Reviews Bear Canyon Rehabilitation Center

Excellent care. I especially want to recognize Michelle Alarcon in the business office at Bear Canyon. She was always very kind and professional. I had lots of questions over the three years my loved one was a resident and Michelle was beyond patient and knowledgeable! I also want to recognize Justine and the staff back in memory care. Such a loving staff that truly cares about the individual needs of the residents.

A big thank you to the staff for assisting my mother. Our experience was exceptional. The staff was courteous, compassionate and cared for all of our needs.

As a medical social worker, I work closely with patients and families who need skilled nursing care, respite care, and support during some of the most challenging times in their lives. Finding a facility that provides compassionate, high-quality care and gives families peace of mind is incredibly important. Bear Canyon has been a wonderful partner in helping ensure our patients receive the care they need. The staff is caring, professional, and truly dedicated to both the patients and their families. I especially want to recognize Geordyn and Channing. They are exceptional to work with”responsive, knowledgeable, and always on top of every detail. They go above and beyond to make the transition as smooth as possible, communicate effectively, and genuinely advocate for the best interests of the patients and their loved ones. I highly recommend Bear Canyon to anyone seeking skilled nursing or respite care. It’s reassuring to know our patients and their families are in such capable and compassionate hands.

Tristen and Julia are some of the most attentive staff members I’ve met at any of the facilities I’ve been too. They are both willing to go above and beyond for any of their residents. Their actions clearly show how much they care and how great they truly are at their jobs. I would not hesitate to put any of my family members under their care! Keep doing a great job advocating for the folks you take care of!

I recently came to visit a friend who lives here, and I was really impressed. The building was clean, the staff and residents were friendly, and my friend seemed genuinely happy with the care she was receiving. As someone who works in nursing homes and knows what to look for, I pay attention to things like cleanliness, the way staff interact with residents, and the overall atmosphere. This facility made a great impression on me, and I would feel comfortable recommending it to others looking for quality care for a loved one.

Check in to visit our patient friend was easy. Once you are in their system, subsequent visits are even easier to check in. Their factial recognition system is quick once in their system. Check out is just as easy. Staff were present as we entered our friend's room. Our patient friend was just concluding being served her lunch meal. Following lunch, the staff assisted in repositioning the patient in her bed and made her comfortable. It warms our heart to know she is receiving the care she requires while she recovers from her injuries.

I rate this at 3 stars due to care given to the patient and the facility itself. First, Bear Canyon is impressive when you walk in; very clean and Nurses workstation is organized. Scan system at reception for security. When you check in they provide a nice marketing packet of the facility and services offered. My 92 year old Mom had hip surgery and this location was recommended by the hospital. Since I have a medical directive (my Mom had dementia) I signed paperwork and got her checked in. Marketing material discussed meals, rehab services, showers, etc. She was assigned to a room by herself. She was there for 10 or 11 days and after 7 days they finally gave her a bath. The Nurses Aide asked me to help as there was no one else. It brought tears to my eyes to see feces falling off my Mom in the shower as I washed her hair. I had asked repeatedly about for her bath. The LVN or RN who delivers the meds didn’t exactly do her job either. My Mom was on Anti-Anxiety medication if she doesn’t take it her mood is confrontational. I asked about her Meds and the Nurse said my Mom didn’t want them. No call to me or her doctor to advised that meds were not being given. She had been on these meds for years so you can’t just stop giving them. I know she had rehab for an hour or so a couple of times while she was there. She had hip surgery, could not get up or walk on her own. Yet they left her in the bed with no rails, no ability to get help and the walker was at the other end of the room. Call bottom was always tied to the side and hanging away from her. Phone was disconnected after first day but they had said she would be able to get incoming calls. Her bedside table had drawers that would not stay closed, never repaired and to me could have caused a trip. It brings me no pleasure to write a bad review but I was there everyday to visit her. This review is based on facts. The bottom line, they should not have accepted the transfer as they are not equipped to handle patients with dementia. I transferred her back to her Care facility after 10-11 days. I do feel Bear Canyon hindered her recovery and did not help her.

i would like to respond to the review that said her love one was at fiest park. my mom is currently there and I am desperately trying to get her out of there. that place is awful. leaking roof in the center of where the residents walk. torn privacy curtain . Anthony, the new nurse caring for my grandmother, has been an absolute blessing. From the moment he started, it was clear that he truly cares about his patients, not just professionally, but with genuine compassion and heart. He goes above and beyond, pouring his energy into making sure my grandmother is comfortable, respected, and well cared for. It’s rare to see someone make such a positive impact so quickly, and we’re incredibly grateful to have him. Bear Canyon is lucky to have someone like Anthony on their team! Thanks.

