The Care Center’s entire staff, from the doctors, nurses, aides, physical therapists, occupational therapists, social workers, dietitians, recreation department, support staff, cleaning & laundry staff were very approachable, courteous, and attentive. They treated our mom and our family as if we were their own family. They always went above and beyond to provide compassionate, attentive, and professional care. Nurses and staff are attentive and it's evident that they truly care about the patients and try to go out of their way to accommodate requests. Everyone was always positive and smiling, which is rare these days. The building itself is rather old and somewhat outdated with an "old building" smell to it. Food can be hit or miss, especially at dinner time. Overall, it was a positive experience and I would recommend this facility to those in need.

My 100-year old mother stayed here for three weeks after a hip surgery. We both are very satisfied and extremely grateful to the personnel of this rehab, from the aides and nurses and cooks to the wonderful PT and OT specialists, doctors, social workers, and the entire administration staff. Everyone has been very gentle, caring, polite and helpful in any possible way. We greatly appreciate the way these people do their very hard work and wish every one of them the best!

My aunt was at the Forest Hills Care Center for over a year before her recent passing. Ms. Enandra and Ms. Judy were absolutely wonderful with their expertise and guidance in making sure the services my aunt needed was provided. The entire staff - the nursing team, the rehab team, and supporting staff members were very professional, friendly, and took care of my aunt's needs. This is a safe and caring place. I had peace of mind knowing that my aunt was in good hands!

My father in law was a patient here for about 6 weeks recovering from pneumonia. From the start he was welcomed and given excellent care. Shout out to Connie in admissions, and Judy in case management, as well and Enandra and Troii in SW for managing his stay and discharge seamlessly. During his stay his nurses and aides were very caring. Thank you Inna, Jill, Rose and Leeki(spelling). My mother was a long-term resident of three years before she passed recently. The care she received was excellent on all fronts--medical, PT &OT, dietary support, daily care. The nursing staff is superb--skilled, compassionate & explained everything going on as she declined in a manner that a layman can understand. The social work staff is superb; both for issues the patient has and for helping families with Medicare, Medicaid etc. The care center is immaculately kept. The food is good! I cannot say enough positive things about the FHCC. In my mother's last few days they kept her comfortable and were extremely supportive to us, her family, as well. Her passing there at nearly 99 years old was gentle and peaceful.

The facility needs a facelift, but it is clean and well run. Definitely understaffed and the many aides are unhappy and unmotivated. Management is very capable and takes care of things. Doctors and Nurse are excellent and caught issues and resolved them quickly. Food is hit or miss. Social workers are excellent and very helpful. This is not a hotel, it is a care facility and they do take care of the residents.

Dear Forest Hills Care Center Team, Thank you for the genuine care & support you provided to my father Viorel P & to myself during his stay in Nov/Dec. The entire team is lovely! Starting with Jake who was spectacular with the onboarding process. Connie & Judy in the office are professional, kind, caring & more than helpful. Judy went above & beyond to guide me step by step through changing insurance companies to best meet my father’s needs- her knowledge and unbiased opinions, were exactly what I need to help navigate me through that confusing process thank you Judy!!! The two lovely social workers were available at all times for any questions and concerns I was having. Special shout out from my father to the entire PT staff - he loved his training sessions which were essential for helping him regain his strength . Thanks to the entire team on the 3rd floor! Especially the evening nurses who I saw every night, they have so much to deal with and manage to do it showing grace & dignity to the patients. Many of the aides like Marre have been there for 30+ years and genuinely care about the patients! The experience of having your loved one enter into Rehab is always difficult- however having people around you who care & want to help makes all the difference in the world! ™ Gaby C

My grandpa stayed here for a little over a month after having a stroke. He’s also in stage 4 kidney disease, so some days were tougher than others ” but the team here gave him the encouragement and push he needed to give his best during physical therapy. They also made sure he always ate and days he had a hard time chewing nurses made sure to get him soft foods. He never went hungry ™ Because he’s a fall risk, they checked on him frequently and were always attentive. One thing that really impressed me was how respectfully they handled diaper changes. It can be such a vulnerable situation, and they had the proper techniques down ” they always made him comfortable, never rushed, never complained, and never made him feel embarrassed. He’s now back home, and the facility helped set us up with an at-home aide and therapist so he can continue his progress. We are very grateful for the care and support he received here.

I was only in the reha facility for a week, so I didn't know that much about the other parts of the facility. The aids and nurses are very helpful, they kept asking me did I need anything and help, and they were so friendly. I needed the medication for each four hours in a day for the whole week, sometime they were little late to give me the medication because of busy schedules. I was ok for that as long as I got my medication. The facility is open for 24 hours and the public transportation is close by, so your family members or friends can visit you easily. The social workers were very helpful too, one of them help me to buy the usb adaptor that I didn't bring with as I checked in. I believe it is a nice facility.

These types of facilities unfortunately have a bad reputation for how they treat their residents. My godfather stayed here for 3 months and the experience was very pleasant. He enjoyed the activities, therapy, food and staff. He even shed some tears on the day of his discharge. Social workers Enandra and Troi stand out as two of the people who truly helped him and advocated for him during his stay. I have nothing but good things to say and definitely recommend the Forest Hills Care Center.

My aunt was in Forest Hills Care Center for three weeks for rehabilitation. I was pleasantly surprised at this institute. It was clean, professional, everybody very helpful and conscientious. She was very weak at time of admission, not able to take care of herself. After three weeks she was ablate feed herself. I am very grateful and thankful for all the help. I wanted to thank each and everyone of you for the excellent care you gave my mother Margarita O. I realize she could be very difficult at times and all of you were subjected at one time or another to her not so nice ways, esp Lillian (so embarrassed) I am so sorry for that...I/we appreciate all the tender care you gave her, and I know in her heart she did too. I was completely unaware of what to do when she was admitted, TY Connie and Judy for making this stressful process seemless. Her Social Worker, Enandra is amazing. She helped w Ins, helped w more hrs w her aide, facilitating things she needed at home when discharge was happening. She went above and beyond TY!! The Nursing Staff, spot on! TY for being so kind, happy, understanding and so very caring. To ALL my moms CNAs, without your help, compassionate care and tons of patience you made this 3 month experience very pleasant. TY! I started seeing Dennis because of a recurring sports injury and was pleasantly surprised at how quickly he was able to relieve my chronic pain. He is a model healthcare professional who takes the time to ask about your day and to answer your questions. The care that I received from Dennis (and Kim) has given me relief from pain and allowed me to return to the level of physical activity that makes me feel like me. My grandmother spent the last two months of her life here. I visited her here several times before she passed. Most of the time, this place was heavily understaffed and most of the staff that was here had no clue what they were doing. The only reason I'm giving 2 stars is because there were a couple of staff members who went above and beyond (unfortunately, I don't remember their names) to make my grandmother comfortable. While this place isn't the absolute worst possible option, there are better options out there.

The staff here at Forest Hills has been great to my husband and I. They were very helpful throughout his entire stay. The staff here was easy to communicate with and never hesitate to perform any task that were asked. Thank you to Nurse Sliver, Joanne, and Yoni for all that you guys have done.
