I stayed in this nursing home for several years after my stroke. I got along with all of the staff and all my needs were addressed and met with compassion and care. Rikki in admissions is a rock star and always went above and beyond the call of duty.

Brookside has been a great partner to work with our Hospice Team, The floor staff are always helpful and friendly, the house keepers and custodians are fantastic and do a great job keeping the facility clean and neat. The DON Andi does not hesitate to put a hand out to help and goes above and beyond with customer service, her phone is always on. The Admin Stacy is wonderful and responsive. We recently received a call from a family member who lost a loved one there, she reported that Dignity Care Hospice Team was phenomenal and she couldn't have gone through losing a loved one without the team, she also recognized DON Andi at Brookside as well who took the time and looked up everything she wanted to know on the computer amd was so patient. Dignity Care and Brookside make a Great Team!

A long over-due big shout out to the administration, nursing staff and all the wonderful aids. My husband, a Vietnam Veteran, was admitted mid October 2025. From a smiling face who welcomes me each day at sign-in to constant conversations with the head of nursing and to the good folks on the nursing/aides team, my husband has received excellent professional care. It is a testimony to Brookside’s leadership team that has created such a positive environment. It shows each day through the good folks who care for residents with such grace and devotion to them all. It is very reassuring to know that when I cannot be here, there’s an entire team of professionals who always have my husband’s comfort and well- being at the heart of their efforts. I will never be able to thank Brookside enough for all that they are doing to help our family navigate our very difficult journey.

If you have an organization that is looking to do volunteer work the fourth floor of this facility could really use some cheering up. The staff are friendly but completely overwhelmed. Brookside has taken on way too many patients and don't have near enough staff to provide adequate care. The building itself smells terrible and is extremely dirty. Luckily my dad was only here a few weeks. I feel terribly for the patients who live there long term.

As a 44-year-old unemployed college graduate having lived with while being full-time caregiver of and finally recently within the past couple weeks had no other choice than to place her disabled 72-year-old mother, now suffering from clear signs of dementia into a long term nursing home, please let me assure you that the team running this place has nothing but their best to offer your loved one and you. Due to my mother's recent health issues of the past 2-3 years, she has been admitted short term basis to virtually every nursing rehab facility in our state of Kentucky that would accept her medical insurance at the time; recently, after having been rejected long term service by every previous place based on her likely dementia-influenced behavior and no facility referred to that she hasn't been a patient of accepting her current insurance, the only chance left to find long term care was somewhere outside our state. Though the process of placing her in a long term nursing home continues to be challenging, especially considering it isn't in our home state and is requiring more effort than usual on my part to get all that must be done on behalf of my mother accomplished, I genuinely could not have been blessed with a better place or team of people to partner with than this. My contact (Rikki) at this nursing home has compassionately gone above and beyond to be there for me every step of this difficult journey Mom and I are on, so I am wholehearted at peace knowing my mother is receiving the very same tender loving care and confident anyone else under any difficult circumstances they may be dealing would be too. Disclaimer: I am not a paid spokesperson for Brookside Healthcare Center, simply very appreciative of their services as the youngest daughter and former full-time live-in caregiver to a loved recently admitted current resident. Rikki has been a blessing in helping us find care for my cousin. The staff was friendly and helpful as soon as we walked in the front door. The provided a meal right away and it was very hardy and appeared fresh. Through her admission and placement process, I spoke with Rashiid multiple times. He was so helpful and a friendly face when you walk in.

I’d like to give some well deserved recognition to Rikki Perkins. When my parents was under her care she went well above & beyond! When I first met her she was covering the front desk she had such a warm & welcoming presence that made me feel okay with my Parents being there. I had two parents at the time with Alzheimer’s so it was very hard at first. Soon after Covid happened and when the Governor put the lock down into effect it really changed our Lives. We was at the Nursing home at least twice a week to not being able to see them at all it was one of the worst times for me & my family. But Rikki would take a lot of her personal time to go up to there floor and would FaceTime us so we could see & try to talk to our parents, she would make sure they would be at the windows on the forth floor so we could show our signs and wave at Mom and Dad, she would send us pictures and cute videos of them at times, If we dropped things off for Mom and Dad like clothing or food she would make sure they got it. She made a tragic situation, not so bad because of her personal commitment to helping us. We will be forever grateful to her. I would recommend this place to anybody, especially as long as she’s working there she was truly heaven sent!

Rikki was great. She helped us make the decision to put my uncle in the facility for short term rehab. The therapy department was very attentive and really pushed him. He’s currently back home in his own apartment doing well and if we ever need a placement again, Brookside would be the choice all over again.

I can't brag enough about the quality of care I got from this facility. From the moment I came in I was greeted with nothing but smiles. Immediately after I got to my room the social worker came in introduced herself and so did the rest of the team. We talked about my goals and returning home. The transition from the hospital to the nursing home was seamless. I really enjoyed participating in activities and getting to know the other residents. Miss Vanessa I think that is the activity director name anytime I asked her for anything she got it immediately for me. Their therapy department was top-notch and they never let me give up. I would highly recommend this nursing home. Very top tier

My mom was there for a whole 7 days and I brought her home. It's ok if you're not able to care for whoever on your own, but if you can don't send them there. Nice people but I'm glad a law has been passed that family members can put cameras in loved ones rooms now.

Recently the wifi has been turned off for the patients here at Three Rivers Rehab Center at 7800 Jandaracres Dr. I do a lot of business with my device and it's costing me 75.00 a week for for extra data. I haven't had a valid explanation as to why this service was taken away from your clients, the rehabbers and residents deserve better treatment than that.

All these previous reviews are a joke. My mom been there over three weeks and still isn't in the directory. I visit and it takes hours before anyone checks on her. She has to wait until high noon to be bathed for the day. Most of the patients don't receive one on one personal communication from aides. I've had to walk a patient back down the hall who was trying to run away. They don't provide ambulatory services therefore several doctor appointments get missed. There's never a social worker when needed. My Mom will be moved soon as possible. They have two aides to tend to a whole floor of patients that's horrible. It maybe a great place for others but not my family.
