I have worked at the Heartwood Homes facility for a little over a year now. This is truly a place I would feel so comfortable putting my loved ones in for their care. Not only are the owners and the managers very hands on, exceeding expectations when providing information, care, empathy and showing compassion to the residents and their families, but they treat the staff with the utmost respect. This is a place I am so very proud to work at. I have been told from others, on more than a couple occasions, who have worked at other facilities that Heartwood is the cleanest and most involved facility out of others they have seen or worked at. To hear that made me appreciate this facility even more knowing how much everyone here WANTS to give their all for the residents. Working here, as well as getting to learn, grow, and care for all of the residents I've gotten to know, has been the biggest blessing. There really is no other place like Heartwood Homes.

Residents are all so content every time I visit. I used to work as a hospice nurse and have seen a lot of local memory care facilities and how they operate. Heartwood Homes is by far and away the best place for someone with any kind of dementia / neurological issues.

Where do I begin? Six years ago I became the activated power of attorney for my mother. I had looked at a few places one of them being Heartwood , and then I also went to American grand and Neenah. Unfortunately, I was swayed by American grand and all their newness. But it proved to be a very big mistake. it was the most difficult four years of my life. I struggled to make the move to another facility because I worried about upsetting my mom's routine. Finally I decided it seriously CANNOT be any worse. My mom has been at Heartwood Homes a little over a year and a half. And unfortunately she is now on hospice. I was always terrified at American grand of this happening because I knew they would not care for her properly even in a minimal capacity. Heartwood has been amazing! Lexi has been amazing! She has come with me to take my mom two different procedures and sat with me the whole time, even holding the trashcan while my mom threw up in the waiting room. Every single member of their staff has been so kind and respectful to me and my mother. Anytime I have ever had any concern or any request, they have always delivered and with a smile. At American grand I never saw the owner. I only heard of him building a house in Palm Springs. Here at Heartwood I have had conversations with the owner many times and his wife. If you are looking for a place that you can truly trust to care for your loved one. This is it. Do not look anywhere else. You will not find a better place for your loved one. It may not be brand new, but it is homey and kind. Thank you to absolutely every single employee at heartwood. I felt the need to share a conversation I had with a hospice nurse. He mentioned that there aren't many places he would recommend but this is one of them. He mentioned how the staff really care and know their residents. They can tell him how they are without even going straight to the residents book.

My brother Karl and I recently got together and were talking about mom and her gradual decline into severe dementia. It is a sad process to witness especially since less than two years ago she was mobile with her walker and could communicate with the help of her hearing aid. One thing we absolutely agreed on was that she is in the perfect residential care facility for someone with her needs. Our admiration and respect for the caregivers at Heartwood Homes Senior Living Center is a topic we repeatedly discussed. Every single caregiver in the facility is sweet, loving and caring, and provides gentle and nurturing assistance for all of the residents' needs. When mom has been particularly lucid, I have had caregivers thrilled to share her memories with me, and I could tell that they truly enjoyed being a part of her life. The caregivers function as a team especially during meal times. They are extremely flexible about making sure that everyone is well fed and get the foods they desire, and provide the necessary assistance if they are unable to feed themselves. Since the staff provides continuous care for her, they are the best source of information about her mental and physical state as it changes and they frequently update us on her condition. If there are any significant changes, they promptly address the immediate issue and recommend actions that will improve her happiness long-term. One example comes to mind. In an effort to make the transition to Heartwood Homes less jarring, we moved some of mom's furniture and memorabilia from the family home into her room. She was surrounded by family photos, and her favorite chair, loveseat and queen size bed met all of her needs. When she could no longer use the walker and was required to use a wheelchair, getting her to transition from her bed to the wheelchair became difficult, especially when she was determined to do it without the assistance of a caregiver. The assistant manager Brianna suggested replacing the queen size bed with a smaller twin and installing an alarm to notify the staff when she was getting out of bed. This worked well for a time, but as her condition deteriorated, she was put under hospice care and received an adjustable bed designed to ease her transitions to her wheelchair. I would also like to share my observations about the facilities. The first things I noticed about Heartwood Homes is that it is extremely clean, pleasant smelling, and well maintained, the shared spaces are well decorated and inviting, and the grounds are well taken care of. I credit Jim the maintenance person and all around handy man for keeping the facilities functional and spotless, although I am positive all of the staff participates in the facilities upkeep. When mom first moved in, Jim was there to help setup mom's room, assemble her bed, and hang pictures on the walls. Well done Jim! Brianna has been my primary contact and she performs roles above and beyond being a manager. She is on top of all of the day-to-day activities in the facility, including all medical care provided by outside nursing staff and hospice providers. Perhaps the best reflection of the ability of Jason and Brianna to create an outstanding residential memory care facility is their ability to hire and retain caregivers that truly have the resident's best interests at heart. When my mom was more verbal than she is now, she volunteered repeatedly that she liked being there, she was comfortable, the food was good and everybody was so nice. Heartwood Homes is a wonderful place to live. The staff is well trained, friendly, caring, and efficient. The rooms are comfortable and inviting with private half baths which are always clean. The food is delicious and varied and alternates are always available if something does not appeal to a resident's palate. Care and attention is given to all medical and pharmaceutical needs with meds given correctly and on time according to doctor's instructions. Additional assistance such as toileting and dressing are handled with care and compassion. The staff seems to enjoy their work and they work hard to create as much of a home environment as possible given the varied circumstances and needs of the residents. Activities are plentiful and varied as well and it seems that the residents are having fun. While it's not "home" per se, Heartwood Homes does an excellent job of making residents feel welcome and "homelike" as much as humanly possible. I highly recommend them.

