As someone who travels to different facilities daily I would highly recommend common wealth - Grand Rapids to not only families but employees as well. All the staff from Jennifer the executive director, Amy at the front desk to the care givers such as Lolita they are all welcoming, friendly and helpful. The facility is clean and always stocked with essentials! Definitely an ideal place to call home or work.

My grandpa lived the final weeks of his life at Commonwealth. I would often visit him and was impressed with the dedication and professionalism of the techs. I especially want to give a shout out to Shantee (forgive my spelling). My grandpa called her his fourth child and she was proud to earn that honor! Sometimes he would be in a sour mood and Shantee would immediately put a smile on his face simply by entering the room. She understood him because she cared enough to take the time to do so. She is an asset to the organization. Katina is another amazing staff. My grandpa, while his body was going through the shutdown process, refused to eat. Katina used her own money and drove to Popeyes to get him mashed potatoes and another time brought him Panera soup. While he was hospitalized for a few weeks, she texted every day asking for updates. She cared about him and wanted only the best for him. The other staff were exceptional, also. Thank you Commonwealth for helping make my grandpa's final days comfortable.

Last fall we moved my uncle to common wealth from another facility and what a change he has made. He was not eating or leaving his room and even hospitalized. Since his move he eats 3 meals a day, joins in activities and loves bingo. We now moved our mother, his sister into common wealth January 1 2026. With her dementia we knew from our uncles care that this was the best place for her. 5* care, staff that cares for their wellbeing and knows them on a personal level and as a patient. The directors and all the staff are true professionals who care. Highly recommended.

Our mother has been living at Commonwealth for a little over 6 months now and we really like it. She is comfortable, has an excellent room, and says that she really does like living there. The best part of it all is that I can feel the good vibes and love that are being bestowed onto the residents. The aides really care about what they are doing to help everyone that lives there....from small needs to larger needs. I truly appreciate that I can "really" not worry as much about my mom and her happiness. I know that Commonwealth has our best concerns in mind. Thank you ALL for that because this rodeo takes a village to wrangle and I need all the help that I can get.

We found that all the healthcare aids that worked with our mother were very sweet and gentle with her. They seem to truly care about her, which made us feel very comfortable. Deja’s quick thinking saved our mom’s life when she had her stroke. So many thanks to all the staff that cared for her!

There are many wonderful things I could say about Commonwealth but I'll keep try and keep my review short. My grandma, who had vascular dementia, lived there for over 3 years and to say they did a wonderful job taking care of her is an understatement. The place was always clean, the staff always friendly and my grandma's needs and wants always met with with far exceeded expectations. Everyone there is always cheery, both residents and staff and you're always greeted with a smile. I would recommend this facility over and over again for family, friends and the like. Thank you Commonwealth for making my grandma's last three years comfortable, fun and relaxing and for making it easy on my family and myself knowing that she was always taken care of and never went without.

My Mother- in- law lived in this Community for almost 3 years. The last 2 years she resided in the Sweet Memories wing. We recently lost her, I would like to take the time to thank you all so much for taking such wonderful care of her. My friends room is usually dirty when I go to visit and I find this so very discouraging. She always kept a clean and tidy house and now crumbs under the chair and Kleenex pieces all over the floor. The bed is often unmade and things are on the guest chair that I have to move. I often find her anxious for help with using restroom or most often waiting on medication. She has lost a lot of weight since moving here. She has friends who bring her food but some of the kitchen help won’t open a can with the can opener or add cottage cheese to her salmon. They say the outside food will contaminate with their food”like that food wasn’t brought in from somewhere! There are a couple people there that are helpful but many are just drawing a paycheck. Mobile phones shouldn’t be allowed while working! Maybe break time but not work time.

I have enjoyed playing the ukulele for the residents at Commonwealth Senior Living. It is a lively group and people love to sing along with every song that I strum. It is a lot of fun for everyone. Thanks to Erica and Irene for setting up this opportunity for me to share something I love! Ed.

My 97 year old father was admitted at level two and then bumped up shortly after to level 5 . Almost doubling the price Which was okay considering we were told he needed a much higher level of care. So we expected that care to be inline with the over $7000 a month we were paying. Yet he developed pretty serious bedsores that were not accurately addressed. A rash on his butt that he suffered with for months that could have cleared up within a week if properly attended to. He could not even get simple grooming done like fingernails and shaving, when he pressed his call button for help it took close to a half hour to get a response. A few times he fell and was left to wait for long periods for assistance and was scared. He spent most of his time in his chair although he begged for more socialization. The billing department was a nightmare to work with and when issues were addressed excuses were made . We have a lot of experience with senior living and in general the day to day staff is not paid enough to keep them so they are always shirt staff, which I have to wonder why at the price we paid, this community was a very big disappointment from the promises made at entry to the reality of the care. Thankfully we are moving him but if you are looking this would be a hard pass,

I'm so impressed by everyone at Commonwealth. I could tell immediately that they really care about all of their residents. It was clear this was a safe and well cared for place for my dad. Unfortunately, not all places have the standards of commonwealth but I'm grateful that they do and so glad we chose them. The kindness and understanding of the staff shows in the routine daily interactions with their residents and I am so grateful.

