I had a wonderful experience visiting this skilled nursing facility. The building was clean, the residents appeared well cared for, and the staff were welcoming and attentive. What stood out most was my interaction with Alexus in Admissions. She was extremely helpful throughout the entire visit and made sure I had all the information I needed. Alexus was professional, approachable, and took the time to answer my questions in a way that was easy to understand. The tour was thorough, and she did an excellent job explaining the programs, services, and level of care available.

I had an excellent experience touring this skilled nursing facility. From the moment I walked in, everyone was welcoming and professional, and the facility was clean, well-maintained, and had a warm, caring atmosphere. A special thank you to Alexus in Admissions. She was incredibly knowledgeable, patient, and compassionate throughout the entire process. She took the time to answer all of my questions, thoroughly explained the services offered, and made sure I felt informed and comfortable every step of the way. The tour was detailed and gave me confidence in the quality of care and the genuine dedication of the staff.

Choosing a skilled nursing facility can be a difficult decision, but my visit here gave me a great sense of comfort. The facility was clean, the staff were friendly, and the environment felt welcoming and well cared for. I want to give a special shout-out to Alexus in Admissions. She made the entire experience exceptional. She was professional, knowledgeable, and genuinely took the time to understand my concerns. The tour was informative, and she explained everything clearly, from the services offered to what families can expect during the admission process. My mom’s last year was here at the Broadmoor. From the first day they treated her like a friend and family member. They truly cared for her and were patient and loving. She passed away over the weekend and I am astounded by the impact she had on her caregivers. Everyone is exceptional and my family is grateful she spent her last year of life here with these people. Thank you all.

Most of the staff deserve 5 stars. They worked hard and seemed to care. But, it only takes a couple (who apparently have forgotten that they are dealing with old, sick people) to ruin the whole experience. If a family member is willing to stay there most of the time then, it’ll be fine.

Calling this place is absolutely ridiculous. I called three times this morning, holding on each call for almost 20 minutes. Asked to be transferred to the nurses station when I finally got though. It rang and rang and then disconnected. I’m holding on another call as we speak.

One of the worst experiences I have had. Nursing Aids would walk in my room to provide care and be talking on their cell phones via their Air Pods. One of the Aids actually walked in my room talking on her cell by holding her cell phone to her left ear with her shoulder. So not only rude but how can good patient care be given while talking on their cell phones. I couldn’t stand or walk without assistance. One Aid was so mad that she had to come and change me. She drug the wheelchair over to my bed and said Mr. Woods here i is a the wheelchair for you to get up and get yourself to the bathroom! I said that I am unable to walk or stand without assistance. Her reply was I only work part time and PT didn’t tell me. Obviously there needs to be communication between the shifts. A counselor/ therapist came in and asked me if she could ask me a few questions and I agreed. About 10 minutes into the session she said you aren’t a woman Woods are you? I said no I haven’t had gender reassignment surgery. She walked next door and found the woman Woods she said I’ve been doing that a lot lately.’ I thought WOW that is very concerning. That same counselor came back a few days later and asked if she could finish asking me questions, I told her to leave. I have a lot more to say but may run out of space. The PT and OT were so kind and all of them were encouraging all of us even though they weren’t working with each of us. They offered to help each other, they displayed great teamwork and a very cohesive team in comparison to the nursing staff who truly do not understand by working as a cohesive team that good patient care can be provided. Now T and Gemayel had only recently been hired and placed in charge of the nursing staff. They have made some positive changes, but they have a along road to travel down to having a successful staff. I truly wish T and Gemayel good luck. The Housekeeping staff was so friendly, helpful and worked well as a team by offering to help each other. As you can see I could only give this facility a 2. If you place a family member or friend at this facility please check on them on a regular basis. You may need to be their advocate while at this facility. The reason that I only addressed the woman that the counselor confused me with.as Woman Woods because I didn’t have her permission to list her name.

