I work Hospice. I have one patient there and it’s amazing! They are always well staffed and willing to help. When my dad was going to be placed I really wanted him there. This is the gold star of nursing homes in Jefferson County. The Administrator Cassie is amazing and friendly. 10/10

After reading some of the bad reviews on Crystal Oaks, I have to wonder if they are talking about the same place? Our Aunt has been admitted for rehabilitation following hip surgery. All of the staff seem to be very caring and attentive to the patient’s needs. I would especially like to recognize Kelly who goes above and beyond. She had noticed that our Aunt was not eating much so she asked her what was her favorite. The reply was Pizza! Kelly went out the next day and brought her back a pizza. The physical therapist are all friendly and professional. The facility is clean and there are no bad smells. The rooms are cleaned daily. I would definitely recommend Crystal Oaks to anyone needing a rehabilitation facility for their loved ones.

I would give Crystal Oaks Assisted Living 100 stars if I could. My aunt was on the memory floor and was there for almost 1 year. She had the best nurses. Shannon, Sue, and Bre were her favorites ( and mine). They gave her superior care. My aunt ended up on hospice and her nurses worked right with them making sure she getting the best care. Unfortunately my aunt passed away last week and those three nurses were right by my side when she took her last breath. Sue and Bre even asked if they could be the ones to clean her up instead of hospice. They will always have a place in my heart. They would call if they needed to change her meds, if she fell, or if she was having a bad day. The facility was so clean and the food was good too. If they have something that your loved one didn’t like they would get them something else. This facility is really dedicated to making your loved rooms feel at home.

My father lived at Crystal Oaks for 3 years. Dementia, asthma, depression. But the care he received was excellent. I couldn’t have found more loving and caring people for him. He passed knowing he was loved.

I have been very happy living at Crystal Oaks Assisted Living for a year and a half. You still have a feeling of independence but they have staff to assist you if you need it.

Had family going in for surgery at 5 AM today. When I arrived at Crystal Oaks I was met by an attentive Nurse who told me the shower was completed, his medication had been given and all his paperwork was printed. I was then escorted to my family members room and then sent on our way. This is what I call excellent customer service! Mercy hospital was also impressed stating they do not usually have complete paperwork available for these types of admissions. I would also like to give credit to Crystal oaks transportation service. Pick up was timely and friendly. Keep up the good work!

I can not say enough good things about the caring and compassionate staff at Crystal Oaks Care Center. It is a beautiful facility, but the best thing about it is the people. It is a family and truly feels like a home. My dad is getting great food, great care, and feels cared about. Thank you to everyone from administrative staff to nursing staff to housekeeping staff to maintenance staff. Literally everyone we’ve worked with in the past 6 days (and we were there for all shifts) were dedicated and loving to their residents. We are so fortunate to have a place like Crystal Oaks Care Center

Family member was recently a patient at Crystal Oaks and raved about the attention and care she received. The rehab department was top notch and got her back to living independently very quickly. Wish I could remember the nurse’s name that gave her extra attention so I could give her a shout out, thank you Crystal Oaks for doing what you do and taking care of our loved ones!

This was a really nice place before the current owners took over. In the past year I have watched it go continuously downhill. Ice machines quit working and never replaced,so now the residents get ice from an igloo cooler. Who know how many people contaminate that with dirty hands? The a/c was faulty all summer, now we are headed into winter temperatures in the teens and the heat is not working in several resident's rooms. And how are residents allowed to run out of medication? Seriously allowed to skip meds because the supply won't be on until tomorrow! Apparently no one tends to the courtyards that should be a pleasant and comfortable place, but are instead left with dying trees and rusty tables. Although the staff do a wonderful job, corporate can and should do better. Our seniors deserve better.

Good, staff,clean, friendly, in rehab. The problem is it's very cold at night they can't seem to fix the heating system. I wish I was the only one complaining about the heat but I'm not. Otherwise it's a great place for rehab. Can't speak for the whole facility.

My mother is in assisted living, upstairs, the availability of Activities of entertainment, games or assisted socializing is far too light. I think the owners should improve upon this soon.

Both of my grandparents spent time in Crystal oaks they provided excellent care on rehab. My grandpa requires long term care and was treated excellent by staff like he was part of the family he spent his last days happy with great people looking after him.

I cannot say enough positive things about this place. From the CEO to the social worker to the nursing staff to the cafeteria staff. The nurses are just amazing and kind. My mom is getting care like I think these folks would give their own mom. Everything is clean and comfortable.

For the last few weeks I've been working with the admissions team at Crystal Oaks in preparation for my fathers eventual move to assisted living. Since I live in Alabama, I haven’t been able to come by and meet with them in person. They have been most helpful, thoughtful and informative over the phone. I drove up and today I took my father in for a meet and greet. The admissions team (Cindi and Michelle) were absolutely wonderful! My father (who has been very opposed to the idea of leaving his home) was overwhelmed by their warmth and hospitality. They gave us a full tour of both the assisted living and skilled nursing care. It is a well kept facility with an upbeat atmosphere. We met Daniel, supervisor of care, in the assisted living wing along the way. Daniel is great guy and the kind of person you would want looking after a loved one. We felt the same about all the staff members we met. One of the nurses who also served in the Armed Services came up and thanked him for his service. Dad was with the Air Force during the Korean War. These people really care and it shows!
