My dad has been a resident at Discovery Commons for 2 1/2 years. It is a facility that he has truly thrived at. It is never easy to change your loved ones home. Every facility is different to what they can provide. LeAnne assisted us with making sure he was a good fit, and helped him get settles. What I truly appreciate is that even after all this time she has never stopped checking in on him, making sure he is happy, and being a great support. She always has a smile, and time to speak to the residents and share her joy. They have a new chef, Eric, and he has provided fantastic meals, a variety of foods, and an enjoyable eating experience. It can never be easy to please so many different people and preferences, but he does a great job. The activities are endless! They vary from Bingo, exercise class, speakers, singers, crafts, etc to the many outings including to restaurants, shows, and movies. Their activity director, Anna, is fabulous! She is so genuine, has many hugs to give out, a smile on her face, and truly loves her job. One of their physical therapist, Patricia, has been a constant presence since my dad has been there. She knows the residents whether she works with them or not, and adds to the overall positive experience of being there. She truly cares about the people maximizing their independence, and wants what is best for them. Finding caregivers in this type of setting can be very difficult. It is a demanding job, and not everyone is a good fit. I appreciate the management helping those that do not fit their vision to work elsewhere. It is so important to know that your loved one is treated with respect, kindness, and general care while there. He has a few people that have been there since he started, and truly views them as friends. There are too many great CNA staff members to mention them all by name, but I want to give my heartfelt thanks to all of them! I appreciate you providing my Dad compassion, care, respect and a lot of humor!

Moved our 90 yr old family member here from Sunset Woods in New Berlin, what a difference! He describes the meals as fine dining and the staff is incredible! LeiAn did a wonderful job at introducing us to the facility and made the transition so easy. Katie is the executive director and we can see how much she cares and values families and residents here. The place is clean and the staff are all happy to help. It’s been three weeks since the move and we couldn’t be more pleased, looking forward to visiting for Greek night tomorrow!

From start to finish, the staff was caring, well-trained, respectful, and incredibly responsive to his rapidly changing needs up until his death. I cannot say enough great things about the team at Discovery Commons Brookfield. The value placed on a quality lifestyle is top-notch. From their beautiful apartments, to cares, to the dining experience, and to their robust activities program, it is all geared around creating the best lifestyle for our seniors who have physical and cognitive limitations. Their residents are family. No place is perfect and every resident/family is different. There will be an "adjustment period" for everyone...the community, resident and family alike. It is not an easy transition. The difference at Discovery Commons Brookfield is how the team works with the residents/families to immediately address issues or concerns to provide the best outcomes for everyone involved. That is what makes this team exceptional.

Discovery Commons (Brookfield) was one of the many communities we looked at to find assisted living for my mother-in-law. In the end she did not choose this community. I am not here to review the facility, though we liked it very much when we toured. I am submitting this review to discuss the wonderful experience we had with the amazing Director of Sales (LeiAn). The way she connected with us as a family was crucial for us finding the "Best" community for our families needs. We had several face to face sit downs and there was much laughter, tears shed and a few good hugs, as LeiAn truly understands the process. She was here to guide us through all this and not "sell" the Discovery Commons community. To find the best community for those we care for can be challenging. I encourage you to look at many options, but I would start here first. Sit down and talk to LeiAn, tour the community, and it will give you a much better perspective on how to find the best community for your loved ones.

