, but each and every resident at the facility. You are all incredible! Our whole family is eternally grateful to each and everyone of you. You are Greatly Appreciated!!!

Very neglectful , treat residents badly, 'inefficient meals. I can go on , They just did change hands and I pray things will change. I suggest spending a lot of time visiting to see for yourself

So far, so amazing. Table Rock has excellent staff compared to other facilities our family has lived with. The communication and staff are fantastic with helping out families the best they can. My mom needed to move from Portland to Medford and the staff was so accommodating to her needs. The beds, rooms, and facility structures are much cleaner than the past facility my mom was living at. I couldn't be more happy to know my mother is in good hands! From previous posters, I think the facility is under new management so hope all is going better!

My family just recently decided it was time for my mother to move to an assisted living facility. So, I went on a search for the best fit for my Mom and that is a tiring and scary search. After touring several facilities in Medford, Oregon we decided that Fern Gardens would be the perfect choice for her at this time. It's a very homey environment with wonderful staff and a very cozy feel. A BIG "Thank-you" to Joe Lords who is the Marketing Director for not being just a "salesman" but a very compassionate, caring person and helping us through this difficult transition. My mother moves in next week so I'll be reviewing again after she has lived there a few months. My hope is that they are as great as they appear and can hold up their end of the bargain. That is taking the very best care my Mother because she's an amazing women and deserves the best. Stay tuned.

I have had friends and family at Fern Gardens for some time now. They had some issues in the past with the State because of prior management. They now have a new management company called Avamere Healthcare running the facility and have made and are making many changes. I am very impressed with the new operations and how well people are cared for. I think the staff just needed some better direction. When it comes to caring for dementia residents, any facility can be knit-picked as it is a very hard job to do. I believe the team Avamere Healthcare has assembled & trained at Fern Gardens is a good choice to provide care for anyone who suffers from dementia and needs excellent Memory Care services.
