Park Vista Health Center provided outstanding care for my mother. My mom has been to Park Vista twice now for over 4 months total. The nurses and CNA's are attentive, kind and committed to their patients' well-being. They were very good with the restrictions she had & keeping up with her medications. Additionally, the physical and occupational therapists were absolutely phenomenal. They were patient, encouraging, and tailored the rehabilitation specifically to her pace, helping her regain her independence. Roger made sure her TV was working each night so that she wouldn't miss a Dodger game. The facility is very well maintained. The building is always clean, bright and comfortable. I spent a lot of time there and felt very welcomed by the staff. We felt at ease when we left her for the night knowing she was being taken care of. Thank you to all the staff for helping in my mothers recovery!!

I stumbled upon this building by chance and asked around the hospital where my friend was recovering. Everyone knew of the building and everyone agreed it’s a top choice for care. I was warned often that they rarely have beds, but if I call and tour, I had a higher chance of getting my friend accepted. They were right. My friend recovered there for a few weeks and throughout her stay she would tell me- she wished she could stay. She enjoyed therapy, food, and nursing. When I would visit, I would see how the staff interacted with others, and it made me feel happy knowing when I leave, my friend was cared for. I would recommend this health site.

I had a really frustrating experience with Park Vista Health Center, mainly with rehab. What I didn’t realize at first is that the therapy isn’t actually run by the facility. It’s handled by a company called Select Rehabilitation that comes in separately, and that disconnect really shows. As a family member, that was confusing because you assume everything is coordinated by the place your loved one is staying at. My mom was taken off therapy because she was told she wasn’t making progress, but I saw other patients in much worse condition still getting therapy for long periods of time with no real change. One patient a few rooms down had been there over a year with no movement or communication and was still on services. That didn’t make sense and raised real questions about how those decisions are being made. The therapy itself felt very basic. Mostly passive range of motion, not much active work, and not much visible effort toward real improvement. The biggest issue was communication. I asked multiple times for clear explanations during care plan family meetings and when I spoke to staff, but I kept getting redirected or given vague answers that never explained why decisions were being made or why therapy was stopped. After looking into Select Rehabilitation, I also saw mentions from other families of concerns and even some legal disputes in situations involving how therapy is handled in facilities they work with. It made our experience feel more familiar, especially given the lack of clear communication we ran into. It also makes me question why a company like Select Rehabilitation would be the contracted therapy provider here when there are other rehab companies with strong reputations. It raises questions about why this setup was chosen if it creates this kind of disconnect in communication and accountability between families, therapists, and the facility. Overall, Park Vista looks like a nice facility when you first walk in, but the rehab experience didn’t match that impression. When recovery is the main reason someone is there, that matters a lot. If you have a family member here, stay very involved, ask a lot of questions, and don’t assume everything is being clearly explained

Staff is friendly and responsive. My family member required suction to manage her secretions after eating some ice chips, and the CNA had to leave to get the LVN to do it. I asked if they had the smaller soft suction catheters and she said no because the LVNs can only use Yankauer, which is too large to adequately reach her secretions. That is unfortunate

Park Vista is one of the nicest skilled nursing communities I have visited. The staff is very kind and caring. Compared to other communities I have visited, this community has fewer residents assigned to each caregiver and so the residents receive prompt response when needed. Park Vista offers many activities to keep their residents engaged and maintain their cognitive function. A friend of mine is in this community and I have seen the Activities Professionals intentionally engage my friend with unique activities that meet her needs, even when she is bed bound. I have regularly seen them go the extra mile to make sure their residents feel valued and appreciated This skilled nursing community is always clean, sanitary and well staffed with caregivers and yet it doesn't feel like a hospital, it feels like a home.

Park Vista Skilled Nursing is one of the best skilled nursing in OC. Their staff all around is incredible, so compassionate, and caring for their patients. From the nursing staff, to the physical therapists, to social services, and even the Maintenence and food. Highly recommend Park vista for a family member in need.

I don’t normally write reviews but I felt compelled to write one today. I was looking for an assisted living home for my grandmother and Park Vista was the perfect option. I got all my questions answered kindly and promptly ” something that you always want to receive while going through this troubling time of putting a loved one into a space that they’d feel comfortable with. My grandma doesn’t require a lot of hands-on care but just having her in a space of care was obviously my number one priority. Thank you to the staff at Park Vista!