I am writing this review on behalf of my family. The review from Carol was unwarranted, unwanted and completely inaccurate. My father is the person she was brought into to help out with. And was only supposed to be there for him. She helped him for 12 years. Before I address her horrible review. I would like to talk about Discovery Commons of Brookfield. Katie the Executive Director has always been there if we had any concerns. She has been a great resource when we needed her. The facility is nice and clean. I can’t thank them enough for taking care of our father. We asked him if he wanted to leave and go somewhere else. He declined because he likes it there. And that’s what is important to us as his executors and children. If you are looking for a new home for your loved ones, I would absolutely recommend Discovery Commons. Now as far as Carol she was caring towards my father. And unfortunately that’s where this compliment ends. Her persistent inability to follow rules almost had him removed from his last residency at another Assisted Living facility. I have pleaded with her to just attend to my father’s needs. She has had problems with every Director before Katie. There is a pattern here. From accessing restricted areas and using the facilities property as if she was an employee. To spreading gossip and causing problems among their staff and administrators. She was brought in there by us to just take care of my dad as stated before. Come to find out that now she is taking care of other people. We discussed this and I told her that’s not what she’s there for. Unfortunately, I will never know if we were being billed correctly for the time that each one of the people she was taken care of and not being double or triple billed. We have cameras in our dads place. I’ve seen her with a small toddler and a small dog unleashed running around my fathers place. This is while she’s supposed to be taking care of him. I’ve seen him sleeping in his chair while she is on her cell phone doing who knows what. I would not be writing this today if she would just did what we asked her to do. I have also asked her to please take down her negative post. This will be for my father sake and for ours. Unfortunately she cares more about herself than she does about our feelings and our father. I don’t want anybody to think that we have our father in a bad place. Because we don’t. I was the one that had let her go because she was causing too many problems there. Then I spoke with Katie and she agreed to unfortunately have her removed from the facility. She is a disgruntle employee and is lashing out. Again, all this unfortunate circumstances are happening because of Carols actions. I am really saddened to write this and air this out in public. But I cannot have anyone thinking that we have our father in a horrible place as she described. Completely not true. Ever since she’s been gone, I have looked in on my father and so has my family. I do not see any changes in his healthcare since she has left. Maybe this is all for the best. And again I am sorry that I am airing this out here.

Beautiful facility with a handful of caring people. Heartis really struggles with aide staffing and it shows. If your loved one needs a higher level of care, stay away. This is by and large an independent living facility. Any needs above that - they are not able to accommodate in a caring way. Weekend staff especially (with no management around) was a hell trip for my mom. Verbal and physical abuse were evident. Complaints to management fell on deaf ears with no follow-up or were greeted with a very defensive stance. Residents commented that they were scared to complain for fear of the retribution. Sad.

Beautiful facility with a handful of caring people. This really is more an Independent Living facility. If your loved one needs a higher level of care, stay away. Heartis is not staffed properly with the type of aides required to support additional needs in a proper caring manner. No one there wants to be bothered with your loved ones additional needs. The weekend staff in particular (with no management present) can be abusive. Any concerns are met with a very defensive stance from the management team and will go unanswered unless you continue to push for a solution. Some residents are even too scared to complain for fear of retribution. That is an unacceptable environment to be in. My mom spent the last 2 months of her life there and looking back...our family wishes it could have been anywhere else.

My mother was a resident of Heartis Brookfield up until her passing in May. Heartis is a special place that cares deeply for their residents and it shows. The staff was incredibly kind and loving towards my mom in every way possible during her residency there. They are hardworking and compassionate and treated her with respect and dignity, and that was never more obvious than when she entered in hospice there. Our family will forever be grateful for what they did for our mother. Abbey Nickerson, who is the resident care director, deserves special attention for all that she does to ensure the health and physical well being of all the residents there. It's not easy. The business side of assisted living and memory care is credibility challenging and requires a tremendous amount of effort. The current management team in place there does a tremedous job juggling all of the day-to-day complexities of senior living. Are they perfect? Absolutely not. They can't be everything to everybody. And some seniors and their families may not be the best fit there either. It's a two-way street that relies on open and honest communication between all parties involved. In our case, it was the absolute best place for our mom to live out her remaining years in comfort, safety and dignity.

I want to give a shout out to Jeremy in PT, Alexis and Tatianna as caregivers for always being cheerful and making me smile, Dee, Malek, and Shari in the dining room for remembering what I like and making me laugh! If you can’t be at home, this is a nice place to be I also want to give a special thanks to Steve and Abbey in the nursing dept for helping me with insurance and minor medical attention. My mother has been at Heartis since, January 2023. Her care has been phenomenal! Leanne in Marketing was my first contact with Heartis Brookfield. She took time aside from my tour to talk to me and share her personal experience and understanding of the transition for my mother. The Generations Manager Latoya truly LOVES her residents and makes sure ALL caregivers provide excellent care. Latoya knows my moms history and I am updated as soon as something needs to be addressed. Latoya considers the residents as family and provides any cares needed along with her roll as manager. Never hesitation to help! Shows with her heart excellence in all aspects. Hailey a caregiver too has the biggest heart and takes the time to sit and talk with my mom. Has an amazing relationship with her always makes sure she is well taken care of! The caregivers all know me by first name and am always welcomed when I come. I toured 8 memory care facilities prior to coming to Heartis Brookfield. None even compare slightly to the cleanliness, care, and compassion showed at Heartis!! My mom is in amazing hands everyday. Even when she was hospitalized the staff came to see her in the hospital. They go above and beyond not because they have to, but because the support and love is apart of them. My husband has been a resident since May 2023. Prior to coming to Heartis Village, he resided at a different memory care unit for 9 mo. and were so turned off by the care or should I say lack of care. I decided to look for a better facility for my husband. I toured 3 and decided on Heartis. I found the executive administrators to be friendly , kind, empathetic, transparent and very professional. The staff is attentive, very caring, so kind to their residents. All residents are treated with respect. I see hugs and an occasional kiss given to residents. I truly believe that I’ve found the best care for my husband. I couldn’t be happier. I also want to mention the activities people and the amazing job they do to keep the residents active and involved. There is also physical therapists that work with the residents, Patricia is so kind and patient with my husband. The meals are amazing and I’ve seen the chef come out to ask the residents if they enjoyed their meal. And thanks to Maintenance and housekeeping everything is well maintained and clean. I am so pleased and happy that I found Heartis Village for my dear husband of 45 years.

