As a discharge planner I give them 1* because they are ready to admit a patient without even meeting them. the resident was so paranoid of losing a bed in the AFH. discharge is happening today. A good AFH will not hasten a discharge to secure a bed. they have not talked to the facility or met the resident. they have not helped the family or resident set up pharmacy account for medication refill.
