We visited Copper Trace in December for the Grinchmas party held for residents. It is a beautiful facility with very nice employees. The residents were really enjoying the party. The food was fantastic, and this facility was so inviting and ultra clean. Top notch!

I have felt generally well cared for at Copper Trace. My day nurse, Jami, has been wonderful. Her cheerful attitude makes the day much better. She seems to be very knowledgeable and competent. Another nurse, Chad Streeter, serves with common sense and a caring attitude. He got the ball rolling to get my insulin reduced to avoid low sugars during the night. THANK YOU! Another employee that I appreciated is Alayna. Her very cheerful, energetic personality really made the days so much better. She even takes the time to write uplifting messages on the ice water cups! If the dining service makes a mistake, she does her best to make it right. I enjoy every time she comes into my room. The therapy department is one the things that makes this place great. I looked forward to going to therapy each day. Unfortunately, the dining service prevents me from giving a perfect score. Most food is cold when delivered and needs to be heated in the microwave. For one week I never received menus, even after a nurse, one therapist, and 2or3 servers requested menus for me. During my first two weeks it was a guessing game as to what would show up on my tray. Most of the food actually tasted good, but it would have been nice to get what I actually wanted. Wouldn’t hesitate to come back because of the people. I will just make sure I have plan B food in the refrigerator.

1. Mu mother went into Copper Trace rehab after surgery on a broken hip. Before surgery she could walk and the surgeon said with proper rehab, she should be able to walk again. My mother has dementia, is blind and deaf (if not properly charged) in. Every other day when I visited my mom, her hearing aids weren’t charged so my mom laid in bed blind and unable to see. The rehab department attempted to work with my mom at times without charged hearing aids. Can you imagine being blind, deaf, with dementia and in a new facility with someone trying to touch you without understanding who they are or what they are doing? In those cases she would get extreme anxiety - so rehab came to the conclusion and reported to Medicare that she was unable to be rehabilitated. They sent this recommendation to Medicare without discussing it with me. The rehab director simply sent me an email and said my Mom was coming off rehab and that she needed to be moved into a paid room. I strongly disagreed with the rehab director and appealed to Medicare twice but was denied because of the strong letter Copper Trace wrote to Medicare telling them that my mom was not able to improve and was unworkable - Which is directly in contrast to the Surgen saying that with good rehab, she should be able to walk again. So my mom laid in her bed confused, blind with dementia and unable to hear due to uncharged hearing aids, unable to get up and go to the bathroom by herself or without the help of two people for the rest of her life, which was only five more months . My mom was not incontinent, but because it was too much work and according to rehab too dangerous since my mom could not see and was unable to walk and afraid - rehab instructed the nurses and aids to require my mom to go to the bathroom in her brief. I could forgive a lot, but making my mom go to the bathroom in a brief because they did not want to use two people to take her to the toilet is unforgivable. I wrote a letter to the director, expressing my concerns and she sided with rehab. So I took her to the doctor a few months after her surgery and asked the doctor to prescribe rehab, and so Copper Trace made a feeble attempt to restart rehab. They didn’t understand that my mom would get extreme anxiety if her heating aids were not charge because of her blindness and dementia. The level of care of the aids was excellent but two months after my mom was in the facility, the the lead nurse of the section stepped down and for the next four months, her area was without a head nurse. - prior to that the head nurse made sure everything that needed to happen would happen. She would even attempt to take my mom to the bathroom by herself and check to make sure her hearing aids were charged . And got her a shower chair. The attention to detail and the needs of individual residents completely disappeared without a head nurse. The head nurse was wonderful and they never did replace her. At 10,500 a month I would expect a discount or a temp head nurse but instead four months after her death, they’re still sending me threatening a $62 bill every month for a special mattress that used in her last two weeks of life. To pour salt and the wound, my mom had her own wheelchair that she paid for, and when I went to clear out her room, the facility had taken it thinking that it was theirs. They didn’t even wait one day to take the stuff out of the room that they thought was theirs and they have so many spare wheelchairs it got mixed up in the crowd, and I never got her wheel chair back - however, the threatening $62 bill keeps coming every month. Oh, one other thing because my mom couldn’t see she needed to be fed and many times they didn’t have time or resources to do that, so my mom lived on protein shakes the entire time that she was there. The facility could be so good if they were adequately staffed with a head nurse and used an outside rehab company that was interested in actually doing rehab versus getting them off rehab after a few weeks so that they could move the patient to pay rooms.

