Feedback on care coordination and communications regarding my mother I am writing to provide feedback regarding the care and communication surrounding my mother, who recently received services from your team. Positive feedback: The care team, including nurses, janitorial staff, and therapists, delivered high-quality care and treated my mother with professionalism and compassion. Concerns and areas for improvement: Office support and communication: I did not receive the Zoom meeting links or advance notice that there would be weekly meetings to review my mother’s progress. I was invited to the most recent meeting, at which it was announced that my mother would be released. I would have appreciated timely notifications and reliable access to scheduled meetings in the future. Professionalism during meetings: In the most recent Zoom meeting I attended, I felt that the Director of Operational Support Services’s conduct was not aligned with professional standards. There were comments that touched on “family issues,” which I believe were outside the scope of the meeting and not appropriate for a clinical or administrative discussion. Communication between family members: My sibling and I currently have limited direct communication due to personal reasons. I am concerned that this has affected the flow of information and coordination. If possible, improve how to ensure all relevant parties are informed and that information is shared respectfully and effectively, while respecting privacy and any applicable training to ensure Director of Operational Support Services can maintain a neutral dialogue when speaking with family members.

Concordia is wonderful!! Our family agrees that it's the best place to help those you love! We are receiving a variety of care and assistance for our love one. They have all the care levels we will ever need. The staff is so amazing. They are kind, helpful, and very caring. If you need a place I would encourage you to call and set an appointment for a tour.

It appears that the nursing skills is the poor part. I have been in the independent living part and it is wonderful. Nice, clean apartments and the staff are very friendly there. They will do your linens for you if you are unable to put them in your very small washing machine which is what happens to my friend.

They take excellent care of my grandmother. She is able to have her own apartment with her own furniture and the safety of having nurses and nurse aids that she can call with the push of a button. She is always on the go playing games with her friends she has made. It got her out of a bad situation that wasn't safe for her anymore at home, to a place where she can socialize and have friends and be safe where we don't have to worry. The nurses are great about calling if anything happens at all. It makes me feel good to see her happy there.

Concordia is a wonderful retirement area but it is also a great rehabilitation center. The staff is attentive and friendly and knowledgeable. It is spacious and the semi private rooms are double the size of most nursing centers. My friend is in there with a broken hip and said the food could be better and the mattress is too soft but other than that she is pleased.

Concordia is simply one of the best communities for active older adults in our area! They are faith-based and many of the residents volunteer out in their communities and support local events. Several of our clients live at Concordia now and after moving often say they wished they had done it sooner.
