I have had my mother in respite care several times at Evergreen Commons. When I bring her to Evergreen, I leave knowing she is in very good hands. All of the staff I've met and observed have been attentive, courteous, pleasant and professional. They have excellent attention to detail and even remember her name even though months go by between visits. They communicate effectively with family members by keeping them apprised of any changes , issues, or even just with reassuring updates that every is going well. The same extends to the administrative staff. Jenn, the business office manager, and Heather the intake manager answered my questions promptly, made the process smooth, and went the the extra mile to explain options and details.

Evergreen Commons proved to be a haven for our family and our mom as she needed end of life care. The staff assessed her needs and made changes accordingly on a daily basis. The staff provided her care with dedication, skill and compassion. They were also attentive to the needs of our family members. The facility was easily accessible and we appreciated the outdoor courtyard which provided our family with a peaceful setting when we need to take a moment.

Gracious, friendly greeting at the info desk by Dawn. Pleasant clean halls. No smells. Loved the sunny windows both in our friend Tom's room and the corner movie craft rooms. Quiet and relaxing. Breaks our hearts our friend is here but at least the place and people are gracious.

I love the location of Evergreen Commons Rehab and nursing home. As I enter into the front door I felt welcome without anyone addressing me. The place is so peaceful, clean, and organized. The staff were very respectful and courteous. I love how they treated the the patients that is there or the clients that is there. A very beautiful place breathtaking location.

Evergreen was beautiful, very clean, smelled fresh. Activities were going on. Rooms are bright and cheerful. Residents look happy and very clean. I would recommend Evergreen very highly. I’m not sure about price because resident is there for rehab so insurance will cover. I wouldn’t hesitate to stay there.

Although they are chronically understaffed the aides do a good job & seem to really care about the residents. Unfortunately they have nearly everyone in a wheelchair for mobility & decreased fall risk, they stop walking residents that need assistance. Sometimes the wait to use the bathroom has been 1 1/2 hours. Many complaints about the food (chicken,pork & mixed fruit cup) being terribly repetitive & many times cold. Oversized mattress (air mattress) does not seem to have correct size fitted sheets available. Many different reasons have been given for these issues. After being a nurse for many many years, I do sort of understand some of these problems. They do an excellent job keeping the building clean.

Set my mom there about a month ago for rehabilitation and they have done a great job with her. facilities are clean ,staff is friendly and my mom really seems to enjoy being there. She is almost ready to make the transition to long-term care and she seems to like it, so I think this will be where she will stay.

I was very pleased with my visit to a dear friend who is in the rehabilitation program at Evergreen Commons. The grounds are well cared for and the building is neat and clean and welcoming as I entered for a visit. Receptionist staff we’re busy talking with other people so I had a bit of a way to check in, however as I found my way down to my friends room: I found the halls to be decorated with attractive and contemporary artwork and encouraging Plex. The activities room is bright, colorful and stocked with puzzles and activities resources for residence. Residence rooms are spacious and sunny and have a window that looks out on a green area. My friend and I attended an outdoor cookout with musical entertainment activity for residence and guest. Food was good and well prepared. I would give the Evergreen comments an excellent rating and would recommend to friends. May God bless the staff and the residence there Mrs. Russo.

Very happy with the care my mom is receiving. We moved her from assisted living to Evergreen a few weeks ago for long term care as her condition declined (she's 96). We had an awkward arrival but I'm so pleased with her ongoing care. All the staff are wonderful with her. I also appreciate that they call to update me on changes to her condition or medications. And mom actually thinks the food is ok!!

When my mother first arrived at Evergreen Commons, I had some concerns about her care. While the facility is clean and most of the staff are very pleasant, they face the same staffing challenges that affect skilled nursing facilities nationwide. Unfortunately, this meant my mother wasn’t receiving the level of care she needed, which prompted me to reach out to the administration. I’m pleased to say that the administration has been very responsive to my concerns, and my mother’s care has improved significantly since our conversation. I recognize that healthcare workers in these settings face tremendous stress and that the ongoing shortage of healthcare providers creates genuine challenges for facilities like this one. I’m grateful for how quickly the team addressed the issues and turned things around for my mother.

The staff today were phenomenal! The area where my family is, Is closed off due to Covid. The nurses were very accommodating in terms of supplying Clorox wipes, And allowing us to go into the snack room to fill up glasses of ice and water. They were kind, Understanding and you can tell they were all working very well together as a team to get patients fed and comfortable. It’s a very positive atmosphere, which is good for not only the residence but for the family that come in to visit.
