I would give this place 10 stars if I could but I guess 5 will do. From the time my mother stepped into this place they have addressed all of my concerns quickly and made sure my mother was very comfortable in her stay. This review is long over due. My mother broke her femur on Dec 22. After surgery and a hospital stay she was sent to The Pavilion at Creekwood. We didn’t know what to expect as this was a first experience for our family. The staff couldn’t have been more caring. Keeland and Lynette, 2 of the PTs, were especially noted for their positive and supportive attitudes. When Mom lacked confidence, they gave her theirs. Cynthia, her weekday room aid, paid extra attention to Mom’s needs and went overboard for her comfortability. Saudi, Mom’s evening nurse, was simply PHENOMENAL. She made a point of greeting her patients when she arrived and said goodbye when she left. She worked beyond regular hours making sure her patients were settled and comfortable before saying good night. Her room was cleaned twice daily in addition to being tidied if she needed it. I never worried about Mom when she was there because I knew she was in caring hands. Mom became friends with the staff and was sad to leave.

I think this place is very good. The worker is very kind and friendly house keeper are good clean. they take care my father very good. my father has been here a week for therapy he has broken hip he has therapy 5 times a week The therapist are good too. Very nice place

One night was one too many My mom was recovering from a bacterial UTI and was too weak to move back home. In addition, she contracted covid almost 6 weeks prior and was still testing positive. Because of that, her options were very limited for nursing homes who would accept her. I received a call in the morning from the Pavilion at Creekwood and they confirmed she would be arriving in the afternoon. She arrived at 4 PM. Despite it being a Friday, her room had not been stocked - there were no blankets, towels, hand towels, washcloths or hand soap. The bed had not been made. The gentleman who transported her from the hospital put the only sheet on the bed. She did not receive towels and a blanket until 8:30 PM. There was zero orientation. And the “food” she received certainly can’t make anyone feel better. The room was disgusting and had not been cleaned. More on this later. The air conditioner beside her bed was caked in filth, blowing directly on her. There was even a shopping bag left behind with trash and personal effects, proof the room hadn’t been cleaned or prepared. At my first visit, the receptionist reprimanded me for not following procedures instead of offering guidance. Her only help was a dismissive wave of the hand when I asked for directions. While I was moving stuff to her room, not one staff member acknowledged me in the halls and there certainly wasn’t any assistance. Most of the staff were on their personal phones or computers. Even the halls were disturbing. Residents were left to wander aimlessly with no stimulation or dignity. That still haunts me a year later. The paperwork I filled out allowed her to opt-in to manage her own medications. However Saturday morning, the nurse told my mom that she was not allowed to manage her own medications. Not only that, but the nurse actually yelled at her about this. When I arrived Saturday afternoon, her only trashcan was overflowing. As I removed the bag, I noticed there was feces inside the small bathroom trashcan. Obviously this is not an ideal situation if you’re recovering from a bacterial infection! If that’s what was visible, I can only imagine what wasn’t. This was the final straw. We then made the decision to leave. I made multiple trips moving her stuff out. It was less than 24 hours later and not a single staff member noticed. The real kicker is the final pass. I escorted my Mom on her walker, straight out the front door and didn’t look back. No one even noticed! When I called from home to say we checked out, the nurse said they had been looking everywhere for her. If that’s the case, why didn’t either of us receive a phone call? The staff seemed subpar at best. They were rude, unhelpful and indifferent. This place is hell on Earth. I cannot fathom living out my years in these conditions. If you have any shred of dignity for your loved ones or even your enemies, don’t send them here. Supporting pictures can be seen on Google

My sister-in-law has been living there for past 4 months. She loves it there and our whole family has been very pleased. Heather in the finance department along with the front desk have been so helpful. Pavillon is well taken care of and very clean.