The staff employed at this senior living facility is in my opinion what makes it above any I’ve seen. As a social worker, I have had years of experience with my patients being placed in different short term and/or long term settings. Witnessing the staff interact with them here at Copper Trace is what really makes the environment feel like home. LPN Brittany B. specifically treats her patients like they’re her own loved one. It’s genuinely from the heart while providing top notch care at the same time. I applaud the employees for going above and beyond and showing each individual so much love.

I moved my mother into Copper Trace assisted living 1.5 years ago. We were nervous about this move because mom lived in her home for 45 years. She is now well adjusted and happy in her new home. I am certain that the reason for this is the people who work there. The nurses and aides have been fantastic. Specifically, Wendy and Shelly (nurses) and Brooke (aide) have been great. They are kind, hardworking, dependable and approachable. We could not be more appreciative of their care!

From one Nurse to another, Britney B is hands down the best nurse. I would give her a Daisy award if i could. While visiting i noticed she takes exceptional care of her other patients and my grandmother. When i was watching her work I noticed her patience light up in a way that every nurse should strive for. She was patient, kind, thorough and thoughtful. However, i felt the facility was either understaffed or the Nursed patient ratios may need to be adjusted. Despite the heavy workload Britney managed to keep her cool all while prioritizing my grandmother and other patients care.

I’ve had more than one experience here at Copper Trace but the gal I really rely on is Brook K. She helps me out when ever I can’t do something, like getting my seasonal clothes out from under my bed. This is definitely a no no for me. One call and she will be here to put the carton back under the bed. She has a great personality and always has a smile. She makes life much easier for us.

Copper Trace Rehab is by far the best rehab facility in the area! The care my mom has received from RNs, CNAs, Speech Therapy, PT and OT has has been excellent. Tammy May CNA, Chad Streeter RN, Miguel CNA and Shelby S RN are just a few if the staff that has been so great to my mom. They are professional, caring, responsive, and so patient. I know she receives great care when I am here, and I know when I leave her for the day she is in good hands! Dietary has been great too! The food is delicious! The facility is beautiful and spotless as well. Copper Trace Rehab is truly the best place if you or your loved one is in need of PT, OT and/or Speech therapy. All of the therapists, nurses, CNAs, dietary and cleaning staff have been very attentive, professional, caring and patient with my mom. She is doing great thanks to all! Shelby Myers RN is her nurse today, and is AWESOME! Jojo is her CNA and is so sweet to my mom! She really does a great job!

My father-in-law is in the memory care unit at Copper trace. We have been extremely pleased with this facility. The staff is friendly and they treat the residence with respect. They are quick to address medical questions or concerns and we have been very impressed with their level of occupational and physical therapy. He especially enjoys the meals!

My best friends mother lives at Copper Trace. I went with my bf last Wednesday 4/17/24 to visit her mother and was very pleasantly surprised by what I found. I have always had a negative idea about senior living places and that was corrected during our visit. What I found was a young nursing student named Alyson who gave some much time and attention to my friends mother. I expected to see the staff treat her like a number like someone who didn’t really matter and yet this young girl treated her like family, like a friend. She came in the room and you could tell right away how much she cared. She spoke to her in a way you could tell that she has always listens and remembers what is important to her. My BF mother smiled and laughed with Alyson and it filled my heart with a new sense of what Senior living can be when you have the right people working. She is going to make an amazing nurse!!! Thank you!!

Overall good experience. Therapy was great! There were times call response was slow, however when they were made aware of issues, they did address them. Just needed to make them aware. Menu choices for food sometimes mixed up but again they were willing to fix issues when requested.

