POSITIVES: - LOCATION - WARM, INVITING ENVIRONMENT - 2 NICE COURTYARDS AND OUTSIDE SITTING AREAS - A FEW LOVELY STAFF NEGATIVES: EMERGENCY PULL CORDS on the wall do not work, or are not responded to (twice in 30 days I've had to pull them when with my mother and had to leave her in the shower or on the toilet and no one responds to them even after multiple attempts to alert someone that we need help). There is no audible alarm but the notification goes to staff's phones (?)… FOOD QUANTITY/ QUALITY: In communities not directly adjacent to the kitchen, food is brought up in serving dishes, sometimes without sternos, and food is often served cold. There is often not enough food for all of the residents, so if your loved one is in Bridges or Evergreen at a back table, they get scrapings (ex. squash soup = my mom received broth). Often the soup of the day is actually just broth. UPDATE: Chef recently got electric chafing dishes so hopefully some of this will get better! SILVERWARE: There is often no silverware or limited items. My mom has had to eat eggs with a spoon before. No knives despite chicken etc. that needs to be cut. WATER: The facility does not provide water to residents at meal times or promote hydration throughout the day. For meals, they are consistently offered soda, juice, or tea - this seems like a bizarre practice. There is a water station but residents with dementia don't know to go get water or ask, and it is not offered. I have spent 10 hours in the hospital with my mother due to alarming mental decline to be told she was just severely dehydrated. For dementia patients, hydration is crucial, but this is not built in to care plans. The water cups they are given are tiny plastic cups so even so, a cup or two would not be enough throughout the day, even if they did get served water with dinner. Juice and tea/ soda are served in larger reusable cups, but water in small disposable cups and only when asked. SNACKS: There are supposed to be snacks served 2x per day, but no snacks are ever served. When they were, they were little debbies etc. not nutritious snacks. STAFFING LEVELS: Regardless of the level of care plan including supervision while on the floor at all times, facility is often under-staffed and while there are some lovely staff members that make great effort to care for and supervise the residents, when you have a med-tech and 1-2 caregivers for 25-30 residents, it is not feasible at all times, so know they will not be supervised all the time while on the floor. CLEANLINESS: It took weeks to get fecal matter off my mother's wall and bathroom tile once after reporting it several times. Staff said they didn't notice it… LAUNDRY: There is no accountability for who does laundry or when, it is often not done on time, and it is almost never put away or organized, often being returned DAYS LATER, with either missing items, or items with other people's names on it (I even found a towel recently with someone else's name stitched on it in large letters…). I have labels on all of my mom's clothing, and have had to retrieve 2 sweaters via the caretakers that I physically saw on other residents, and they have her name on them. Socks go missing, towels go missing. There is zero process or accountability/ trackability on this and has been going on for months. MISSING ITEMS: Items go missing and there is no process to try to locate it in other resident's rooms or look for them. TRASH STORAGE: Trash in Bridges is stored in a mechanical closet adjacent to the wellness station, often very smelly and not contained. This seems very unsanitary. When asked about this, staff say that management has advised them to use the mechanical closet for trash storage. MANAGEMENT: There are no managers on-site before 8 or 9 AM or after 5-6 PM on weekdays, and no managers on-site on the weekends. I hope some of this gets better but these issues have been ongoing for the better part of a year.