I was in the Broadmoor at Creekside for 2 weeks for rehab. Abby was great in housekeeping. The nurses were Nisha, Jennifer, Erica and Aya. They were excellent!!! They have a great Physical Therapy team!! I wouldn't want to live there but for the PT activities it was ok. The food wasn't that good.

Grounds and physical facility is excellent however a service provider should be judged on their people and service provided. As a business manager for twenty-five years I don’t tolerate incompetence or poor service. Therefore I will grade jobs/functions separately. 1. Therapists A. 2. Food B. 3. Call button response F. 4. Day shift nurses. C- 5. Night nurses F. 6. Housekeeping B+. 7. Staff concern for patient hygiene F. 8. The staff are friendly, helpful, and caring. My mother’s RN Zanata was extremely caring and knowledgeable. She assisted in my mother receiving her correct medications (long list), modify them as needed, and obtaining tests to help her recover.

Very disappointed in this facility. At $350 a day you would expect better service. It’s not that the staff is bad but they are severely understaffed. Patients are continually crying out for help many times waiting an hour or longer for staff to get to them when they have soiled their diaper or bed. These patients are treated very unsanitary and disrespectful because of the storage on staff. This facility needs more staff to cover the patients needs. And when you call sometimes it’s 45 minutes before they even answer the phone. I would hate for a loved one to have to endure this treatment when they are dependent on a staff that can’t provide decent care for the quantity of patients it takes in.

I was here for rehabilitation 1/20/26-1/7/26. Every member of your staff was so cordial empathetic and sympathetic. Everyone took very good care of me, explained all procedures to me and made sure I understood before they proceeded with procedures. The physical therapy department was absolutely wonderful. I made great progress with their help towards being able to care for myself when I get home. The food was good. My favorite meal was the fried fish. The RN was very prompt in giving me my pain medication the doctor was very understanding and I appreciate that. I will recommend your facility to all who need you. Thank you so much for your accommodations and staff. I had a very pleasant experience here

My dad was here for a couple of weeks, and the care he received was far below acceptable. If you don’t have a family member who can be there with your loved one constantly, I would not recommend this facility. My father was left in soiled diapers for long periods, and over his entire stay, he only received one shower”despite the policy of two showers per week. It took my mother raising her voice to even get that done. The physical therapy team was somewhat more attentive, but even they left him slumped over in his wheelchair afterward, without any assistance until my mother returned. My father is paralyzed on his right (dominant) side, and we made it very clear that his food would need to be cut for him. Still, this basic need was often ignored. This experience was heartbreaking and exhausting for our entire family. If you’re looking for a place that will care for your loved one with dignity and attention, I cannot recommend this facility.

We have been so impressed with this facility for my mother. The therapists are great and really support and work on functional goals to regain independence. The facility is beautiful and very clean. Any concerns are addressed promptly. Staff are caring and compassionate. I would highly recommend this facility.

I stayed at the Broadmoor for almost four weeks and it was one of the best stays in a medical facility I've ever had. I'm 27 so I was one of the youngest people there other than the nurses but it was still such a lovely experience. The nurses, PT, and caretakers were all so kind and patient with me. Staying there I made so many friends and I'm going to miss them all so much. From day staff to night staff, everyone was just amazing. My lunch and dinner was really yummy and when my dietary restrictions were a problem(not a problem, bad word but I can't think of the right one) they did work with me on trying to figure out something I could eat. Overall amazing stay and healing to my medical trauma.

Broadmore at Creekside Park is not the Plaza on Central Park. Some of the equipment is outdated and the nurse’s assistants need some extra training on customer care. That said, most of the nurses are on top of there game. Zaneta is by far the best nurse in the building. She is on time with my meds and checks in to make sure I’m ok. The Physical Therapy team is amazing. I have worked with Brian and Ashton who are both amazing!! They definitely make sure you are taken care of and are comfortable. They say the best time to eat crow is when it is still warm. I feel compelled to share my experience in the past few days at the Broadmoor. When I first came to the Broadmoor I was not impressed and just wanted to go home. Since my arrival I have been impressed with the people and the level of care by so many people. People like Dr. Gucci, Jorah, Rhonda, Crystal and so many others that I can’t remember their names. The nurses and assistants are so busy but find time to make me comfortable. Thanks to all the people on the 2nd floor at the Broadmoor for your first class service!!!

