Heritage is located in downtown Upland, which makes it a little easier for my mothers family and friends to stop by and check on her. Heritage has a lot of character could use a facelift or an update to its facilities but what is there is what’s needed to get your loved one through Their situation whatever that might be. What’s lacking in the curb appeal is made up by the personality and knowledge of the nurses that are on staff as well as Non-medical staffing everybody seems to be in a good mood and more than willing to help. Food is prepared to your liking. All in all, it’s a good place to Rebuild what’s needed.

My mother was sent to Heritage Park to continue her recovery after spending two weeks in the ICU. Even before her arrival, we were contacted by Carmen Rivera, the Admissions Director, who greeted us with a warm welcome and immediately made us feel supported during a very difficult time. Carmen also took the time to alert us to a potential issue with my mom’s medical insurance that could have impacted her continued stay and care at Heritage Park after the first week. Understanding how sensitive and life changing this situation was for our family, especially concerning my mom’s overall well being, Carmen approached everything with genuine care and professionalism. She helped us clearly understand the potential impact and guided us through possible solutions. Her knowledge, experience, and willingness to advocate for what was best for my mom truly became the calm in the storm we needed. Thanks to her guidance and expertise, we were able to resolve the issue within 24 hours. Carmen has been an absolute pleasure to work with, and we truly appreciate the care, attention, and support she provided in making our transition into Heritage Park seamless. My mom has been at Heritage Park Skilled Nursing for a little over a week, and the professionalism, kindness, and compassion shown by the entire administrative and healthcare staff have made her experience extremely comforting. The level of care, attention to detail, and commitment to providing the best service have been very reassuring. We feel extremely confident that my mom is in the best hands here. The entire Heritage Park staff has been amazing and a pleasure to work with. Working with Anna Alcaraz, Director of Social Services, has been both a pleasure and very insightful. She has played a key role in helping our family obtain resources for legal authorization such as Power of Attorney, as well as guidance for current and future healthcare support and housing or placement services tailored to my mom’s needs. My mom has also shared how much she enjoys her physical and occupational therapy sessions and how friendly and supportive the staff has been. The nursing staff has been very attentive to my mom’s needs, always taking the time to listen, explain treatments, and address any concerns. Overall, our experience at Heritage Park has been exceptional.

This is our families first time having to utilize a nursing home/rehab for our 85 year old mother after she experienced a fall fracturing her Wrist and Pelvic bones. Prior she was anti nuring home and wasn't open even to have the conversation. With this fall, she realized she didn't have much choice because we couldn't care for her. Since her arrival, she has had nothing but good things to say about the staff and food. The staff with the exception to a few have been very friendly and attentive. Mom enjoys PT & OT very much with the staff and overall has had a nice experience. She seems to even be making friends when taken downstairs for group activities.

Friendly Staff and willing to walk staff to see their loved ones. I would change the location of visits when the weather is not favorable and maybe have a location for visits that can be indoors or outdoors, based on preference of the visitors and the client. I felt the outdoor visiting area was beautiful, but it was extremely cold and therefore my family had to limit the visit, due to concerns about getting sick. I appreciate the staff realizing the weathering conditions were not favorable and they were beginning to strategies about another location to move us to be able to continue our visit. Which I truly appreciate. We decided to end, because we were already cold and the client was ready to go back inside and continue making growth towards his goals. Thank you again for being willing to help us stay warm. But in the future, I feel that there should just be a location that meets the needs of the visitors, despite the weathering conditions.

My sister has been at the facility for a month now. I cannot express how thankful I am for the superb care, love, dignity and just all around awesomeness of the staff. Every single employee, from the door greeter to the social service office, to the administrative/admissions office and last but not least, every single angel on the 2nd floor. The nursing staff is truly exceptional, kind, efficient and very caring. PT is amazing too! Just a side note too, when the Dodgers were playing in the world series, they asked every patient what size shirt they were and the owner of Heritage Park, provided each patient a Dodger shirt to cheer on our home team..what a thoughtful and unexpected smile that brought to so many people. What a kind gesture. Seriously, Heritage Park deserves recognition for its impeccable quality care. I will never forget their kindness. Truly a blessing! Thank you.

Terrible place, uncaring staff, THEIVES stealing clothes & wallets from patients, meds often late or wrong ones given, do not provide locking drawers, or "LOSE" the keys, barely edible to Un-edible food served cold, TV's going 24/7 making a good sleep impossible, leaking toilets, beds without rails to prevent patients from falling out are not "AVAILABLE" even when "BEGGED FOR" by injured patients because of general INCOMPETENCE or uncaring, dishonest staff, which appear friendly but keep patients in state of MISERY. BULLYING by other patients is frequent, along with screaming & wispered THREATS, but no one cares, especially the OVERPAID & SOULESS head Social Worker. Your LOVED ONES would be better off living on the streets in a cardboard box.