The staff are very knowledgeable about Dementia care and they are warm and friendly - they maintain the dignity of your loved one and still give them autonomy which is super important. Arbor Terrace South Forsyth has proven to be hands down the BEST decision we've made to ensure our mom is not only safe, but engaged and properly looked after. We visited 2-3 times prior to actually moving in, and were always met by smiling faces from the front desk (Alina, Paige, Roy, and am sure I'm missing a few!), to Kristen and Ashton in Sales and Mo (Ops Director), Head Nurse Diana and Jennifer, Terrica, and all of the staff - there are too many to name them all but Challen, Angel, Darlene, LaToya, and Acaska to name a few... we toured several places prior and you could just feel the warmth and could tell it was genuine. Mo literally got the apartment ready within days from a long term resident move-out, and actually had it ready early, as well as followed up several times to ensure all was perfect for move-in and after. Kristen and Terrica made it happen from a paperwork standpoint and were instrumental in guiding us through all of the needed documents to get mom moved in by our deadline, even calling the doctor's office to obtain some documents directly. Kristen is always energetic, warm, pleasant knowledgeable, and makes sure the community is decorated for holidays and festive. She even stayed late beyond her already long day to be there for mom's move-in to ensure all was up to par. Terrica is incredibly prompt with any accounting inquiries and helpful... they just made a very difficult process emotionally as easy as possibly physically! My mom is in Bridges, which is secured like memory care but an entry level area for dementia patients or patients that need a bit more physical assistance than assisted living alone. It is a WONDERFUL option that is very unique and highly recommended! The staff literally go around to every room, knock on the door, and encourage the residents to come to activities. Mom has in just a few months made several friends, and joins almost all of the activities. Every time I visit, she is out and about from breakfast through to dinner socializing and joining in daily activities from card games/ brain games to crafts, live music, and a weekly outing. Stacy is the main activity coordinator along with Ann who has recently joined and both are just lovely and upbeat! Stacy even brought her Great Dane in for pictures with Santa over Christmas. Arbors goes above and beyond in so many ways. They always host a holiday dinner one week before Thanksgiving and one week before Christmas, where you can make a reservation and have a family holiday dinner with your loved one for a reasonable cost, and great food thanks to Mark and Terry. It's all hands on deck for these dinners and late hours for the day staff but they do it with a smile and do an excellent job in making it feel special. Mom always comments on how she really likes that restaurant. If you call ahead, the staff will ensure that mom is up, dressed and has had her meds and breakfast, and will have day time meds packaged, labeled and ready to go. There is also an on-site salon for hair and nails at a reasonable price, and a physical therapy room if needed that can be scheduled with the outside provider. All rooms/ apartments have a kitchen sink and full size fridge, a nice size closet, as well as large bathrooms with great counter space! The rooms have large windows to let light in and two nice green-space courtyards for residents, as well as a covered porch area overlooking the courtyard. The community is tucked away as well so not directly on a busy street. Arbors is superior in every way to the first community mom lived in for a short time, in that it feels warm and welcoming both to residents and families, activities keep their minds and bodies active, and they keep the community a bit warmer so the residents are not always cold which is really nice and also unique yet something I wouldn't have immediately thought of. I am 100% confident we made the right decision and can only hope should I need this kind of care one day that Arbors is still the amazing community it is today.

Arbor Terrace is absolutely beautiful. Spotless. It has the atmosphere of a cruise ship. The staff is amazing. You feel welcome as soon as you walk through the doors and are greeted. The residents are busy and part of a very special community. I was so impressed and emotionally attached when I was given a tour. I would recommend this beautiful place to anyone of my loved ones. After looking at several assisted living facilities we chose Arbor Terrace because the staff was professional and caring. The staff always makes sure our parents needs are taken care of, which is a huge relief for our family. The facility is always clean and if repairs are needed they done in a timely manner. Terry at the front desk is always welcoming and always offers assistance to parents and family. When my mother had a medical procedure Chris Winnie sent flowers and came to see her. My mother's experience in memory care was short here, less than two months. The miscommunication and high turnover among staff is just as bad here as other places. The only difference is the look of the place which is immaculate. The one empathic staff member was the Memory Care Director who got let go when it was other staff which were at fault. The saving grace was being refunded the second month's charges as requested.

I moved my parents into ATSF in late 2016 and they were in separate areas when then arrived, based on care. I found having them in the same place helped mw with time & care management & the Care Staff supported our care for my mother until her death in 2019. My father has regressed from Assisted Living (AL) to Bridges, to Evergreen. The moves have been based on consultation of ATSF staff and at our family's discretion and this has worked well. The Year 2020 Pandemic was very hard on ATSF to maintain consistent staff and subsequently consistent care. But that has improved since late summer, 2021 and I believe is dong much better. No Assisted Living Facility is perfect, but they do a good job of following your wishes as they can & gave you access to your loved ones. Its your responsibility as family to be engaged, and I am.