This is true beginning with the receptionist....to the dining personnel.... to the managers. In particular, we are so grateful that we had LeiAn hold our hand from start to finish. She made everything happen quickly and carefully, and she tied all the loose ends for us along the way! My dad has been living at Heartis Brookfield for one year now. We have weathered many changes in staff and management. The corporate office staff has listened and responded to our concerns. They have made changes and because of the positive outcomes we moved our mother in as well. They listened and in time we have seen great improvement in food, care, housekeeping and activities. Maintainance helped us set up hands free TV service as my dad can not use a remote. They even lowered his mailbox as he is in a wheelchair and has no fine motor ability of his hands. He can now see his Mail & remove it himself. All these steps taken to keep him independent means a great deal to him & to our family. Th staff they have in place now listen & care. I needed to find an urgent placement for my father, after many falls, and all sorts of other ailments, there was no way we could safely move him into our house at this point as we had previously planned. Heartis Brookfield was absolutely wonderful in helping us out in getting him here safely from over 300 miles away, and in HIS place last minute when he was discharged early. Everyone was wonderful through the whole thing, despite me not knowing what I was doing, I would like to give Stephanie in particular a call out, as she dealt with my dozens of emails and hundreds of questions in a calm and supportive manner. The facility is very clean, and brand new. It doesn't feel like a care facility but like a luxury apartment complex. I would definitely recommend it to others. This whole thing was hard, but Heartis made it easier. Best of all for me, dad is close by! Whenever he needs something, I can now just drop it off instead of having to coordinate from hundreds of miles away AND I can see my dad shining through the fog again. Now we just need this pandemic to end so we can figure out our new normal...

I recently moved my grandmother to the memory care facility.She was recently diagnosed with Alzheimer's. I was helped the whole way through her admission by Stephanie Ruffi. She let me know what paperwork was all needed since I have never been in the position to move someone into a senior living facility. Now that grandma is in memory care Patricia Fabian sends me weekly email updates on my grandmothers progress. Grandma seems to be happier than she has been in years living here. There are always activities for her to do to keep her mind busy. Every staff member i have met from the front desk to the memory care unit have always acted professional and courteous.

Brand new location in Brookfield. Mom was one of the first residents; and I must say that they are working out the kinks with their services. I am pleased with that. The staff, from Michaela at the front desk to Jerry in maintenance, the two Saras: in activities and in the business office, Stephanie and Christine at the helm, Mila in custodial, and the nursing staff are very friendly, know what they are doing, and get answers for residents and family quickly. Mom has made friends and seems to be kept busy. My family and I have been to two events already, so I know that they care about the families too. Communication is the key to success and they are working hard on that as they work through growing pains. They have assisted living and memory care and the building and apartments are lovely. They have a handicap friendly bus to take residents to appointments and group outings. I have ridden the bus with mom to personal health appointments.Take a tour and get info and see for yourself.

Sara's activities have made my Mom more involved than she has been in years. Sara, in the office, keeps us well informed about business related items. Stephanie was great getting everything in order during different phases. She is taken to her appointments when needed. Jerry takes great care of the building. The housekeeping staff, aides, front desk, and the entire rest of the team are fantastic! Christine addresses each concern immediately and takes action. Kudos to Christine, the executive director, for assembling such a great team.