I would like to share our incredible experience with Heartwood Senior living. Our family has been to quite a few Senior living homes in the area. With my father, in 2 different senior facilities, our mother was in one and our Aunt was blessed to have her last months of life in the best facility in our area. Their reputation was the best in the area and this was an understatement. From our first day to our last day, Heartwood over exceeded all our expectations. When we arrived Jim was there to help us make our aunts room perfect. He was there ready to help move anything we needed and to hang artwork that was done by our aunts late husband. After our aunt began living at Heartwood SL she started to thrive. We appreciated the homes cleanliness, soft music playing throughout the halls, beautiful decorations and the staff friendliness, and caring attitudes. Their attention to detail in patient care and taking time to educate us and provide reassurance when our aunt was going through difficult times. As she progressed in her disease of Alzheimer's, the management team, resident Doctor, Nurses, CNA's etc., were exceptionally trained and helped our family understand the progression of this disease. They gave us peace knowing she was under the supervision of an experienced staff. We were able to visit Her anytime we needed too. Heartwood Senior Living's overall skills, abilities, and level of professionalism is second to none . The best in the Valley and further. Thank you Heartwood and your amazing staff.

Heartwood Homes took care of my brother for 6 years. Prior to that, he was in a facility in Illinois that although caring, had so many residents that there was little personal caring. Jason and staff always had my brother's best interests in mind. They made him feel comfortable and actually happy while he was with them. Jason was a great source for information and guidance in managing my brother's care for 7 years, through hospice, and ultimately his passing. In his final days before passing, his cognitive abilities were greatly diminished but he actually said that he knew he was well cared for. That was such a great thing to hear from him.

A little over two years ago at the end of December, my parents moved into Heartwood Senior Living. I had spent two weeks looking at many facilities in the Appleton area. Both of my parents were at different stages of dementia, my dad at the beginning stage and mom more advanced. Mom was 92 and Dad was 93. They were inseparable after well over 60 years of marriage. Jason, the owner, went out of his way to make sure they were able to stay together. They were given two rooms next to each other so a door could be put in allowing one room to be a bedroom. My father has since passed. He was quite a character at the beginning because he did not want to be there. The staff were amazingly kind, patient, and understanding. My mom is still there and her Alzheimer's has advanced quite a bit. I feel a tremendous relief knowing that my mom is safe and cared for with love, kindness, and patience. I was so very thankful that Jason returned my call on that evening just before Christmas during a very hectic time of year!

