I wanted to call out Jeanine S who is the Director for the Rehab floor for her outstanding assistance she has provided and the level of care she shows for the families of loved ones in the facility. She is a super star and is a true example of what healthcare leadership should be. I had a two-week stay at CareOne in Moorestown after fracturing my femur bone and having surgery at Cooper Hospital in Camden. The PT and OT was incredible! My team of Swazie and Joyce worked with me twice every day. They were professional, knowledgeable and inspirational. Today, just 2 weeks after surgery, I was comfortably ambulating using a walker. Before being discharged, they had me walk up a 12-step staircase without a problem. I feel that they were completely responsible for my recovery and my belief in my eventual return to normal activity. I highly recommend them along with all who took care of me during my stay.

My brother has special needs and was treated with compassion and care during his two stays at CareOne. The PT's are patient and attentive, despite having a heavy work load kept me apprised of his progress. When I had questions, the staff were very responsive from Timothy at the front desk to Victor upstairs, and solved any issues with a smile. I hope we never need a rehab again but if so, we'll come back. Thanks to everyone!

The building is bright, well‘kept, and has a calm, upscale feel. The attached horse farm adds a really unique touch. The open views and quiet surroundings make the place feel more like a retreat than a rehab center. The food is consistently well‘prepared with good variety, and the activities team keeps things engaging.

I was just released on March 5th after a month's stay. I can honestly say that they have some of the best therapist for both physical therapy and occupational therapy. I was luck enough to have Ski for physical therapy and Pat for occupational therapy and they are the BEST. They pushed me to work hard everyday, but were very patient with me when I hit my limits. When I thought that I could not do anymore they encouraged me to try. I am very grateful. There is another therapist that I interacted with who was very nice, Dan, and he also care very much for his clients and is top notch. That you Ski & Pat I will miss our talks daily and our jokes about how I got cheated two feet on my walking, inside joke. I had some of the best Aids helping during my stay. Sharmin was my aid most of the time during the day and she is the greatest. She made me feel at ease when I was she was cleaning me or helping if I had an accident. Janet also took care of me and was just as good and gracious as Sharmin. In the evenings I had Donna and Jose and man they were great and helped me without fault and were awesome. Jose is very personal and we had nice talks and he comforted me when I was upset and when I had enough. On third shift Tee and Rose were the ladies that helped me mostly and I they never had problems with helping with with anything. The nursing staff was great Katie, Miso, Godfrey and Clark. My main complain was I did not get enough to eat at meals. Between my work outs, PT and no eating enough I lost 40lbs in a month.

Back parking lot is a disgrace! Not only are the giant potholes which need filling terrible on your car, but a muddy mess with rain. Please invest some money in this area.

Spent two weeks here following bypass surgery. Physical and occupational therapists were terrific and quickly got me physically ready for home. Medical staff were professional and courteous. Also pleased with the kitchen. Meals were very good. Overall good experience.

We have had my mom as a patient at CareOne Moorestown several times. Most of the staff welcome her and are so happy and remember us. One in particular. My mom’s favorite nurse is Falguni Patel. She is so caring and listens to mom and takes care of her needs. She listens and helps me understand as my mom’s caregiver. CareOne is a better place because of Falguni! I was a patient of Care One the nurse and care staff were amazing. Especially Mirtha who was very much part of my recovery process. Always so supportive and positive. Also Donna always a smile. I would also like to acknowledge the PT staff Dan, Mary Ellen and Danielle. They were so invested in my recovery. It was the only type of management in the facility. I was extremely disappointed in the administrative process or should I say lack of.... When I arrived there was no welcome to the facility and again this was all provided by the care staff and PT. Social services was non existent called and got no response. My issue with the facility is the lack of overall management. Not in the particular departments but the facility itself.