My father is currently in the Pavilion and I would like to praise the excellent staff that have helped take care of him. Debra at the front desk is exceptional as well as Gene who distributes medicine and both Freshair and Boomie, two of the wonderful hard working nurses. Leaving your parent to the care of others is very difficult but these woman, as well as many other staff at the Pavilion have made this much easier for me. No place is perfect but I can say without reservation that these people are exceptional!! I have recommended the Pavilion to others and in fact now have other friends whose parents are now residents at the Pavilion. They are also happy with the care given to their loved ones. Laura Bickel

The rehab team at The Pavilion is awesome. They are all so positive and upbeat. I was wheelchair bound when I got to The Pavilion and I walked out of there with the aid of a walker. While I was in the gym doing my exercises, I watched as they worked with the elderly patients and saw the wonderful care they took with them. Anu and Lynette, who were my primary therapists, worked with me 5 days a week. They were encouraging, patient and supportive of me through this whole ordeal. I always felt safe. They never had me do any exercise that they did not believe I could do. They are true professionals who I now consider my friends. I was at The Pavilion for approximately 2 months. I was in a wheelchair and relied on the nurses and aids for almost everything. The nurses and aides are professional, caring, hardworking and dedicated to their patients. I want to give a special shout out to my nurses, Hilda, Gloria and Ms. B. and my aides Cynthia, Nicki, Guy and Job, who were my main caregivers that took wonderful care of me.

I have been favorably impressed by the care and services provided in my time at the Pavillion. In particular, I am grateful for the rehabilitation staff. The team worked to allow me to recover from a broken leg. The experience was pleasant and professional.

Due to Medicare rules on days allowed, I arrived at the Pavillion from another rehab facility unable to walk/stand, feed myself/shave/brush my teeth. 100 days later, thanks to my dedicated/beautiful therapists Anu and Lynette I am almost ready to go home. Medicare limited me to 100 days at the Pavillion with Anu and Lynette, another 30 days and I feel confident my early morning rehab team would send me home with a walker for my mobility. Miss you Ladies and Thank you again.

This facility is the second one we had to put my father in. We toured it before placing him there. It smelled great, it was clean and smelled cleaned. The staff was nice, professional, and welcoming. We decided to place my father here, we visited daily at least twice a day. Whenever we visited my dad was always cleaned, room clean, and he looked happy. The nursing staff, technicians, kitchen staff anyone that had a hand in caring for our love one was top notch. I can't say enough about the positive experience we had. My family is greatful and thankful Please keep up the good work!!! Thank you

My friend is at this facility. I helped move her from the facility she was at, due to less than perfect care. This facility is so much more of a positive blessing for her to be there. She absolutely loves her aides. I've read all the negative reviews and wonder how could they even be true?? We have had nothing but a good positive experience there! Therapy department is AMAZING! Her nurse is the BEST! I get it! No one wants to be in a facility, but this is what has been handed to us in life. I just wonder did all of these bad reviews even speak up with their issues? I'm thinking no. Just write a bad review about it. All I can say, is everyone from housekeeping to dietary, etc have been wonderful! I'm so glad she agreed to move to this facility. We have truly been blessed!

I recommend The Pavilion at Creekwood if you need to get a little stronger after a hospital stay! Staff here is great! Food is really good and you can tell they care about what they serve you! Care here is great! Nothing but positive things to say!

I was recently released from from here. I had to stay on the skilled nursing rehab side. I will tell you that if I ever need rehab again or long term care. I will be returning here!! This is the cleanest facility I have been in not one time did I smell urine or feces going up and down the hall. The aides do an amazing job of making sure that the residents are up and in their chairs every day!! They have a great activity program... EXTREMELY wonderful nurses and attendants... And I only had one or two MINOR issues and the ADON took care of them IMMEDIATELY she is fabulous. The therapy team is wonderful they have an excellent gym. The kitchen people are even extremely nice and friendly!! So again if I ever need care again I will go here and if you or your loved one needs care putting them here would be the best best decision you could ever make!!!

The facility is absolutely stunning! The leadership team is truly exceptional, showing genuine care for the residents and the wider community. They go above and beyond to support the loved ones who entrust them with the care of their family members. Moreover, the business office manager is remarkably knowledgeable about financial matters and offers a wealth of resources to help guide and support every admission or potential admission.

when I called to see if my mom had arrived from the Hospital they told me "no" when in fact she was there. I called to get her discharge started, they never called me back. Slow to respond unless you owe money. Quick to discharge when insurance runs out.

The place looks really clean.... It doesn't matter how things. Look, it's hell the doctor's a nurse's treat their patients. I've been up here every single day with my mother for over a week.And not once have I been impressed with any of them. Not just what I see with my mother, but what I see them with other patients. Most of them should not be in the medical field. The people who just assist! just helpers have a more bedside manner than anyone else.

