My husband was in Creekside Transitional and Rehab care. I was very impressed with all the help and concern of the staff on hall 5. I appreciated that he was in a one person room, the quality and quantity of food was good. They took notice of his needing a special diet for swallow purposes. The rehab therapists were excellent. He had PT, occupational, and Speech therapy. They also arranged for Home Health to send a therapist for follow up PT and speech. My husband came to Creekside from a Memory Care facility under a stressful situation. It was hurried and I was not prepared for the need. I was dubious about the quality of the facility my husband was going to, as I was not the one who chose the Skilled Nursing Facility. There was no time for research on my part. Though this facility is old, compared to others, I could not be more pleased. It is clean and the staff friendly and professional. I'd like to give a special shout out to Nurse Erika! My husband, who is living with advanced dementia, is being well looked after. All of the various entities, from the Social Workers, Nursing, Therapists, Dietitians, Activities Director and Transportation have been in regular communication with me. In observing how the caregivers interact with those needing their services, I've seen nothing but kindness and respect from them towards those they are caring for. I visit my husband daily at different times of the day and evening. Because he falls frequently, he is in a wheelchair and placed where there are eyes on him. I recognize that no place is perfect. I also recognize that my husband has not been there long. But I met a lovely lady yesterday, who has been a resident at Creekside for five years! She is there due to mobility issues. She said that she has been very happy with the staff and the care she receives at Creekside. So if anyone is looking for a Skilled Nursing Facility for a loved one, I would highly recommend Creekside in Meridian. Should things change, I will update my review.

I was very well taken care of at Creekside. The occupational and physical therapists literally got me back on my feet within a week. Ashley, Kathleen, Star, Lola, Erica and Rosalinda come to mind for going above and beyond. Too many other names I’ve left out.

The staff is what makes this older facility truly shine. They are amazing and kind. I take a plethora of medications and they were on top of them. They also worked hard to keep my pain in check from the moment that I arrived. While I was there for a problem related to my total hip replacement, many were there with memory problems. I watched as these nurses gave round-the-clock true care to those who could not say thank you. I have insomnia, so I saw at all hours how helpful and kind they were. My stay through the holidays was actually made joyful, not just bearable. Thank you so much, Creekside!!!

In this difficult time, the care team really did go above and beyond to help my family navigate next steps of what life looks after his hospital stay. Whoever you have working your front desk and answering the phone, needs an attitude adjustment. Every time I call regarding patient care she is rude. I called your facility @ 3:30 regarding a patient who needs a ride and your front desk staff hung up the phone. I called back and she said very rudely "yeah I already told the transport team" and HANGS UP THE PHONE AGAIN. I was in disbelief after that interaction; I work the front desk at my clinic, and no one should ever experience her attitude.

I can't tell you the amount of love and respect that I have for the staff at Creekside, they have treated my dad like one of their own. Even after he checks out, I have to make sure I say goodbye to them, they feel like part of my family. I always feel they are giving him top notch care, keeping him safe and trying their hardest to help him improve and move forward. They have been a huge part of my life for over a year and they hold a very special place in my heart.

They did an outstanding job taking care of my 80 year old mom with dementia. The staff and I all worked as a team and communicated almost daily. They even helped her eat after I expressed my concerns about her inability to do so.

My husband was in the long term care side of the facility for many years. During that time I observed that his dietary and medical needs were not being met nor were his mental health needs met. On numerous occasions, when the call button was pushed we would have to wait for a long time for someone to respond to it. I think that he was allowed to 'give up' and then do what was necessary to essentially starve himself to death. I don't know if there is someone on staff to address the mental health needs of the patients but if there is then there was no assistance given. I also don't know what the job is of the social worker that is on staff to know if that was the person that should have addressed my husband's mental health needs. I know that my husband might not have opened up to anyone even if they had tried. And it might have been that by the time they might have tried it would have been too late. I think that he needed psychological help a long time ago to help him accept what his life was going to be. But even in my visits, because he didn't open up to me, I didn't see what was coming until it was too late. I think that at the end, they only gave him the minimum amount of care needed to take care of his immediate needs such as providing him with his medications and some sort of liquid meal alternative and changing his diaper. On the plus side, the alternative menu was amazing. I would not recommend that facility to anyone.

Creekside has the best activities. From bingo to crafts, entertainment and black jack you never be bored. Food is pretty good and you get decide what want to eat! I enjoy going outdoors and sitting outside my room. The staff is pretty good too!

My Father recently rehab at Creekside Transitional Care for several months. The staff at Creekside are professional, caring and listens to their patient's needs. The physical therapy and occupational therapy teams are excellent. When he entered, I had little hope that he was going to be able to get his strength back, but the therapy team worked with him and now he is stronger than he was when he entered. The nursing staff is also excellent, including nurses and med techs. I highly recommend Creekside for your loved ones.