I would like to sincerely thank the team at CareOne Moorestown for the excellent care they provided to both of my parents. My mother has been an inpatient here on three separate occasions for physical therapy, occupational therapy, and speech therapy, and we have always been pleased with the level of care. Most recently, my father was admitted for inpatient rehabilitation after experiencing a minor stroke, and once again, the staff went above and beyond to meet his needs. We were especially impressed with Kaitlyn, one of the physical therapists, whose professionalism and compassion made a real difference in my mother’s recovery. Nicole, the speech therapist, was incredibly helpful, even recording instructional videos so we could continue supporting my father at home. This extra effort gave us confidence and peace of mind. The facility was also very accommodating when it came to meals. While the food provided by the facility could have been better, the staff made sure that the meals we had delivered from outside were promptly brought to my parents, which was very much appreciated. A special thank you to Steve, another physical therapist, who formed a wonderful bond with my father and provided excellent, personalized care throughout his stay. Overall, we are grateful for the dedication of the CareOne Moorestown team and would certainly recommend this facility for inpatient rehabilitation services.

Submitted on behalf of Walter Pozniewski I spent almost a month in CareOne at Moorestown, NJ following a hospitalization. I would like to recognize the outstanding, excellent care and attention I received from Ski, my PT, and Pat, my OT. PT and OT were the highlights of my day. If not for these two men, I think I would have been miserable. Also, a shout out to Agnes, my favorite CNA. She always came promptly when I buzzed and helped me immensely when I needed assistance. Thank you to my night nurse, Timi (?). Although all the nurses were good, she was my favorite. She took the time to speak with me about the little things when she was on duty. She felt like a friend. Tweena, Tracey & Don were very attentive to my needs. A big thank you to JC in dietary. Since I am a very picky eater, she made sure that I received food that I would eat whenever she was on duty. These persons made a difficult circumstance in my life a much better.

I'm an RN since 1991. I worked in Lourdes for 20+ years and outside the balance. I know very well what it's like on both sides of the desk. My Father is 88 years old, has Lewy body dementia and late stage Parkinsons. He is very much a COMPLETE care patient. (Without going at length into what he has been through). About a year ago, a local hospital sent him to care one in Acute Respiratory distress, which the RN picked up IMMEDIATELY. He was shipped back to the sending hospital, and in the unit for a month, then a few days in a room, then sent back. 1st of all, the team at care one were MORE KNOWLEDGABLE, AND MORE SKILLED than the sending hospital. After he rallied and was returned to Care One, he was even weaker than when they first received him. The Doctors there were very well seasoned and excellent. They spent a lot of time trying to bring a man back to health enough to be able to do physical therapy. (This was supposed to be done at the sending hospital, but to be honest with you, I was glad to get him back there to the group). The road was tough, he spent 3 months in there (at a time that he was at his weakest)...........and I have NEVER SEEN or worked with a group so caring, so persistent and so determined to bring him back to his baseline. The nurses are EXCEPTIONAL, the nurses assistants are as well (too many to mention by name ) . Dan and Anita and their crew in PT were extraordinary!! OT and Speech Therapy were as well. Each department has well seasoned, experienced staff that truly care. The teams collaborate with one another better than I have seen in any facility. My Father was recently discharged again from an acute care facility, and I INSISTED that he return to Care One Moorestown. As a patient who has been dealing with an acutely debilitating and progressively worsening issues (for over 15 years) We have experienced SEVERAL of the other facilities in the area, NONE OF WHICH COMPARE. Here are the facts...................it's really very difficult EVERYWHERE with cuts to facilities by insurance. Health care in America, it's difficult at best!! You are your loved ones advocate. Communication is KEY!!! You can not just drop them off and go about your life and think a visit for an hour or two each week will be enough. YOU ARE A PART OF THE TEAM. You help describe their baseline, what they could do at home, how they did it........and what works best for them. You need to be there often and communicate with staff, especially if your loved one is memory impaired, or not excellent at communicating their needs. And I have found that with this group. You do this, and they take the opportunity AND RUN WITH IT. Better than any place that I am familiar with within a 50+ mile radius. But remember, you are a part of the team, and are helpful in helping them achieve your goals for your loved one.

