My Dad transitioned to this facility last October and has been a resident of Cherished Memories (Memory Care) ever since. I have been highly impressed with the care that he has received thus far. Always looks happy when I see him. I truly do appreciate that every time there is a clinical recommendation for my Dad's care, the care team consults with me first to make sure I'm comfortable with any proposed changes prior to moving forward. Quarterly care plans are also an option, which I appreciate. My Dad's mobility isn't the greatest and it's been a relief to know that the offer onsite services such as: dentist, podiatrist, physician, physical therapy, etc. Highly recommend for their memory care services. Best facility in Plainfield and maybe even Hendricks County!

I recently had a stroke and am residing at Cumberland Trace. Since I have been a resident here, I have relearned how to use my left foot and even walked today in therapy. And thanks to my speech therapist, I can speak a lot better too. And I like the activities here, such as bingo, and I appreciate the church services; I feel very accepted. The people here have been really good to me and they really work with me, I am proud of myself and I have no complaints!

I spent a week in rehab at Cumberland Trace, the staff/ nurses/Dr/ were all excellent, the food was great, 3, meals a day always on time, if you wanted anything extra they would bring it, I needed prune Juice and hot Coffee each morning, as I had a bowel obstruction they cleared it at the hospital,we are considering renting a unit for the wife and me to live there, I’d recommend Cumberland Trace for Rehab or,a permanent resident, again all the staff, Dr’s and Nurses, and Rehab staff were great,

I will once my loved one is no longer there. Now that my loved one is no longer here, let me say that Tulo and Beatrice should not be taking care of sick lold people. They were condescending to our family, smart mouthed and downright mean. Beatrice actually told my sister that my loved one took too long to take medicine and so they would be last to receive theirs. This was already 9::00 pm. Also instead of putting padding on the lower tailbone for cushioning she stuck a bandage on the lower back. She must have thought we wouldn't check. And that's just to name a few incidents. This place is well maintained but if the owners aren't checking not only their nursing staff but the language barrier then they aren't invested in the care patients are receiving. If the family has a hard time understanding their staff, then how do the expect folks in their 90's to understand them. And then for them to be rude about. Then we were told if we needed to come back, to ask for a different ward. How about they get Tulo and Beatrice in check or better yet, get rid of them. Very disappointed. For all the good things, we don't feel like night time was a safe environment. And I forgot to mention, after 6 days they could not or would not get medication list straight. Even had the nerve to send us home with the wrong medication. Can't make this stuff up. And where is Emily this entire week while we are having trouble with medication. Never saw her

My mom was in here for Rehab, and I have to say, the care and love that staff has provided for my mom has been nothing short of spectacular! Words cannot describe how wonderful and clean this place is. My mom has only said great things about the nursing, and therapy. We were extremely skeptical of the services that she was going to get. But the team really treated my mom as if she were family. From the speech therapy to the nurses on the floor taking care of my mom. They all did a wonderful job! Mom couldn't have been any happier there. They even let my dad visit every day and he was able to eat and spend time with my mom inside her room. The love we felt as soon as we admitted into Cumberland Trace has been a blessing. I'm so glad we choose Cumberland Trace for my mom to do rehab!

Cumberland Trace has been taking care of my Dad since 2022. The care is fantastic. Apartments are nice and the cafeteria is great but what really stands out is the staff. The nurses always make sure that Dad is taken care of and is taking his meds. They do his laundry, help to keep his apartment clean and bring him meals when he doesn’t feel like going to the cafeteria. Staff will respond to me by call or text when I would like them to check on him when he doesn’t answer my calls. Cumberland Trace also has an in house rehab center, so when Dad does need extra care he doesn’t have to go to another facility. If you have family member that needs assisted living please check out Cumberland Trace, you won’t regret it.

I’ll always recommend Cumberland trace for a rehab stay! There’s great staff and a top notch therapy department! The LTC side of the building is beautiful! Semiprivate and even private rooms available with personal showers vs a shower room. The food is excellent considering the environment and they do a lot of cookouts, even stir fry some days! It is the nicest facility I’ve ever been in!

My mother has been a resident at Cumberland Trace for about 7 years. We were impressed with the facility at our first visit. The assisted living units are well laid out. Each contains a kitchenette, living area and separate bedroom. An added benefit is a washer/dryer within each unit. Warm, delicious meals are served three times a day in a cafe / bistro setting with attentive staff who take time to know and interact with residents. There are many social activities available to the residents. There are movies, outings, crafts, and worship services to name a few. The most impressive and important feature of Cumberland Trace is the staff. They are professional, attentive to resident’s needs and they keep family caregivers informed about any issues or problems. The team is very responsive to our questions, issues and concerns. This is important to us since we live in Texas and can’t personally monitor Mom’s care. Our request for information and / or help are always handled promptly (often with a call back confirmation). We highly recommend CT if you are looking for a place for yourself or a loved one.

my father was here last year for rehab and fell and broke his hip, but he he never pushed the call light button. the menu that they get are confusing because they are for the next days meals, i filled out my fathers meal tickets for him. the meals are great but sometimes i liked to bring my father a good old fashioned home cooked meal complete with plates bowls and flatware and enough food for everyone at my fathers table. the facility had no problem with that, the only problem that i have had that they couldn't fix was that they didn't serve diabetic meals but i found that most if not all nursing homes don't but this facility will at least let you bring food in for your family member but some places won't. great staff and caring, sometimes takes them ten minutes to answer a call light but its not often and they were all busy with other people, i know because i went looking, but that doesn't happen often.

