I can't say enough about the care provided at Legacy Nursing Home in Pleasant Grove, AL. My nephew is 49 years old. He had an auto accident when he was 16 that left him paralyzed from the chest down. He cannot sit in a regular chair, nor stand or walk. He requires help to get dressed and undressed and a hoya lift to get to and from his bed and wheelchair. He has been paralyzed for 70% of his life. He requires more than average care to function than many people in a nursing home. I cared for him for the last 8 years after his mother died suddenly. His father died 25 years ago. He has no immediate family and has been at Legacy for the last 9 months. The entire staff of Nurse3s, CNAs, Aides, and Administrative all under the leadership of David Thornton make every effort to provide the special care he needs. I appreciate so much, people like Barbara Carter and Crystal Barber Charge Nurses, BreAnna Smith, Quintella Clopton and Irene Muetembei, Nurse Practitioners, David Jackson, Wound Nurse, all have his health and wellbeing first and foremost and work as a team to communicate. As well as Jessica Felder, Social Service, Kimberly Brickell Financial Director and Donna McCaain, Dietary Manager. This is not to say that everything is always smooth sailing 100% of the time. Life doesn't work that way. I have learned that caring for 150+ people with various needs, deadlines, priorities and feeding 3 meals a day is a daunting task. Legacy measures up! I also appreciate Mr. Thonrton's open door policy to meet with residents and family members. Sincerly, Robert E. McArdle

Legacy has a large outdoor area that residents can enjoy. Our friend who was there several years appreciated that area daily. She has transferred to a facility in another state to be near family and friends. Legacy continues to support transferring her information to her new facility. Kim Brickell, the Legacy bookkeeper has been particularly helpful and responsive. Jessica Felder who was the social worker at the time of the transfer was also very helpful.

Awful place. I only picked one star because I had to, it deserves zero. They don't respect elders at all.

My family recently had a family member stay there about three weeks. Things seemed to be okay at first. Most of the staff were friendly to many of the family members. They stayed informing the family members' children about how they were doing in the room. The therapists (occupational, physical, and speech) were great with our family member, and was able to tell us what they were doing good and not so good in. On the family member's third week there, they got sent out to a hospital in the area due to them going into respiratory distress. However, the children of the family member found out from the ER doctor and nurses some information from the EMT's that contradicts some of the what the staff at the facility said. You can image how the children and family members are feeling about it. Afterwards, one family member went to the facility to pick up the belongings, and once one of the children got their parent's belongings and went through it, that child realized that are items that are missing and shouldn't been used up in a three week stay. We are disappointed at the facility, but we do pray and wish them well. We hope the staff will step up and do better across the board. If you work there and take care of the patients, you should take care of them like your closest family and friends. Love of them, take care of them If you can find a place that will love and take care of your family member as they are your family, please that them there.

I cannot say enough about how thrilled I was with the love and care that my brother, Frank, received from Legacy Health and Rehab “ Pleasant Grove. I moved Frank to Birmingham after his dementia diagnosis just about the time the world shut down with COVID. As his health declined he was moved once before landing at Legacy. I was impressed by the low staff turnover; I saw the same faces month after month. I was equally happy that he was always clean and well cared for. But more than that, I was thrilled by the way he responded to the staff of Pleasantville when I was there visiting. It was obvious to me that the care and attention he received while I was present was matched by the love and care he received while I was not there. His reaction to the staff could not be faked or put on for my benefit. They cared about and loved him, and that meant so much to me. If I had to do it all over again, I would have moved him to Legacy earlier. He obviously got better care there than either of the two other facilities he stayed at in Birmingham. Thank you, Legacy, you are the best!!!

My mom is currently at Legacy Health and Rehabilitation of Pleasant Grove. After a week in the hospital, she was referred for rehab services for an additional time to recover due to experienced trauma. She needs therapy to help her get stronger and regain the physical abilities she lost before coming back home. I thought the transition from hospital to rehab would be difficult and disorienting for my mom but just the opposite, we barely waited to be helped when she arrived to check in. The staff were all very friendly and helpful. I especially loved that those who would be overseeing her care introduced themselves and really took time to make her comfortable. From the moment she entered everyone has been attentive to her needs, open and point on with her plan of care. They have graciously involved my mom and I in conversations relative to improving her health, physical wellbeing and care. After one-week, administrative staff set up a meeting with my mom and I with the Nursing Team, Physical Therapists, Floor Unit Management, MDS Coordinator RN’s, Social Services, Admissions, Restorative Nurse, Finance, Housekeeping, Dietary, Activities Team all of whom provided updates on mom’s current plan of treatment and care. We also collectively discussed options and how to prepare for the next steps after she is discharged. As her at home caregiver, I appreciate the Care Team for taking time to go over and clearly explain their assessment of my mom’s condition and treatment options.even more the patient and helpful instruction with her insurance. It was obvious from the tone of all those that attended the meeting their genuine concern that her discharge from a managed care facility be coordinated, smooth and safe. Caring for others is a big responsibility. Legacy Rehab is more than a skilled nursing facility, they have become family. I am so glad I chose Legacy Health and Rehabilitation of Pleasant Grove, LLC and would highly recommend it to anyone in need of short-term rehab services in an inpatient setting. OH..MOM raves about the FOOD!!!! HAT’S OFF TO DIETARY!!!!!

During this time of COVID 19 some of their staff could be a bit more considerate of how they speak and treat family members. Before, my experience had been really satisfactory. But recently some of the staff has fallen short of that respectable mark. We must do better we are all embarking on something we never been through. I can’t see my love one I’m expecting you to relay how she doing and no to seem annoyed that I’m calling and asking. I appreciate you all for the work you do and looking after my love one. But some of you need to examine your behavior toward the family. We’re counting on you.

Before we placed my dad at Legacy Nursing Home in Pleasant Grove, we met the financial team, some of the nursing staff, as well as the Administator. You could not have ask for any nicer people. They explained the entire process and asked if we had any questions. They led us on a tour of the entire facility. After my dad left the hospital, he was then given his room and was introduced to the nursing staff that would be taking care of him. He was reluctant at first because he did not want to give up his freedom which is understandable. They made him feel at home and gave him encouragement. He was practically bedridden except for when he was horiored out of the bed and into his wheelchair. He was given three hot meals a day and they gave him a shower three times a week and a sponge bath in between. He enjoyed the meals immensely and really enjoyed the company he shared with the staff. My dad was a real cut up and I think the staff enjoyed him as much as he enjoyed him. Thy even gave my dad a nickname....JoJo! Although he was only there for one week before he passed away, the end of his life could not of been any better. My dad was a preacher and he didn’t mind praying with anyone. He prayed with the staff and they prayed with them. My sister and her family visited my dad the day before he passed away and they had a real good time. He gave them a tour of the facility and was introducing him to the people he had met. I spoke to my dad the next morning on the telephone after he finished eating breakfast and had his coffee and he seemed so happy. After his breakfast they gave him a shower and experienced some shortness of breath, so they gave him some oxygen and he took his last breath and touched the face of God. They tried to revive him but was unsuccessful. My family and I have joy in knowing that he died meeting new people, sharing God’s love and praying with the new friends he had made and grown to love. We are so happy that he was happy. I love Legacy Nursing Home in Pleasant Grove Alabama and would recommend this facility to anyone I come in contact with who needed them. God bless each and everyone of them!
