My grandpa loves being here, and that gives our family so much peace of mind. He's made some great friends, always talks about how good the food is, and his room is always clean. The staff has been so kind and caring, and his therapy is going really well. Liza is incredible! She is soooo helpful, knowledgeable and sweet. She interacts with the residents from such a heart centered place, and we couldn't be more grateful. Liza gave us a tour of the facility, answered our questions, and warmly welcomed and oriented us when we moved my father-in-law in. When we realized he had missed dinner and was not able to get food himself, Liza immediately told me exactly whom to ask for help. Her clear, confident direction made sure he received a sandwich that night. Her warmth and willingness to help made a stressful transition SO much easier for our family. Thank you, Liza!

This is not a facility that you want your family to reside in. I will say immediately, that some of the staff work so hard they do their best and out perform many of their co-workers. Miguel Green, is an amazing worker, a great inspiration and the best example of caring man. Shannon, poured love and dedication in to my mother everyday that she worked. Her kind words and patient attitude helped my mother to gain the confidence she needed to walk out on her own. Tony, was such a ball of energy. He encouraged and added motivation to my mother's mind to help her continue to increase her confidence on her own abilities. Angelica, was wisdom by my mother's side everyday that she was there. She made her laugh and not keep her mind on her pain. She was quick (as she could be) to handle what she could for my mom as necessary. Trina is an angel sent down from heaven, she was like an old friend to my mother and made her feel comfortable and listened to her needs and concerns. I am grateful for the staff that showed up everyday and did their jobs above and beyond what you can ever ask for in this circumstance. Now as for the facility itself. Any person with a title, in the administration, in this place where's that like a badge of honor but it means nothing, They do not communicate well with the patient/clients. They do not speak to the patient at all. They avoid conversation, even small talk for easy and confort. They do the very basic, I am talking the absolute minimum. Do not give detailed information that is important to the patient/clients care, stay, personal needs and medication, rehabilitation needs or expectations, or transitional care. I do not even know why they have a dietician. They met with my mother three different times with her stay and they never met her dietary needs not once. The communication is non-existence in the facility. My sister physically filed two complaints about the diet that they had my mother on, the concerns where never addressed, no one that received the complaint ever reached out to confirm or deny any wrong doing. My mother is on diabetic medication but she does not have to monitor her blood sugar or take insulin. They poked her fingers everyday and gave her insulin on their own say. Pertaining to the diet. If my mother had friends or family bring her food from outside the facility her blood sugar would be between 100 and 130. Whenever she ate the food they served her in the facility her blood sugar was 150 of higher the highest being 209. This is unacceptable. When my mother had completed intake into the facility, they has her on oxygen, it was set at 7.0 when she was supposed to be set at .5. it took two days to correct it. The staff can only do so much but the administration are the ones that facilitate what is to happen and how the care is to go. PT/OT staff do their best to work with a persons own ability and what they are told to do in their daily work assignments. My mother had a few doctor appointments while in this facility and they did not setup the proper transportation at any time. She was hurt in the transport, she was hurt at the facility. My mother is a plus sized woman and they nearly dropped her off the hoist they didn't have it locked down before moving her. Some staff members used the wrong size belt when moving her she was scared, nerves and consistently worried about her care and safety. They come in a few weeks ago and told my mother that since she was not better she will be transitioning to long term care. The person said they will take her SSI and only leave her with $100.00 of her own and she will be in the facility indefinitely. My mother refused and then they had her discharged from the facility in one week. She is not able to close her right hand ( she is right handed) she is not able to walk long distances without help or supervision. My mother needs help to shower and to prepare her meals. They told her three days before her discharge that she will be leaving. They did not send a referral for home care assistance, they didn't tell her she needed 72 hr. notice for transportation. Only one PT staff made sure she went home with a basic walker. I know this is long but I feel this all needs to be said, the biggest and most egregious situation is the cooking staff, night staff and weekend staff. They have a certified chef on staff, but he is only allowed to clean he can not cook. The food is disgusting and I would not feed it to animals, every cook looks like they just got out of prison and served the food, cold, burnt and inedible, with a messy or slop like presentation. Making people feel like they are not even cared for, or respected. The night staff and the weekend staff do not perform the duties even subpar. They know no supervisors or administrator are going to do some surprise inspection. They sleep on shift. They do not do rounds at night. My mother didn't get turned on the night shift or the weekend shift and she developed bed sores. The night staff and weekend staff would not clean them or change the dressings. This facility need a major overhaul not just in administration and finance. The long term staff that actually do their job should be promoted and placed in the position to help upgrade the facilities standards of care and expectations of patients and clients as well a staff. This is a facility that has a contact with the VA. I would never let a veteran stay here. When I was doing research to find a director to contact I came across the BBB and 7/09/2025 Canyon Transitional Rehabilitation Center has filed for Chapter 11 Bankruptcy, they still have the ability to operate but their finances are monitored. That is not the only thing that needs to be monitored. I want to give this facility double zeros. Thank you for reading this all the way to the end. I have spent a lot of time in many facilities, and this one is by far the best in every way. The building is always very clean, and it even smells fresh and welcoming as soon as you walk in. The nursing staff consistently goes above and beyond for every patient who comes through the doors. The therapy staff is just as dedicated, working hard to help patients progress and feel supported throughout their stay. The teams communicate and work together so well that most patients I have seen are able to meet their goals and discharge home. I would highly recommend this facility to anyone needing skilled care for themselves or a loved one.