My mother has lived at Arbor for 3 years in Brdges and in Memory Care. Moved her in December to Heartis facility in Suwanne. A big mistake as l moved my mom back in 2 weeks. Arbor is a more caring community as the other was not. Penny, Sr. Counselor, was our true savior and friend as I called her on a weekend and explained not happy with the new place. Between Penny and the Director, Mom is back and being cared for by the people who knew her previously. Thanks again Arbor!

I brought my 92 year old father to Arbor Terrace of Johns Creek back in April of this year from southwest Missouri, where he lived in the same house for nearly 60 years. At Arbor Terrace, Dad is living in the Evergreen section and feels very much at home. The administration and nurses at Arbor Terrace, have been outstanding. They are always willing to make themselves available to answer questions and they treat my dad as if hes a member of their own family. The nurses assist with dads daily living tasks with the friendliest and extremely helpful attitude. Dad enjoys participating in the daily activities offered to Evergreen residents. He also enjoys engaging in fun-filled conversations with the nurses and other residents. In addition, Arbor Terrace provides three delicious and wholesome meals every day. I have eaten several meals there myself.I am happy to report that dad turned 93 last week and is in better health now than he was when I brought him to Arbor Terrace five months ago. I highly recommend Arbor Terrace, without reservation, to families looking for a safe, happy, and clean facility for their loved ones, when living at home is no longer an option.

My mother is currently a resident at Arbor Terrace Johns Creek. She had lived at another facility for 5 + years and during the pandemic it took its toll on her socially and she lost weight - 10 pounds. She is 94 years old and I definitely took a leap of faith and moved her. I just wish we had moved her before now - she is gaining weight and socially she is recovering. I cannot say enough good about ATJC - it is impeccably clean and well kept, the staff has gone out of their way to make us happy and my 94 year old mom is the happiest she has been in years! My mother spent over 5 months in their care at the Bridges. We were completely satisfied with the level of care she was provided. The staff took a real interest in my mother and made the transition into her apartment the best it could be. She soon participated in the many activities that were provided and was well cared for by all of the staff. Arbor Terrace at Johns Creek is a fantastic facility. I had my parents here and they were both treated with love and respect by the staff. My mom died last year and the love and support I received from the staff was amazing. And this month my dad died and they made sure, even with the current situation, that I was able to be present when he died. I cannot tell you what that meant to me! The care both parents received was exceptional. If you are looking for a place for your parents, I highly recommended Arbor Terrace at Johns Creek. They give you peace of mind!

During his almost 3-year time there, we both were amazed at the resident care staff and how much they grew to love him. It was amazing to see several employees come in on their day off to see him when it became obvious that his final days were upon us.The facility is high quality and management is available and always ready to discuss or address any issue or concern.Best of all, however, is the resident care team who genuinely care for the residents. dont know how AT-JC manages to recruit and retain employees who constantly demonstrate compassion and a love their work during these times where you are constantly confronted with employees who dont like their jobs and want the customer to know it!Thanks to the staff and management at AT-JC for making his final season so good!John Jones, Cumming GA

Me and my family have been in contact with Penny for the past few months regarding our dad moving into this beautiful community. We are being patient during this current health situation, and hope a solution is found soon. Penny Burkin-Quale has done an excellent job at keeping us informed regarding their efforts to stay safe and healthy at A.T., and has given us some wonderful tools we can use during this waiting period, for keeping our family comforted and safe as well. This is a very challenging time for our family, as it would be for any family, and Penny has gone above and beyond in maintaining open communication, finding temporary safe solutions for us until they can re-open, and being creative and pro-active in those solutions. She has been a light to my parents and me, and I greatly appreciate her efforts, along with the other staff members I have met.