I would like to thank the administration office and nurses for taking such care and concern for my mother’s year long stay at their nursing home. She shifted from skilled nursing to permanent resident soon after her falls and strokes, but they made the entire process as good as possible. Since her passing they are still helping me through all the final details. I appreciate all they have done to help me through her sickness, sadness, and passing with each step showing respect for her and myself.

I have been amazed at the recent changes. Staff more responsive and attentive. Rooms smell better and place looks good. My Aunt has been here for over two years, very impressed on my last visit. She is very happy here, would recommend.

My father has been at Pavilion over a year. I visit him 4-5 times a week. I believe the care he receives is good, he is mostly confined to bed because of multiple strokes. He must be bathed in bed, etc. I do his laundry so I see that his clothes are changed daily. He loves to be clean shaven and the CNAs shave him which he loves. The food is not bad. His breakfast is usually more than he can eat - 2 eggs, toast, sausage, oatmeal, milk. There are some stand out caregivers that deserve a shoutout - DK, Guy, Travis, Damari, Helen, Voomie, Erin in PT, Debra (front desk), Tim (all things handy) and so many more but I Am bad with names. Will you see some less than stellar reviews? Sure but I spend a lot of time visiting. I have seen that Elder care is not easy. Some patients are not nice and some family members are not nice. There is a special place in Heaven for the good people when choose elder care as a career. Are there some days that seem more chaotic at the Pavilion? Yes but after watching the ebb and flow for the past year I have learned that things happen - multiple people may call ill making the care givers spread thin, sometimes a patient may pass away taking extra time, or a patient may fall taking extra time. I feel over all pleased with the level of care my dad receives. Anytime I have expressed concerns they have addressed them in a timely fashion.

the place is not clean , I asked for a broom so I could sweep her room .I also had to clean the bathroom my mom was supposed to be on a kidney diet which she did not have . also she had catheter which was not connected to the bag so when I got there she was sitting in her chair with pee all over her. they told us she would have therapy daily which she did not get. kept asking to speak with a social worker called many times, when ever was there and ask to speak with her they told me she would come to the room but she never did. pls pls don't bring your family they do a good talk and photos of the place looks good but once your family member is here it is a different story. my mother put her dentures on her food try and they threw it away by accident and did not pay to have it replace . I walked in the all way, and saw a man in his wheelchair with his pants down to his knees sliding out his chair and no one came to help him , it took awhile to find someone to help this poor man . was so sad to see how the people were uncared for. then I thought about all the people The Who had no family members to look out for them . my mom was there for about three weeks and I came almost every day and, she was not properly card for . I can only imagine what happens to those with no family. pls people look at the reviews and take it seriously .this is not a place for someone you love.

My mom (Shirley B.) just completed her most recent rehab stay at The Pavilion at Creekwood. She and I have both been very pleased with the wonderful care and physical therapy she has received each time she’s been there. While I do not have all of the names who provided such great care to my mom, we would both like to give special thanks to Adrian and Sydney for their exceptional service in overseeing my mom’s care, and to Anu and Carly for the outstanding physical and occupational therapy they provided. My mom said without you all, she wouldn’t be back on her feet and safely at home today. All of your efforts are greatly appreciated! Thank you so much!

My brother is currently a resident here and receives wonderful care. The therapy staff is excellent and his nurse Tabitha and CNA Sherry and Travis are beyond excellent. The staff works very well with us and they truly go above and beyond to see to his every need.

My father has been at the Pavilliom for nine months and my mom and I have had hundreds of interactions with her...she always goes above and beyond to make us feel welcome and cared for! Nursing staff and PT are wonderful. However front desk lady is by far the worst. She was highly disrespectful to many who came in. Not a people person AT all. How she speaks to others is not friendly. The lady on the weekend however is WONDERFUL AND HELPFUL. Does not speak to you disrespectful at all. Made me feel 100% welcomed. In this kind of facility the first person you are greeted by makes a world of difference. The lady during the week doesn’t make you feel at all welcomed. I’m thankful this lady didn’t go near my loved one.

My grandmother was most recently discharged from the Pavilion after her completed rehab stay. I would highly recommend this facility to anyone in the community. My grandmother received exceptional care, the food was amazing and very "restaurant style", management was extremely helpful throughout this whole process. Thank you again Pavilion at Creekwood.