I was pleasantly surprised when visiting my Mom in the memory care unit. It was was clean my Mom was clean just finished eating lunch & she said she liked the food. The Activity person comes throughout the day to invite my Mom to activities & when she doesn’t want to go they do an activity in her room Desi the nurse was especially welcoming and showed me around she is a gem! I met the new Administrator I believe she will fulfill the vision of turning around the facility & making the improvements needed. Overall my Mom was calm & content

With the exception of one nurse imparticular, the staff are so kind and comforting. My mom is at Bear canyon, and has a couple more weeks to be there. She is comfortable and getting better. She was at Fiesta before, and that place needs to hire better people. If your loved ones need to be in a rehabilitation center, Bear Canyon is a very nice clean place. And the food is actually good!!!

I am so grateful to the Business Office staff at Bear Canyon! Along with some specific other CNAs, Nurses, and PT staff. They always go above and beyond to make sure our family needs are met. They are respectful, caring, and even though short staff, they make sure the residents are taken care of and their needs met.
April 2025
I am a former skilled unit cna and the caregiverfriend of a patient here and I actually feel pretty comfortable with the situation when I leave each day. I've had the staff go above and beyond to help me and on their own make her feel more comfortable . All of her personal belongings were accounted for upon arrival. The specifics are recorded and followed well and the management team definitely tries hard to make the friends and family of their residents at ease and make them feel heard.

Leaving someone you love in a new place called home is never very easy when you want them with you. But when it became too difficult to tend to his needs, Bear Canyon was there to help and so far so good. The staff is welcoming and really that made a world of a difference. They understand the transition is emotionally draining and they were supportive but gave the space we needed to settle in and by morning had everything prepared for him and a game plan set for success. Despite the reviews I have come across on this facility, I choose to be optimistic and guarantee their performance based on our experience for today. The place was very clean, they are understaffed but they are understanding and address his needs to the best of their ability. Most importantly, my husband is happy for the first time in a long series of facility stays

This was my first day visiting a new patient here, and I was impressed with how everyone was interruptible to help me when I had now idea of how I could get some service. And the few things I asked for were addressed immediately or very quickly, despite none of them being a particularly time-sensitive issue. It seems that all such places receive mixed feedback about the level of care, from those who visit friends or family, and maybe people who are seeing patients with more serious problems will see it differently because the person I’m seeing is in pretty good condition and probably won’t be here long. But in my few trips to get assistance and on my way in and out of the building, I also saw several interactions between staff and more challenging patients, and I didn’t witness anything that wasn’t reasonable or compassionate. Off to a good start.

My stepmom was transferred to Genesis from the hospital last night. I visited this morning and found that she was comfortable and happy. The facility was clean. Residents were up, moving around, and interacting with each other. My stepmom had a roommate and she was so happy to have someone to talk to when we are not there. The nurse, Edward, was extremely patient with me and helped me fill out all the required paperwork. I feel very comfortable that we picked the right facility.

I can’t say enough about the staff here at the Juan Tabo location. I’m happy with the care my mother has received so far. It’s been 4 months of consistent care. Every staff member is extremely caring and supportive toward their patients and family members. Michelle in finance is the best. If you need insurance help, she will assist you every step of the way. She does it all. If you have an elderly loved one do not stress about how they will be cared for because this is not just a rehab facility. Bear Canyon has long term and memory care. To the entire staff here, thank you and God bless all of you. Everyone here has become like family. I thank Genesis with my entire soul.

My mother was in care there during the period February 9th through March 1 2024, at Juan Tabo. The care she received was outstanding and a great help restoring her to health and mobility. We wish to thank all of the staff for their contribution in letting her be restored to the normal life she was used to in everyday life. The stay there at your facility was a great help in getting her back to a normal life. There were several staff members which went beyond the normal and caring for and helping her heal and to get back home. I want to say that these employees did outstanding work, and stood out above all the people we were in contact with, in our recollection. Jenna Vickie - days Monique - nights Edward-days Jason-therapy We appreciate all of their hard work and the hard work of the others that we were helped by.

We had a very good experience when our mother spent 3 weeks there until this past week. She had fractures and was very weak when she came in and the therapy department was awesome. Jason, Noah, Brandon, Alexis, Deanna, Brian and a female speech therapist whose name I can’t recall at the moment were sooo competent, friendly, respectful and encouraging to mom. She regained her strength quickly. She also had a wonderful nursing assistant named Vicki who went out of her way to help, and nurse Eddy was very good, friendly, professional and I liked how he explained the medications to her and what they were for, instead of just handing her pills. Mom also really liked Monique on nights. We had no bad experiences and I recommend this facility. If I needed rehab, I would choose Bear Canyon.if I left anyone out, I apologize.