My mother stays at Copper Trace for almost 5 years. She is taken very good care of. Many staff members know her habits. They do all they can to make her and other residents’ stay as comfortable as possible. Among staff members who goes above and beyond are nurses Jyotsra Dhakal (LPN), Heeline Emi (MA), Jo-Jo Glascoe. They like their job, do it diligently and in good faith.

All the staff is very kind and caring. They treat you like family. They take pride in the cleanliness of the facility. All the staff in the rehab also do a great job helping me get my independence back and preparing me for home. Choose wisely, and make certain that you are constantly there to advocate for your family memberWhile my spouse was there, he was injured by a staff member who was performing a procedure we were assured they were qualified to perform. He ended up in the ER and struggled with pain over the next couple of weeks. While negotiating a rate for an extended stay for him, I was told by their finance manager (of all people) that I was exaggerating his situation. She shared with me that he was only on pain medications for one or two days, and if he told me otherwise, he was not being forthcoming. In fact, the Director later confirmed, when confronted, he had been indeed been receiving pain medication’s for almost 3 weeks. I expressed concerns, but did not file an official complaint with the state of Indiana at the time. That was a mistake. The facility is close to my home, and I didn’t want to make them angry so that I could have the option of bringing him back if needed. I would, of course, allow no further procedures, and would try to keep a closer eye on him. After a recent hospital, stay requiring rehab, the hospital reached out to Copper Trace to locate an opening in one of their facilities (Cardon). I was willing to do so as again facility is very close, and the procedure he needs on a monthly basis could be performed by his specialist. The hospital was told that my husband was not welcome at any Cardon facility. I give them two stars only because the Rehab team was exceptional, the social worker. Amber was exceptional, and there were a couple of nurses and aides that went above and beyond. I did reach out to the Moore family specifically, as this is a family owned business I ultimately did receive the message they intended to send. Don’t complain, don’t advocate for your family member.

We were struggling to find a home for my mother until we visited Copper Trace. Ashlee and Jaye were amazing and helped to make the transition seemless and smooth. I can't say enough about how all of the staff had been amazing! Always cheerfull, accommodating and professional. My mother is thriving, making new friends and participating in all of the activities. We feel very blessed that mom is in a safe, comfortable place and is being well taken care of.

The facility was nice compared to some I visited. Absolutely loved their residents. Very clean. Their ADON Zack is very disrespectful to ppl. I was personally talked to terrible by him. He was the only person I ever encountered that wasn’t pleasant.

I was in the facility for rehab follow a very serious full knee replacement which became infected. I can't say enough good things about the cleanness of the facility as a whole. The staff, in rehab, the nurses that closely monitored my recovery, and the support staff that keep my room and especially the bathroom very clean did a great job. If you must find a facility to take care of yourself or a family member you will not find a better group of people or facility.

Unable to get nursing staff to return calls to me when I had concerns about my mother's health that needed checked in to. My mother's shower was delayed 4-5 days until the administrator intervened and asked someone to follow through. No intake interview was given when my mom entered the facility. I was not listed as her primary contact because of this, and they did not notify me of her Covid results and room change. I have never experienced communication issues...lack of communication...like this ever before with a medical facility. Physical Therapy has been good, but I do not feel as though she is altogether safe at this facility outside of P.T. I will be pulling her out a few days early because of this.

My dad has been in the rehab part of Copper Trace since Thanksgiving. When touring different facilities it was abundantly clear that Copper Trace was meticulously clean (didn't have that "facility smell"). During the tour, their staff was also very joyous and they continue to be! The admissions director was instrumental in getting my dad's insurance approved, insuring for a seamless transfer from the hospital to Copper Trace. He has had some excellent nurses, that really take an interest in his needs. All of the staff personally interact with him and challenge him to do his best and get involved. You see the same faces on a regular basis, insuring the continuity of care. The therapies he receives have made such a huge difference in his progress. Medically, they are addressing all of his needs in a timely manner. They even have transportation to appointments if needed! My dad says the food is fantastic too! I couldn't be more grateful that this part of his journey is going so well. Very thankful!
