Thelma, the administrator, provided outstanding leadership taking care of the staff and residents. She is a leader who rises above and beyond expectations. She was personally involved in my Mama’s health care decisions. Her administrative staff follows her lead to provide flawless work. My Mama was at Town and Country for over three years. Delia, the Nursing Director and her nursing and CNA staff provided phenomenal care to my mama Esther R. Colon before, during and after her transition from this life. Words cannot express my eternal gratitude and I will forever be indebted to all at Town and Country. Based on their relentless caring attitude, I personally recommend Town and Country!

I was at Town and Country for their PT, OT and memory therapy. I thought it was just a nursing home and was resistant to going. In 2 short weeks I gained strength and balance. I'd had my hip replaced and then a couple of bad falls. All of the nursing staff is wonderful. They are nurturing, prompt and very kind. Delia Martinez is the Director of Nursing. She has formed a team of very competent people. The fact that everyone works there for a long time speaks volumes about the staff. I will definitely go back and have them build me up again should need be .

My mom had fallen and she was taken there after a short hospital stay. She was very sore but nothing was broken, thank God. She's with the rehabilitation side and has physical therapy Tuesday-Friday. She's doing so much better and is soon to come home. Her insurance has covered everything so far. Im so grateful for them all!

This facility is Awesome! I had the opportunity to do a walk through visit to understand the quality and structure of a place for a family member. The staff and management were on point and answered all of my concerns! Highly recommend!

JULIE, hd of pt, as a person is fantastic , more important runs and excellent at department. My brother is making significant progress after backsliding at previous nursing homes. His room is cleaned routinely and personnel, Derek, a hard working nurse, is upbeat and knowledgeable.

I currently have a family member at Town & Country. The facility has been around for a long time. I researched the other nursing homes in Boerne, Tx and Town & Country by far has a better reputation for their care and compassion. The quality of care, safety, staff-to-resident ratios, the specialized sevices, and cleanliness of the facility was very important to me. Town and Country met ALL! A special KUDOS to the therapy department for working with my aunt with the disabilities that have hindered her. Because of their dedication, my aunt is now able to wheel herself around the facility independently. In addition, she also has become more vocal when expressing her needs with the help of speech therapy. My aunt is especially fond of Peggy the occupational therapist. My aunt tells me Peggy is her best friend, and for me she her guardian angel. The activity director, Leticia is simply an amazing and kind person! She ensures to include my aunt in the activities she able to participate in. Jennifer the dietary director, has made sure my aunt is meeting her specific nutrtional needs for her overall health. And she has always has been happy to address any changes or concerns with her diet with a smile. Delia Martinez, the DON is a kind and compassionate individual. I have seen first-hand how she makes it her mission to educate the nurses and aides to esure high quality care. Thelma the adminstrator is quick to address any concerns, and alway takes time to listen to me. Moreover, many of the aides provide excellent care! Overall, I have seen my aunt blossom in the year she has been at Town and Country because of the care of the staff and their dedication. So thank you all from the bottom of my heart!

It is with a heavy heart that I write this review. My sister was a resident at Town and Country for eleven months. She passed away a short time ago surrounded by family and staff. The care she received was from people treated her as if she was their own sister or child. My sister was only 52 and was special needs so she had the personality and child like innocence of a five year old girl. Having only lived at home until her needs were more than the family could handle we were all very worried and concerned. I installed camera’s in her room with permission from the facility. The cameras told a story we never expected. She was having a blast and making friends with other residents and winning over the hearts of the entire staff. Choosing to put my sister in a home other than the one we grew up in was the hardest thing I ever had to do. First, my family and I went and checked out every facility in San Antonio and the surrounding area. It was clear that we’d made the right decision. Unfortunately, my sister had many seizures and hospital stays which ended up leaving her weak and at the end of life. It was a comfort to know that she was in such a wonderful place with people who truly loved her! When I’m feeling a little stronger I plan to return to visit my sister’s other family. I can’t say enough how blessed we feel to have had found Town and Country. We’d hoped to be visiting for decade’s to come but God wanted his angel back.

The place was clean every time I went, but getting ahold of someone to schedule visiting my mother is almost impossible. My brother and I have tried on several occasions to call and simply talk with our mother but we always either get hung up on or put on hold til the line goes dead. Despite the facility being clean, the residents that require hygiene assistance do not seem to be. My mother was always dirty and her clothes always reeked, not to mention that all of the clothes that have been delivered to her seem to go missing within a few days, so she was always wearing someone else's clothes that were filthy. The staff I've interacted with are nice enough, but don't seem to actually care about their patients or the families.

The place was always so clean, and staff was good, also the main administrative lady was great! At first one staff member was stealing from my mom, but got fired soon after I complained. The physical therapy nurses they have there didn’t care for their job much and we’re always so rude to my mom, that made her feel like she wanted to skip physical therapy because the women were so mean.. If they would’ve replaced those ladies as well then i would of given it another star.. Other then that my mom liked it, and liked the size of the rooms, and the staff.. only reason she was moved is because I wanted her closer to home in San Antonio. But the facility overall was okay.

Has gone through some new changes but all for the better. Staff always make themselves available for any issues or concerns and the residents seem happy being there. Keep up the good work.