My loved one had a difficult situation in regards to finding the right fit as far as assisted living communities went. Commonwealth at Grand Rapids welcomed us with open arms and it couldn't have been a better fit for what my loved one needed. It was a huge relief for our family. From the initial tour where Tracy, Jen and Vicki welcomed us to the caring staff that my loved one formed friendships with and whom became like a second family, we couldn't have asked for a better assisted living community. The environment is clean and homey, the rooms are lovely, the grounds are cheerful and most importantly, the residents are cared for. There is a true sense of community at this community! My family is truly grateful for the experience that my loved one had!

When we first visited Commonweath Sweet Memories it was an "aha wow" moment where I felt angels were singing. I am not kidding. My mom needed more than I could provide and this was the place I could see her thriving. A community which offered activities and interactions and aides who greeted us with a smile. The transition was rocky but they were supportive of my mom and me and I finally stopped living in fight or flight mode and could breathe. Irene was a bright light during this tough time and I could not have gotten through it without her. Fast forward 12 months...something changed. Due to my mom's financial situation we needed to change facilities and ended up in a medicaid facility. First thought when you hear "Medicaid facility" is that they are dirty, over crowded and with aides who don't care, right? This new facility where we actually had our name on the waiting list for over 10 months, actually showed me what Commonwealth was lacking. I believe they are doing the best that they can but the residents in Sweet Memories deserve better. 1)The patient aide ratio is seriously lacking. At maximum Sweet Memories can support 40 residents. I know that there are more aides in the building but I only saw 3 aides at a time in SM. Sadly, I often saw them on their phone. One would be watching and the others on their phones. 2)The aide turn over is a lot. I was in once a week to visit and rarely did I recognize the same aides twice. There were a few whose smiling, consistent faces I loved, but not many. With the high turnovers I saw a few (unwitnessed by aides) patient falls and dangerous lifts and I wonder if they have time for appropriate training. 3)Without having a cooking kitchen in SM, food was often served cold. The warming kitchen is not enough for 40 residents. Commonwealth could be a premiere memory care facility with an increase in aides, training and lowering the number of residents. A cooking kitchen where they could focus on the needs of the residents by providing nutritious and warm food would be a blessing to those who are confused and afraid.

My grandpa was moved to Commonwealth's memory care floor after having a bad experience at another facility in GR. It is such a breath of fresh air to come in every time and see a clean space, with happy people, and kind staff who are actively engaged with the residents. I was having a very bad day on one of my visits and one of the staff members stopped and consoled me when I really needed it. I am so thankful that my grandpa is being well cared for by the incredible staff at Commonwealth.

I have called Vicky five times and left messages to have her call me back and she has not returned any of them. The last message was to have my mom ready for a morning appointment. Not only did I not get a return call she Wes still in bed. My mom has long term care insurance and commonwealth will draw my moms rent out on a timely basis but will turn in an invoice to Riversource until the third week. I have been told twice that Alisha was out of the office for the first week of the month. The person who cleans my mom’s room only cleans the open spaces. Does not clean under or around anything. My mom’s laundry is constantly getting mixed up with other residents. I am not feeling good about having my mom live at Commonwealth.

A relative just came home about 3 months in Commonwealth Senior Living, while they were recovering from some acute health problems. The facility is clean and bright. The staff were friendly and supportive, and we were always able to quickly contact management when we needed to do so. The food is bland for adventurous eaters, but otherwise it was a great place for my relative to convalesce.

The care they give my mom is great! I could go away and not see her for a month and I could see her and she would be fine. For overall care she is wonderful. She had an accident where she fell out of her wheelchair and they were quick to send her in. She is on hospice care now and it will all go through hospice. Overall I am very happy with her care.

We visited Commonwealth for potential Memory Care for my mom. Everywhere we visited seemed ver institutional and sterile. This place was comforting and felt more like home. Mom is going to love it here. The daily activities will keep her engaged and allow her to be social. She currently doesn't leave her house much because it's difficult for her to function in social settings with her onset of dementia. Commonwealth will make her feel comfortable. We are looking forward to having mom receive the care she needs.

Staff is okay but you can tell who is there because they care about the elderly and who is there just because it’sa job. My grandmother was there for respite and the staff is not trained to put on braces or anything else. At 1:30 in the afternoon my grandma was still not dressed because no one had been in to help her. After using the restroom some staff were out the door before she got situated in her chair even though she is braced and a fall risk. The food was mostly luke warm. Definitely room for improvement!

I visited this community today for a family gathering. It was clean, smelled good, and my grandma was so happy! Others were in the living room cheering on their favorite football team during the playoff game. I would highly recommend.

I would highly recommend Addington Place. My mom has Dementia and I was looking for a place where she is able to do most things on her own, but have help when it is needed. The transition of moving her in went very smoothly with Mindy’s help. She was there for me through the whole process and worked with me on making my mom comfortable and making it affordable for my mom to live independent! The staff are so sweet and genuinely caring. The best part for my mom is they allow pets! She has a cat and this really helps with her anxiety. Thank you Addington Place!

Assisted Living Center uses the term "assisted living" or "personal care" to inform users that a community provides assistance with some Activities of Daily Living (ADL's). It may not meet your state's legal definition required for that level of care.