While I was in rehab, two people really stoled my heart. Henrietta and James. The love, compassion, caring with the residents received from Henrietta goes above and beyond. She always had a smile, made the residents feel comfortable. Henrietta is one in a million. She always made everything fun. James is a resident there. He is such a wonderful person.i was happy when I went to activities and James was there.he would make activities fun, make me laugh. The staff was always wonderful to me. If an older family member needs rehab, this is the place to choose! Jon-Jon here, and I share my name because they helped me remember why friends and family call me that. I walk more confident. Falls are dangerous. But their programs help those accidents. Walk in and the front desk has a young lady with a smile of warmth and sunshine. The staff of many are great and extremely helpful. Activities have a Light to brighten your day! Luz is 200 watts of warm light. Hiland is a big dude with an even bigger heart! When you see him you will know what I mean. There are many others. Eric, Rotellio, Dinah is not in the kitchen. The rehab crew has a Tony the tiger saying " your going to be great!!! Again!!!" Many others!!! I give 5 stars because they deserve it! Home now, but I now have an extended family forever. Patients and caregivers. I already miss you! Will visit soon! If you or a love one needs care, this is the place to choose.

After a new hip surgery my 89 year old mother spent 3 weeks at Canyon Rehab Center. We were extremely pleased with the care she received. The concentration on her rehab exercises were consistent and promoted her mobility. The staff to include the PT personnel, nurses, and techs were all extremely friendly and made a real effort to make my mother feel safe and comfortable. We had a great experience here!

I recently spent three weeks at Canyon,, and I was super-impressed with the care I received. The professional medical staff is competent and attentive to patient input, explaining decisions they make re. care. The aides are wonderful -- pleasant to even the most demanding patients and providing immediate response to requests. In the "gym", the physical and occupational therapists tailor their agendas to individual as well as group needs. The meals are extraordinary for institutional food, and the kitchen staff is responsive to individual requests when possible. Activities are popular to long- and short-term patients, since the activities personnel have a great sense of humor. The front office is capable and also attuned to individual needs. I found it a 5-star facility.

My recent experience (end of August, 2025) with Canyon Transitional Rehabilitation Center was extraordinary. I arrived there one late evening after an accident that left me with a head injury and an inability to walk. I was in pain, distraught and feeling totally helpless. My fears were soon relieved. I was welcomed, immediately cared for and comforted. Everyone was kind, professional and caring, The facility was old but immaculate and well maintained. I cannot thank and praise everyone there enough - from the skilled health workers, production staff, PT and OT, to the cooks and meal providers. They are all indeed "angels in disguise!" It was my good fortune to be in their care. Thank you, thank you Canyon Transitional for giving me my active life back!