The CNAs and nurses and therapists are great. But dietary is a joke. They never got anything right. I'm allergic to peaches but even though it says on the slip they just ignored it and kept giving me peaches. If I go into anaphylactic shock and even if I were to push the call button and if the CNAs are behind I would be dead. Otherwise I would have given 5 stars.

Office staff were friendly and knowledgeable when I inquired about my appointment. During my visit I felt very comfortable and in good hands. One of the staff in particular, Lindsey, was exceptional. I recommend this office to all my friends and will be going there for all my dental needs.

Mari, the first person who answers the phones and is the Administrator's secretary is rude and uncaring towards the residents who have to call this place home. I, a resident here, have had to call the facility's main number to request help inside of this place when I have been unable to get a nurse. Many times Mari has repeatedly hung up on me and has refused to talk to me, or will just let my calls go to voice mail. Once I was trying to get help for a resident that was in distress and she hung up on me . After the situation was resolved by someone else I confronted her about it and she informed me she was just too busy to take my calls. I have unfortunately have been confined here for a year and a half with what was originally a fractures femur. While here I developed Covid which lead to a heart issue that has extended my stay. Hopefully I will be leaving soon before they read this review. The physician and mental health care is lacking! Doctors visit once a month if requested; Nurse Practioner once a week if requested. Mental health provider - none. One who asks if your meds are working..sporadically. The nursing staff is great! Unfortunately a few have came through that have been lazy trouble makers. They are constantly short staffed. Social Sevices Director Tayler is new and I already see improvement in that department. Kitchen staff is great and do what they can with the menus and products that they are given to work with. Activities Department needs some new ideas and more money in order to provide quality activities for all residents. About 80% of the residents have dementia should have more care than is being provided! They are often left to sit and just seem to be forgotten. The trouble here I believe is administrative! Administrator walks around like an ex line-backer and I have never seen him taking the time to talk one-on-one with residents! And Mari needs to find a line of work that she enjoys because she definitely hates this one! If I ever get out of here I will fight tooth and nail if my family ever tries to get me into here again! I DO NOT recommend this place!!!

The nurses are great. Food is good. Activities are fun especially the ones Madison and Taylor create for us. garden club, and women group always have something new all the time. nurses all well trained and very kind. Kitchen has an alternative menu so we always get something we want to eat. I would recommend the therapy. When I first came here I had a broken hip and could not walk. With their help I can now walk. 5 stars from me.

The Director The Director Of Nursing Their Door Is Always Open If You Need Them, Or Talk. Good Choice Of Nurses And CNA’S They Try To Be So Pleasant Even When Busy. And A Good Response To My Alarm On 300 Hall When I’m In 401. I Always Introduce Myself To New Ones And Establish A Good Repore And They Know What My AIarm Sounds Like! I’m Pretty Self Efficient Even Do My Own Trash And Water At Night. Just To Help. Love My Room And Sometimes I Make Everything With A Smile! I Am Vice-President Of Resident Council And Going Well lol Everyone Pretty Much Knows Me Since I’ve Been Here The Longest! They Like Me And Feel They Can Talk To Me!! Such A Blessing To Them! I Try To Be! This I Hope Is Like Go Between And Helpful Before Having To Go Any Further And If So We Will Know Who And When! My Son’s Are So Appreciative To Creekside And When They Know They Can Call Director Trent And Speak Such Kind Comforting Honest Words! I Reccomend Anyone Should Please Come Ser Creekside It’s Like 1 Big Family Oriented! Thank You Creekside To Each And Everyone For Caring And Always Taking Good Care Of Me!! When I arrived at creekside I could not walk. I am now walking with a cane, I am in above the knee amputation I have a new prosthetic leg , my therapist McKenna is the most professional individual I know he takes time to make sure that I am adequately prepared for my next step the nurses and CNAs at creekside, are professionals. The people in the administration treat me with the utmost respect. Since I've been at creekside I was elected president of resident council , I would recommend creekside transitional care to my family members and friends that I know

during the short time my wife was at this place , they lost her purse, lost her clothes didn't answer her buzzer for help, she was forced to stay in dirty diapers sometimes up to 3 plus hours which left her backside raw and open sores, they hardly ever answer the phone and never call you back, the outside of building is in need of a lot of work, due to lack of maintance .they have resident people answering the door. this place is a joke !and should be closed down !

Had to remove brother in law via ambulance 911. Tried to tell staff ambulance was coming, they would never answer the phone ambulance got there, staff never did pick up. Care in the home was sub par. Brother in law lost over 20 lbs in 2 weeks. final straw was when he fell out of wheel chair after telling staff he was slipping. so happy to have him out of that place.
