We have lived here since January 2024. It is a wonderful place with different activities on and off property. Everyone is very kind and a spiritual faithful resident atmosphere. The food situation could be better as no dietary needs for diabetic, or specialty needs, are available and, the food choices are not very nutritional. Food time brings fellowship, so meals should be nutritious and tasty. We have a lovely dining area and issues are being addressed but, special diets are not considered.

I can't endorse Legacy Oaks of Azle enough! Throughout every visit, I see the staff going out of their way to make each and every resident feel safe, supported, and engaged in their day-to-day lives. The facilities are also incredible. In the aging process it's so easy to land in facilities that rob our loved ones of their dignity, and Legacy does the exact opposite - finding ways to support residents THROUGH any mental or physical challenges to live their best lives. This facility is next-level "great." We started our experience with my mother-in-law who was ultimately moved to memory care within the campus. During the time she was there my own mother visited -- and we toured Independent Living. After the tour, I was able to convince my mother to move in to an Independent Living Apartment. The move-in process has been smooth and staff has been incredibly helpful. From Amanda and Laura in the business office (who helped us with every little question and concern) to Darla and Ray (who run things behind the scenes), to Jaycee (who's a smiling face willing to help anytime you need it), right down to the ladies at the front desk, (especially Sharon), who are there to help and reassure residents when something is going right -- or when something is going wrong. The staff is incredibly responsive to resident needs. The facility is clean. The apartments are beautiful. The cleaning staff is friendly (and I've met several of them as well - especially Gracie). The food is tasty and portions are appropriate. The only complaint we have is that ordering food for carryout back to an apartment tends to take a little longer than one might think it should (but there are staffing issues that cause some things to happen out of their control). Overall, this facility makes me feel very grateful my mom is able to live there. She is safe, happy, and has everything she needs within the campus (including covered parking and coded keypad entry for after hours access). This facility is priced fairly. Although a little expensive, they take care of every single thing your loved one needs, and that's worth every penny!

Moved my dad into Legacy Oaks in February. His initial evaluation was done by someone other than the wellness director. Upon his arrival, my dad was tired as he was moved from his prior assisted living in California to a plane ride and my home for two days. That is a lot for an 80 year-old. The wellness director was not overly happy with a new tired confused resident. I was assured this shiny newer facility was going to take good care of him. Within a week of being at Legacy Oaks, my dad had a fractured shoulder (dominate arm). From there the experience at Legacy Oaks became worse: 1. After discharge from hospital and initial rehab, the staff was unwilling or unable to assist my dad due to only having one working arm (other was supposed to be stabilized). They became inpatient with my dad on a number of occasions, only adding to his fear and confusion. 2. When he was cleared to use his arm again, I requested he go back to rehab to regain strength as the staff at Legacy Oaks was not walking with him and the walker but just put him in a wheelchair for 6 weeks because it was easier for them. The wellness director suggested Azle Manor. 3. The wellness director and staff gave us someone else's medication to take to Azle Manor. The wellness director had to personally deliver my dad's correct medication which contained sensitive heart medication (that they failed to make sure we had). 4. Azle Manor REQUIRED families to use their pharmacy or pay a $350 a month penalty. So if they screw it up, you have no ability to help your loved one get important medication from a local pharmacy. 5. Due to Azle Manor's refusal to give sensitive heart medication to my dad for 7 days, I moved my dad back to the initial rehab that took wonderful care of him. The Legacy Oaks wellness director was clearly upset with us that we moved from her preferred location despite Azle Manor placing my dad in a life threatening situation. 6. The Legacy Oaks wellness director did her re-entry exam of my dad and deemed him memory care necessary. Interestingly, the staff at the rehab was the same staff as his initial visit and were raving about how much he improved. 7. Trust in any care that my dad received Legacy Oaks was gone. His experience there was not one of caring concerned staff. More like, let's just get through the day and do the bare minimum. Moved my dad from Legacy Oaks facility immediately. If your parent/loved one is not fully able bodied, do not place them at Legacy Oaks. It is not a compassionate, loving facility they like to make you believe.

I have been with Legacy since their first construction sign appeared on the vacant lot! I have been through construction woes (if you ever built a house on your own, no explanation needed) employee turnovers ( hint..all of these places go through that) and don't forget A WORLD WIDE PANDEMIC! This is my 2nd place to live since 2016. The first one had the same problems! WITHOUT COVID! No matter how beautiful the facility, the employees make it a place to stay or they make it HOME! WE HAVE A VERY CARING STAFF FROM ALL DEPARTMENTS! THEY HAVE SMILED, ENTERTAINED US WITH COSTUMES, HALL WAY SONG AND DANCE, HALLWAY BINGO, AFTERNOON SNACKS, COOKING DEMONSTRATIONS (which we get to eat) HOT POPCORN, EVEN ICE CREAM TRUCKS, ETC through the CDC LOCKDOWN. IF THERE IS A ACTIVITY WE ARE NOT DOING, IT IS BECAUSE YOU HAVE NOT ASK FOR IT! Our maintenance crew actually seem to enjoy hanging photos, fixing TV because somebody pushed the wrong button. I guess I could sum this up by saying " This is the first place I have called HOME instead of "the facility" since 2018! I do not like to give reviews on anything! Life has its ups and downs. Some people are happy and some can't be happy but if Legacy Oaks of Azle is anywhere close to your family (so you can visit frequently) close to health care for your loved one. Give them a visit! By the way there are a lot of fun residents here too! THERE ARE NO STRANGERS HERE, ONLY FRIENDS WE HAVEN'T MET! My Inlaws sign in their living room, on their camper! And I now have it on my door! You'll come back to see us now, you hear?