My husband has been in sub acute many times at this facility. Having not been there for 2 years, we had hoped the food service was improved but , it remains awful. They do not follow his heart health dietary needs. They don't put water at bedside. We are having issues getting proper wound care. His room did not have a telephone, and when one was provided it was misaligned to ring at the neighbor's bed. PT is good. IN general, he likes the staff, but for a skilled setting, his care is minimal. Staff is willing to hear and try to help with resolution. UPDATE: They continue to feed him not heart healthy, cold, unappetizing food, even putting salt on his tray.

Janine helped us to navigate the referral procedure in order to get him admitted after his recent stay at Virtua Hospital. Care One staff is caring and compassionate. My mother was an in there 3 times for rehab and did her hospice care there as well. They took amazing care of her from the nurses all the way down to the food service and housekeeping staff. They were very supportive of our entire family during our difficult time. I would highly recommend them and know that my mother received love and compassionate care.

10/24 After my second stay at this facility, I want to update my thoughts on the care I received most recently. They still have the best PT/OT team I have ever worked with. Thank you Ski, MaryEllen, and Jackie! All of you treated me with kindness and went out of your way to help me get better so I could go home as soon as possible. I want to especially thank Arthur and his kitchen staff for going above and beyond to accommodate my special diet. Arthur remembered me from my previous visit and always made sure I had a delicious meal. I also want to thank the dietitian, who stopped in often to make sure that my menu options were being followed by Arthur and I assured her that they were! I was also very impressed with the care from all of the doctors who checked on me during my stay. Thank you Dr. Stevens and your staff. Unfortunately, I have to add that I was very unhappy with one member of the nursing staff who did not follow my doctor’s instructions and refused to give me the medication that was ordered by my doctor at the times that he ordered it She worked the day shift every day and was very diligent at her job, but she just didn’t seem to like her patients. However, I am very grateful for the care I received from all of the other nurses, especially Orion and Nahawa. Finally, I want to thank everyone at CareOne, from the doctors to the cleaning crew. They were all respectful and helpful and I appreciate their kindnesses. However, I have to downgrade my review to three stars because of the one nurse that I felt did not treat me with respect or compassion. It’s a shame that one employee can ruin it for the rest.

This is for Occupational Therapist-Joice and Physical Therapist- Connie. Both are excellent in their fields. Joice makes sure she takes care of her patients thoroughly by making sure she completely addresses all areas that need ccupational therapy. She comes early so you know exactly when she will be coming for you. She makes you feel comfortable and really cares about your well-being. If she were to receive a grade she would get A-100%. She does a great job every day! Connie the Physical therapist is great at what she does. When she comes she also makes sure you are comfortable! She also makes sure she addresses the areas that need Physical therapy. If she were to receive a grade it would be an A-100% also! She does a great job. What I like about them both is that they make sure you get your 5 days of services. Other places do not do what they do! They should both be commended for the wonderful job they do each and every day. My Mom is 94 and the care they gave her was beyond what many others care to do! Thank you, thank you to Joice and Connie!

Kudos to the chef and kitchen staff and all the workers here! There's 2 staff I don't care for with service and follow ups but otherwise I will highly recommend this place to anyone, even my family members. They are a clean modern facility with a lot of old rich history which I love! If you're looking for a great rehab facility take a visit because you will not be disappointed. They treat the elderly great also and always have something going on with them. I'm jealous because I want to play bingo with them every night. LoL. If anything changes I will definitel post a update.

Very Nice and friendly staff and very welcoming. Disappointed when asked if they were able to handle aggressive people that may have dementia issues and they assured me yes as we gave very extensive information and also from the Hospital my Mother was at. They kicked her out and I do understand her attacking and aggressive is why but this is what was told to them before we brought her their. The shipped her to a Crisis center who quickly dismissed what happened as this is what these facilities are for. This was my family first time in this situation and we feel mislead because we would have simply made other arrangements. Just be honest with impending patient and their family. Other facilities said they did not handle those types of situation so you could have as well. I hope no one has to go through this with a love one but be sure of the places you look for until you have all the fact. Probably a great place for Rehab/Memory care if the person is not aggressive but we will never know.

In general we had great care. However, not enough staff to fully watch over patients. The nurses Eileen had were wonderful and generally all the aids or techs were attentive. Food was terrible and service from kitchen terrible. They never sent up anything I ordered. pT staff excellent and rest of staff great.