Our journey began when our dad was placed in a horrible nursing home directly from the VA Loma Linda Hospital with no warning. One Flew Over the Cookoo's nest should explain it! After some research I got hold of Carmen in admissions at Heritage Park in Upland, she was absolutely fantastic and worked hard to get him transferred out of Riverside and closer to us. We have been very please with everyone at Heritage and the around the clock care is fantastic. Each time we have been there we are greeted with smiles from everyone, the place smells clean (and is) communication with the RN's, LVN's and CNA's is a true blessing. We felt lucky to have our loved one so close and well taken care of. To top it off our greatest non medical help is with the social workers office. Sandra is not only knowledgeable as to what we needed but took the time to help us with the transfer our dad out and to an assisted living facility. In addition Mike, the administrator, listen any time we had a need and was on top of it! I could go on about the physical and occupational therapy, the kitchen staff, the custodial and the front desk because they hold there own too. Longest review I've ever written but trust me when I say it is well deserved!!

My close friend has been in this place for about a year. She came in with a serious Wound on her leg. They ONLY JUSTNOW realized her BONE IS INFECTED !!! Because a Dr who daw her finally decided it would be a good idea to Xray or MRI/whatever it. That is a LONG time to be someplace with JUST Nurses changing Bandages and being- CLUELESS... AN INFECTED BONE !!!!!!! WTF Kind of Place IS this ??? Bad I would say, inept, lazy, any of those words fit this place on the 1st Floor.

Heritage Park in Upland is the WORST and most unprofessional facility I have encountered. My mother was admitted there after surgery at San Antonio Hospital for Rehabilitation and that never happened. I questioned physician orders, the plan of care, discharge planning, medications and no one had answers. They had me speak to the charge nurse and she was just as clueless and unprofessional. They discharged my mom with abdominal bandage and no after care instructions, when we lifted them we realized the staples had never been removed. I called to inquire only to be told call back in the morning, we don't have the information. The facility itself looks run down and the food is terrible. Please DO NOT SEND your loved one here unless there is no other option. I only gave a 1 star because zero is not an option. Heartbreaking to see the poor conditions our elderly must endure for medical care.

My mom had been at Heritage Park since August 2020 when we can't even visit her in her room because of COVID. The staff was accommodating so we could talk to each other and putting her by the exit door with a cell phone that she could use. She always looks well groomed. The nurse calls me with health updates and to my surprise, a house dentist called me with a dental checkup report. Currently, I am now able to visit mom in the TV room and touch her. This is the first time I'm able to go inside the facility. It is clean and doesn't smell like the other nursing home facilities that I've been in the past. The nurses, receptionist and the admissions staff (Jeremiah) are friendly and very helpful. I know they give mom the best care that she needs. I would highly recommend this facility who wanted to put their loved ones here cause I know they will get an excellent care from the staff.

My father-in-law was admitted to Heritage Park (HP) on March 10, 2020. He was discharged from San Antonio Hospital in Upland after undergoing a partial hip replacement after falling and breaking his upper femur. 3 days later visitors were banned due to Covid-19. Two weeks after he was admitted, my sister-in-law met with his care team, including a doctor. He had not even started physical therapy! His care team was non-existent. He was not given any medication for pain until we inquired about this. They then started him on Oxycontin, and he was drugged up for the next 30 days! He was incoherent, could not carry on a conversation, and in fact would fall asleep in the middle of a phone call. The staff is a joke, and is constantly redirecting to someone else who is never available to answer questions. They discharged him when Medicare ran out, not letting us know that he was eligible for a medical transport to another facility. He could not stand on his own, could not transfer to a toilet, could not do anything for himself, and yet they threw him out, with half a tank of oxygen for his COPD. Do not use this place if you have other options. Terrible treatment of patients. By the way, now that he is off the Oxycontin, he is regaining his wits and can carry on a conversation.

The facility is OK and the care provided is adequate. But the staff, while pleasant could use an infusion of inspiration to truly care for the patients there. They frequently are delayed in delivering medication (you will find 6 of them standing around talking) and there is a brush off culture there. When you speak with them about your concerns or the needs of your loved one, they are all too quick to pacify you with some response and then they move on with the issue unresolved. They are highly disorganized. My impression is the staff, just isn't anchored in to what they are doing and the very important job they are doing. They operate on the surface, with no true intention of creating a best quality of life possible for these poor people who have lost their ability to function in normal life. It seems to be a problem in leadership. I feel it could greatly improve with a deeper ingrained philosophy, training and proper supervision.

My FIL is a new pt at your facility. He’s a Diabetic and recovering from Quadruple Bypass Surgery. His blood sugar was 300 today, because he has not been given his insulin for the past two days. This is unacceptable! His nurse this morning told him his chart says he’s diabetic, but doesn’t say he needs insulin. I’ve just emailed your facility. If anything happens to my FIL because he did not receive his insulin, your facility will be held accountable.