A couple of months ago, my father stayed at Park Vista, and overall his experience was mixed. Some members of the nursing staff were genuinely caring and attentive, while others did only the bare minimum, which became noticeable at times. There was one older Filipina LVN whose name I do not remember who often seemed frustrated, uninterested in being there, and bothered by any request for assistance, even when it involved basic needs such as helping my father with a brief change. The therapy manager, a middle-aged woman named Jharudee who appeared to be either Indian or Hispanic, seemed rushed, not very engaged, and often gave up on my father’s therapy sessions fairly quickly. The rest of the physical therapy staff, which included about three members, were generally good and tried to help. Beyond physical therapy, however, the therapy team as a whole was somewhat inconsistent in their support. On the positive side, Sunday, the nursing manager, was compassionate and truly did her best. The occupational therapy team, made up of two middle-aged women, were very personable and great to work with. The Asian male speech therapist was skilled and highly involved, and the younger daytime RNs consistently went above and beyond despite working in a stressful environment. The CNAs were hit or miss, but they seemed to be the busiest staff members, and we appreciated the effort many of them put in. If needed, I would consider having my father return to Park Vista, but only if I could choose the staff assigned to him. Overall, the facility is okay, with both strong and weak points

We visit our neighbor, who has been in Park Vista since December, and have found all the staff we've come in contact with, to be very pleasant and caring towards our friend. They treat him with respect and dignity. The rooms are beautiful and cheerful with lots of activities at different levels for the residents. When ee finish our visit, we feel glad that he is getting good care.

Not having a good experience for mom since arriving here 4 days ago. Incorrect med dosage twice. Tested positive for UTI yesterday (Mother's Day). Had one PT session since arriving - she broke her hip a week ago and had begun PT and OT at the hospital before coming here - and no therapy scheduled for her over the weekend. Several blood draws have occurred with no follow-up regarding results, and no one knows when the doctor will be at the facility. The A/C was not working until yesterday. The room door gradually closes, unless fully open, and maintenance's response was, "What would you like me to do about it?" Once star for the facility's clean and well-appointed aesthetic, decent food, and several caring nurses who have helped mom.

This is the BEST care facility in Orange County. The staff is incredible and always helpful for my mother who has been a resident in skilled nursing and in Assisted Living. We are so grateful and blessed that she has such amazing care. Highly recommend!

After shattering my right leg last May I needed a place to recover and chose Park Vista. I am very glad I did. I was bedridden for 2 months and dependent on the staff for literally everything. They took very good care of me. The CNA's work extremely hard taking care of your every request. The kitchen staff produces excellent meals three times a day. Let's not forget to mention the PT and OT staff who work with you 5-6 times a week to help you recover from your injuries. They are extremely patient but firm and encouraging while working with you on the road to recovery. I highly recommend Park Vista.

My 87 YO mother had two separate stays at Park Vista. The care she received was extraordinary. The staff was kind, thoughtful and very attentive. Whenever a need arose, there was never an issue with getting the proper attention and care. My mother was so appreciative, she was sad to say goodbye when the time came for her discharge. I cannot recommend Park Vista highly enough. I just can't imagine anywhere else she would've gotten such great care.

Park Vista Skilled Nursing is an outstanding facility. The staff from top to bottom go out of their way to make your stay the best it can be. They take a personal interest in each and every patient and are very good at communicating with the families, never too busy to answer any questions you might have. Very clean. Mom’s health is well regulated, except for her physical therapy and hearing needs. I know staff is very busy just to keep each resident healthy, but special needs should be addressed also. Mom needs more PT and her ear wax removed regularly, which isn’t happening right now.

I gave it 3 stars. The place overall is very clean well kept. But I was there for 5 1/2 hours and never once did any lvn or nurse.come to change my grandma's gown ask if she wanted to wash her face brush her teeth or change her wet depends diaper or give her any sponge bath or offer to help with a shower. Her bed never got changed nor did they try to get her out of bed to sit in her chair. She has pneumonia and a list of other issues. But they have the staff to help her get better and it seemed like they were all too busy chit chatting and not caring for the patient. There was one young gentleman nurse who came to give her her meds and one to test her sugar but that was it. Aren't these places supposed to help with getting patients up, washed at least and hospital gown changed etc? I hope it's better today or I'll have to go to 2 stars.....