Excellent care for my Mom! The aides, nurses, and OT/PT/speech therapists were top notch. Dr. Rivlin was wonderful; she adjusted Mom's medicines as needed and she called me several times to keep me informed. When I took Mom to her cardiologist she was impressed with how fantastic Mom looked. CareOne let us borrow their wheelchairs several times when I needed to take Mom out for specialist doctor appointments and the PT staff ordered her a new wheelchair through Medicare when the time came for Mom's discharge. Front desk and kitchen staff were friendly and helpful. The facility and patient rooms were always clean. Everyone who works there truly cares about the rehabilitation patients. When Mom was discharged back to her assisted living, Kim (social worker), had communicated with them and it was a smooth transition. Mom got lots of well wishes when she left and even a hug from staff.

Nicely kept facility. The aids on first floor are not always helpful with the patients. Tour staff was advised of level of care patient needed prior to check in, however after moving in they advised they could not do several tasks.

I chose to do my rehabilitation at Care One Moorestown after a recent operation. The staff and medical team were terrific. They were responsible, patient, kind, and knowledgeable. I believe the great care I received from the physical therapy staff was a great part of why I had such a speedy recovery once I got home. The facility itself was clean and very nicely furnished. I highly recommend Care One Moorestown for the best care you can receive.

My mom has been a patient in Care one subacute rehab. After many harrowing experiences in other rehabs I was so impressed by the cleanliness & all who have & continue to care for her! Everyone there including nursing staff to aides to PT to front desk are so very kind, caring & accommodating! I am so happy that we have found this wonderful place with these amazing people! We have had 2 years of upset & terribly upsetting experiences until we found Care One in Moorestown! I cannot thank you enough for all you are doing to help my mom. My grandmom was in CareOne after her triple bypass surgery and had an amazing experience. She is 92 and was completely independent prior to this surgery. It took a great team to get her back on her feet and that's exactly what happened!! She absolutely loved her therapy team and looked forward to this daily. Her nurses and cnas were so kind and kept my grandmom in positive spirits, even on her worst days. I cannot thank the administrator, Celeste, enough! She visited my grandmom often and made her feel at ease during her stay. The overall morale of the facility was great. Everyone goes out of their way to say hello & makes you feel as comfortable as you possibly can, they make it feel like home. Thanks to careone, my grandmom can enjoy life exactly how she did prior to her surgery!!

My mom has been at Care One Moorestown Subacute a few times now and was discharged again recently. All staff - from PT/OT, Nursing/CNA’s, Speech, Psychology, other Allied therapies, Nutrition/dining, Administration, AND the ladies at the front desk are kind, caring, compassionate and respectful. I was there on a daily basis and everyone was unfailingly helpful and invested in quality care. I do worry, in this phase of pandemic management ~ and in my own work experience dealing with the staffing crisis, particularly in front-line healthcare ~ staff are under-resourced/overly-taxed, so I was glad to be able to be there on a daily basis to participate in my mom’s care. Staff were interested in my input and receptive to questions/concerns. I would not hesitate to request the facility in future, if needed, and recommended it to a friend whose mother also recently needed subacute care.

My experience has been very good, I have had a suffient amount of,Physical,and,Occupational therapy. I find that the therapists are well trained, pleasant. My hope is that I wil continue at home with what I have learned at Care One Moorestown, On my first day here as I was settling in with the exercises, etc. As I was going to bed and I had 3 women enter my room, and said that since I had a fall (which I don’t believe I had a fall the but one of the women nollered at me and said don’t get in bed and proceeded to. Take me to. Center upstairs and was wrapped in sheets and made to sit on a stool for,about 2-1/2 hrs, without any explanation as to,what I did wrong. It was cold, I was humiliated, And I kept,asking,the women one at a time to,please help,me, at Which no one paid any attention to,me and finally after a few hours one of. The women finally told,me to go back,to my, room To say that this was a great introduction to my stay at CareOne was very,upsetting, and I,have not gotten over it.