An amazing memory care community for seniors! My mother in law was in Heartwood Homes until she passed away in November 2019 and we are so thankful for the caring staff and wonderful support that she (and our whole family) received from Heartwood Homes. In addition to the staff, the home is just beautifully decorated and so comfortable; we would visit for hours at a time and the staff was just wonderful to us all and made us feel so welcome. We cannot say enough about the excellent care that my mother in law received and the most wonderful staff. It was a difficult decision to find a new home for my dad when I realized we could no longer take care of him in our home. When I toured places that provided care for those suffering with dementia, they all seemed too restrictive and institutional. A friend recommended Heartwood Senior Living. Jason came to my home to meet my dad and from that first visit I knew this was going to be a better place, and it proved to be stellar. The facility was always very clean and the spectacular garden outside my dad's room was a source of great joy for him. They worked hard to keep him socializing and went out their way to honor him as a veteran. He was a proud man who struggled with losing his independence and the staff of Heartwood Homes did their best to show respect and help him deal with difficult issues such as being incontinent. Being in control was important to him and they were skilled at allowing him to continue feeling he could make decisions and at the same time provide the services he didn't think he needed. My dad lived at Heartwood Senior Living for three years and the care he received was respectful, kind, and dedicated. It was important to me that we select a place where my dad could remain for the rest of his life. Too many other places required moving him if he sunk into deeper dementia or if he entered hospice. Heartwood Senior Living assured me they would make it his permanent home and they did. I was so glad he could stay in his same personal space during those final days as he felt he was dying at "home." I felt confident the entire three years he was at Heartwood Senior Living that he was receiving excellent care, but the real proof was in the last week of his life. He gained the blessed time of clarity from his dementia during the last week of his life and he was able to express to so many of his caregivers his appreciation. Many times during those three years he was quite a difficult resident, and he graciously apologized as he shared his gratitude! Their exchanges showed me the bond he had formed with so many of his caregivers had become familial. I stayed at his bedside 24/7 during most of that last week and the staff not only took excellent care of my dad, but also of me and the rest of our family as they visited. It became so apparent that the staff's care for my dad was genuine and several caregivers came in on their days off to say goodbye to my dad and be with him as he was dying. They cried as they shared stories of their experiences with my dad and spoke of how much they would miss him. It was so powerful to realize that they had built a relationship with him not based on it being their "job" to care for him, but because they had grown to love him. I can still see him smiling from his deathbed as a couple of the caregivers told me how most of the residents would sleep at night, but that was often the time my dad was most awake. They would come and "party" in his room and ask him for advice on how to live a good life. My dad was always a "preacher" and to see his pride in having these young people want his advice was a gift I will forever be grateful they had given him. As death came closer, the staff at Heartwood Homes showed him such dignity as they continued to tenderly care for him. They were also attentive to my needs and even brought me comfort foods. His room became a sacred sanctuary. As we performed our faith traditions for the dying, many of them prayed with us. At the time of his death two caregivers were in the room with my husband and I. They were crying with us and it felt like they actually were all family gathered around his bed. Their supportive hugs and attentiveness to us before, and after he passed away, were truly from the heart . . . and so their name is appropriate, Heartwood Homes.

You want loving, competent , trained caregivers, a residence that looks and feels warm and friendly and a stimulating environment that engages all residents. Heartwood Senior Living met all of my requirements. From the very first time I met Jason his experience, knowledge base , patience and understanding was obvious. During a tour of the facility you saw a beautiful home and a friendly and caring staff. My 96 year old father has lived at Heartwood Homes for over a year. All expectations have been met and I couldn't be happier. My father is recognized as an individual and is always treated with dignity and respect. Both he and I feel listened to and heard . The staff and other residents have become an extended family. Jason, staff, myself and my family have become a team to ensure my fathers happiness and health during his final years

I have taken several days to try to find the words to express how amazing this place is! I have found there are not enough words out there to describe this. My Mother has dementia and is deteriorating rapidly. Heartwood Homes has gone above and beyond in taking care of her. I am able to sleep well at night knowing she is CARED for, not just looked after. My dad has lived at Heartwood Senior Living for most of the year at this point in time. The decision to have him live away from home was one of the most difficult to face. I did my research and found Heartwood Senior Living to be a superior home. I had a limited time to find placement for my dad. Jason and his staff bent over backward to help us. The care is great. My father feels loved and needed. They have found ways to incorporate his interests and gifts into his daily life in this home. The facility and grounds are clean and beautiful. Rooms are very spacious and light. The caregiver to resident ratio is fantastic. Their flower gardens alone are a delight to my dad who loves to garden. It really is a home away from home.

My mother has lived within this facility for over 5 years. I have been delighted with the care. I am a retired RN. One of my friends who is a RN, MSN manager over several senior homes (out of state) visited my mother with me. Upon our leaving the facility she commented to me "you can sleep easy at night". Nothing could be more true. I cared for my mother for some time in my home. However, due to personal health issues I am unable to do so. It is good to know she is somewhere I trust. I have had nothing but good experiences from staff and administration. I would highly recommend this facility for your loved one.

I can not say enough about how impressed I was when my Grandfather stayed at Heartwood. The facility was amazing- the rooms were spacious, there was plenty of areas for the residents to roam within, an outside garden space (my Grandpa LOVED) and overall was warm and welcoming. The staff was really nice- when family would come to take Grandpa places the staff was easy to be found and ready to help assist getting him into our vehicles. I was truly touched the day my Grandfather passed I noticed some of the staff outside the door in tears because my Grandfather wasn't just a face- he was person...he touched them like he did with anyone he met. I remember his funeral there was a group of Heartwood staff who attended- how many people can say that? Heartwood takes pride in not only their grounds, but their relationships. I've seen Jason, the owner, in passing several times since Grandpa passed and I asked what could make someone want to open a nursing home of their own and I was astonished when his face lit up and responded that its a passion and knowing residents chose him to spend this part of their lives with him- he was honored by that. He said that when someone dies he doesn't see if for the life gone, but the life lived.