This facility has a remarkable care team and a beautiful setting. When my mother was admitted in December, my sister and I were apprehensive due to a previous negative experience. However, the new executive director, Angela McArthur, and the entire Broadmoor team of financial, clinical, physical therapy, nursing staff , aides, social work, and housekeeping professionals have been exceptional. My mother has received exceptional care, and the team has collaborated effectively to ensure her success. They have not only provided care for my mother but have also kept my sister and me informed and involved throughout the process. My mother has been treated with utmost dignity and respect. Cindy Boyd

The therapy at this facility is wonderful! My foremost concern was trying to get pain medication in a timely manner...particularly at night. I would suggest always having family members available to help monitor any concerns, especially with an aging parent. I did appreciate that executive staff post phone numbers in the patient rooms if there are any issues to be dwelt with and seemed concerned and willing to listen. Most of the nurses/aids were nice & caring. The facility was clean and the grounds were well taken care of. The food was tasteful for a nursing/rehab facility. I also liked that the rooms are private and offer a microwave and small fridge.

Once again I find myself back at the Broadmoor. And once again I am very satisfied with my care here. The Physical Therapy Dept is always on point, they never let me down. The nurses I had had he as been wonderful, always checks on me and make sure all my needs are meet. Want to send a very heart ful Thank you to Mario the head of the nurses department. He has always went above and beyond to help me when ever I needed help with anything. Being a patient here you observe a lot of things. And I see the way he interacts with his staff which I think is wonderful. With the problems I have had here he had always been able to fix them. Cantex need more DON's like him. He is good at what he does I am a returning patient of Broadmoor. I have truly loved the care I get from the Medical staff as well as the Office staff. Here is a list of some of my personal best I would like to recognize for doing a exceptional job. Absolutely gorgeous facility, and as with all places there are many amazing people and many less than amazing people - one of the latter being the director of nursing, Mario. I spoke with him and two nurses mid February to offer some assistance in dealing with my bed-bound mother, who demands a lot of their assistance, for their benefit as well as my mother’s. Needless to say it fell upon deaf ears. I am still awaiting a call back, from Mario, since mid-Februaryeven after calling him on his cell, while on vacation, which he assured me he would call me upon his return. Well, I’m still waiting. Yesterday, I received a VM asking if I knew where my bed-bound mother was???? No. She had called 911, that she had to call herself, and went to the hospital for chest pain, yet the facility did not know where she was taken. Since this is a dangerous situation, I have contacted the management company, CANTEX. Hopefully, they will make some necessary changes. Bottom line, the facility has the potential to be a great one, under new leadership, with someone that possesses integrity. Addendum: Andy P., the administrator, contacted me and he promptly addressed all of my needs. Maybe the team will step up now. 07/21: It’s been 3 weeks since a response was sent to my review and I STILL have not heard from anyone. I left a VM for the Ombudsman on 06/21, and have yet to receive a call back - this is really shocking and SCARY as this is the office that addresses patient issues/ complaints. All of the staff I have encountered seem to care; the apparent issue is with leadership and their inability to respond to family members of their patients. Again, I started contacting Mario, the director of nursing, back in February. Not sure how he can accept a paycheck or sleep at night. When I communicated with Andy P, the administrator, yesterday to let him know I still hadn’t heard from mom’s new doctor (as I’m confident Andy instructed him to call me), and he said he would follow up.again, not sure why communication is such a challenge for this facility. And btw, I did contact CANTEX & you guessed it - no response. 08/14: I am very happy to revise my previous star rating from a 2 to a 1 to a 5!!! Since my last review, Andy, the Administrator, made a doctor change and I am very pleased with her. Also, Mario, Dir of Nursing, finally called me back, apologized for dropping the ball and since then has been accessible to me & my family. The team really stepped up and I am very appreciative and confident she is receiving the care she needs during her decline.