My dad stayed at this facility for about two months. He was just discharged a couple of weeks ago to have surgery. Most of the staff seemed good, but there were a few newer employees who almost mixed up medications a couple times with him. My biggest issue was with the kitchen because despite my father being on a particular diet, they could not get it right. I spent time speaking with the Dietician lead directly and he assured me they would get it done, but nothing happened until about a week before he was discharged. It took them over a month and a half to get his diet right and we ended up filing a grievance because of this before he left. For patients who have had certain types of surgeries, the correct diet is critical and the kitchen did not live up to expectations. It probably wasn’t the worst place, but definitely not the best. The head nurse Lisa was great. Everyone else was kind of standoffish. We will be looking for a different facility to put him in when he gets discharged again.

Canyon Transitional rehabilitation center has done a phenomenal job of taking care of My ex-wife. All of the staff from the physical therapy and OT to the kitchen. Staff have been phenomenal. The nursing staff has been excellent and couldn’t ask for better care. Dorian Billy, the Director of the business office did a great job of making sure that Brenda’s needs were met and the insurance was taken care of completely. A very close family friend of ours is currently staying at your Montgomery and Pitt location. I saw hi for the first time today and was surprised to find out that he knew who I was. Upon my initial visit of my loved one at Canyon Rehab, I was so happy to see she had recently finished her therapy for the day. I was promptly advised by the staff of her progress in therapy that had just completed. She was still in the therapy room where there was much therapy going on with the other patients and she was in front of the TV amongst all the interaction and not cooped up in her room alone as she was at the previous two rehabs. I was even more delighted that Canyon allowed her therapist who has treated her for the last 6 years to visit and share his insights with the other therapist regarding my loved ones mobility, past care etcwhat a blessing. My grandfather, Mr. Kersting, was here for the last year of his life. I cannot begin to explain how amazing the staff is here. Carrie, Sharone, Yvette, Mario, Joe, and the 400 hall were his favorites. They took amazing care of him physically and emotionally. They listened to his stories, they laughed with him, they loved him, they held his hand when he needed, and most importantly they made sure he was comfortable and happy every step of the way! Thank you all so much for everything you did for my Poppy and the peace of mind you brought my family.

I was pleasantly surprised at how this place was run. I have had dealings with Genesis in the past that I haven't been able to give even a 2 star review. My Mother was taken excellent care of. (Except maybe a soiled diaper not being changed in an appropriate time frame.) The staff was kind and compassionate. I would choose Canyon Rehabilitation again for my Mom if it ever became necessary.

My experience with Canyon Rehab was excellent. I was transferred to the facility after a hospital stay for a fall. Upon arrival I was given a through exam by the assigned nurse. The following day I met the facilities doctor, dietitian, as well as an inhalation therapist and was examined by two of the physical therapists. The facility is divided into hallways. Each hallway had a 24 hour monitor. The monitor checked my room, about every hour or so and asked if I needed anything. My room was thoroughly cleaned every day. At specific hours of the day and night I was woken and issued required medication. They also arranged for monitored showers. Mail was delivered to my room. Laundry facilities and hair stylists were also available. My meals were prepared as specified by the dietitian. The meals could be had in the dining room or brought to your room. On Fridays 30 or more of the patients were brought to the large dining room. We were placed in a circle and the physical therapists encouraged the patients to participate in light physical exercise. At first I thought it was a bit corny, but realized it was a good way for the patients to get a bit of exercise and socially interact. The physical therapists are absolute professionals. They determined the needs and abilities of the patients. I was impressed that no matter the physical or mental short comings of the patient, they were treated with the utmost respect. Each week I met with the physical therapists, the nurse and social worker to discuss my progress. Prior to my release the social worker arranged for an appointment with my principal doctor and home visitation. I was given my medications and thoroughly briefed on their use and my safety on returning home. I was in rehab over the Christmas holidays. On Christmas the staff walked by the patients rooms singing Christmas carols and give out small gifts. Quite a nice touch. The only downside to the facility is the really poor internet.