Tonight my wife had pushed the call button before 8 pm needing pain meds. Still as of 10 30 still no pain meds. I saw their motto is that they treat you like family. I would hate being part of this family. Thinking about seeing if doctor will okay us coming home. We do better pain management. The second star is because the room is nice. And the aide was friendly. But don't have a painful hip replacement and come here Though there were troubles the first night the people did contact us about the incident. Her care became better. In fact they caught her infection problem early. Of which I am grateful. So each place you put your loved ones is scary. But a good place will work with you. I am sure this is the first place I would suggest. Just need to communicate

This place is pretty bad. No one knows what the other is doing. They can't seem to get the meals right - one meal they bring her purred then the next is a mechanical diet, they don't answer call lights, and work at a snail's pace. They start nebulizer treatments that take 15 minutes and don't come back to remove the mask from my mom for an HR and a half. I wIould not recommend this place to anyone. They are neglectful and have to be told to bathe my mom, and leave her without oxygen! Never seen anything like it!

This is the second time, my mother has been in Cumberland Trace. After a fall, in June of 15, we placed her at Aspen Trace, a sister property, in Greenwood. It was close to her Ortho, who is at Community South. When we were informed, that there wasn't a step down room available, and mom needed to be moved. Living in Avon, I just happened to stop into the newly opened facility, in Plainfield, and found out, they were a sister property. Having had such good luck with Aspen and staff being top notch, we happily moved mom. She left there last September, to go home. Fast forward. This past March, my mother suffered another fall and subsequent fracture, to the same side that she'd broken the hip and femur, the summer before. After a brief hospital stay and need for rehab, again, we were going to take her to Cumberland Trace. There were no beds available, which made us place her in an ASC, where the fracture became a break, by an untrained CNA, that tried to pull her over the arm of a wheelchair and back into her bed. I wouldn't have believed it, unless I'd not seen it with my own two eyes. Off to surgery, we went, again. After her hospital stay, I once again inquired about a room at Cumberland. They had one. Obviously, my mother, cannot return home and Cumberland Trace, was going to be her permanent home. But...things have changed, in that year. The staff, has gotten worse. For the same reasons, we left the other facility, danger, lack of communication, poor food, cold food (grilled cheese on untoasted bread, with an slab of unmelted cheese)response times(hitting the call light for bed pan and no one comes) go to the nurses station, to find them shooting the breeze or depending on time, no one to be found. Asking for a psychological evaluation and asking that you get a callback when the appointment is set, only to learn after you ask again, that it's already taken place and no one bothered to call you and tell you. Asking the doctor and NP, about antidepressants and told it would be with evening meds and they'd follow up in 2 weeks to see if it worked, only to find out those orders never made it to the nurses. Finding out that things get charted by people, but in actuality, they never did them. It's infuriating. We are private pay, in a shared room, at this point(because no one followed up on moving her to a private LTC room)even though, we had 2 meetings and have asked at least 2 more times. Leaving notes with the nurses for follow up and no one follows up. For the $6,000 a month, I write the check for on time, each and every month and the thousands, above and beyond insurance we've paid in a year and a half...I expect professionalism and follow thru. I expect the right hand to know what the left is doing. I expect the night CNA, to treat my mother with dignity and respect. I expect enough staff to properly care for the number of patients, in the facility. It's shameful, how there's never enough staff. That at breskfast, lunch and dinner, there isn't enough staff to man the dining room and get food to those that don't go to the dining room, before it's cold and disgusting. This is daily. The hair appointment missed, because no one bothered to get her there. The all is in the small. I go from person to person to person, for answers and get none. Where's the management? Where's the person in charge, to nip issues when they happen? I could go on, but I won't.

Awesome facility with an excellent caring staff. We loved this place. They took a personal interest in my 90 year old mother who was in the rehab unit recovering from a stroke and made her feel at home. The food is real good, the rooms are clean and well attended to, and the atmosphere is very pleasant and comfortable. The staff to patient ratio is low so mom was very closely watched. And in the 5 weeks that she's been there, we have developed some personal relationships with the staff that will last long after she leaves the unit. The cost is high but if you can afford it, its worth it.