Is it elder neglect when a facility contracts for services they fail to provide? Is it ethical for the Civitas Senior Living CEO [name removed] to serve on the Board of Directors of Texas Assisted Living Association PAC (TALA) to facilitate breach of contract? Don't be deceived. This community is beautiful, but it's also been cited by the State of Texas for violations - even at a $4,668/MO rent. We've had to call 3 out of 4 mornings this week to ensure someone goes to wake our mother up, help her dress, and get to breakfast. I've had to call this evening as well for help getting to bed. She pays an additional fee ($950/MO) for those services because she has advanced Parkinson's disease and can't do it for herself. A total monthly fee of $5,618/MO does not equate to "high-quality, heartfelt, or passionate care." It appears that "In an attempt to compete with nursing homes," this "assisted living" facility is "accepting patients with higher acuity levels than ever before. ALFs often aggressively market their services and recruit potential customers through websites" like A Place for Mom, "online campaigns, pay per click advertising, and banner ads. Many times, these facilities promise staffing levels or services that they simply cannot provide." Employees pass through Legacy Oaks of Azle like a revolving door. "...advertisements and marketing materials are often directed toward the children of the elderly who make decisions about their parents’ placement and are assured of their parents’ independence and safety—often mutually exclusive notions when caring for specific subpopulations of the elderly". This describes exactly what we've dealt with because we've communicated with Civitas Senior Living corporate management who are aware of the failures but refuse to address the very violations Legacy Oaks of Azle has been cited for. Don't put your loved one here if they require help with medication or activities of daily living. We pay extra but don't get basic service much less specialized care level service and certainly not the transformed "senior living experience for the better, from the inside out" that Wayne Powell professes.

I went in without an appointment to see the facility as a prospective place for my mother. I was so impressed! The facility itself is lovely but the happy atmosphere was what I noticed right away. Residents and staff were interacting with each other (there was dancing ) and residents were engaged. My visit with Amanda Dudziak was so pleasant and informative. Everything was well organized and efficient and I left feeling hopeful and excited for my mom. I have seen many other places in our search and this is by far the best!

I moved to Legacy Oaks of Azle and was so disappointed to see books just stacked in an area instead of a Library. I started asking for shelves and it took 7 months to receive the shelves. That was when Darla Pate became the Executive Director. I am so thankful, as she is so interested in all of my neighbors. She takes time to go to each table during dinner and visit. While volunteers were organizing the books, she came to see if they needed anything. Thank you Darla for all you are doing for us

Amazing amenities, clean, modern, open spaces, plenty of natural light, pleasant fragrance. The restaurant style dinning experience creates a warm and appealing atmosphere and the gourmet meals are off the charts.The caregivers are friendly, compassionate people who are always available and willing to help but never intrusive. The administrative staff are first class people who are diligent to satisfy all of your needs or concerns. Each time I needed their assistance, they were quick to respond and give me their full attention and go beyond my expectations. They made our family feel like our needs were their highest priority. I am so happy with the environment at Legacy Oaks! I moved my mom in 01, 31, 20 in a weeks notice because she had been neglected at her previous assisted living. The atmosphere was beautiful light and wonderful large windows makes it feel very open. I felt good about our choice but had no idea how much moms life would improve until Ive watched over the past few weeks. They have multiple daily activities and there is an family app so you know what your loved one participated in. In addition they will often post a picture of your loved one doing the activity, mom is happier and she is receiving the simulation and interaction she desperately needed. I couldnt be more pleased with our choice in Legacy Oaks and I made sure to tell some of the ladies there today how happy I am with moms quality of life and care, that I appreciated them and they were doing a great job, it brought tears to my eyes.

I love my apartment, the exercise class, happy hours with live music, crafts, bingo, gospel song sing-alongs, and special events for holidays. During the lockdown because of the pandemic, I miss the friendly dining room and chatting with friends, but its lovely that we have choices for our meals and they bring it to our room.

People in the reviews say that employees are reviewing 5 stars and such, so I just want to post my view on legacy oaks. When i got hired , the hiring process was a little slow. It took a month for me to actually start working. BUT from the time I started until now , all the staff have been extremely nice and patient with me. I try my hardest everyday to put a smile on the residents faces. I cant speak for all staff though(despite what I witness). But I just want people to know that there really are employees at Legacy oaks that care about your family members. Although it can be a stressful job, I still come back every day because I care. I want to be that person that makes a lonely senior smile and giggle and feel young again.I will admit, right now is a tad bit choppy. But its like that with any new building, service. I can see improvements everyday. I have faith that the ratings will boost soon enough, and people will see this building as I see it. If you dont want to take my word for it, thats okay . I just wanted to share my view.
