This is by far one of the best places I have had the opportunity to work. Our staff works well together in order to make sure our residents are comfortable and happy. Our residents are the best. Blacksmith Forks Assisted Living has the most amazing staff, when I visited my Grandfather they greeted me with a smile and welcomed me like I was family. Watching how each member of the staff cared for my Grandfather who has dementia melted my heart. I knew even if I was gone he was going to be in the best hands. Huge thank you to each of the staff!

Blacksmith fork is an absolutely amazing facility, it makes you feel so at home as soon as you step foot inside. Their front door leads to a little sitting area, with mints and enough room to sit and talk with your family, and farther inside is warm and so inviting, you can see all the residents smiling faces and you can see the quality of their care is exceptional. Even in the winter they have a bus that will be warm and comfortable no matter their needs. Amy is an amazing person who will make sure you get everything you need to know and will make you feel wanted and loved. I highly recommend blacksmith fork!

Jordan was so amazing to work with and very accommodating! The staff are friendly and so kind. The rooms are made better by the high ceilings and the big window in them that lets in lots of natural light. They made moving my mother in law so easy!!

I cannot say enough good about Blacksmith Assisted Living! We moved my mom there from a senior housing apartment early January 2022 and it has been such a blessing. The staff is wonderful and so caring of their residents. The building and its rooms are beautiful and the meals are great. It is the perfect place for my mom to be independent but have the assistance she needs when she needs it. Jordan and Misty have come up with a care plan that has allowed my mom to get back out and socialize again. Before going to Blacksmith Fork Assisted Living, she would not leave her apartment much or interact with people. It warms my heart to see her eating in the dining room with other residents and making friends. She especially loves getting her hair done in the salon on Thursdays! Her quality of life has improved in just one month! My family and I are forever grateful. Thank you so much, Jordan and staff!

I currently work here at blacksmith fork assisted living! I love my job so much! I love the small community, it makes it much easier to get a close relationship with all the residents! Us CNAs not only get a close relationship to the residents but to their family's as well! All of the residents are so sweet and so loving! The food here is amazing!! The cooks do an awesome job!! This is definitely a place I would feel comfortable to put my own grandparents or parents in!!

We moved our mother into Blacksmith Fork from another care center five days ago. The other facility was recommending a feeding tube. My mother was despondent and I believed if she wasn't moved, she would not make the week. Blacksmith Fork went above and beyond in helping us facilitate this move. It has been touch and go, but they actually had her up and in the dining room yesterday. Today she was actually feeding herself which is nothing short of a miracle. Jordan and her team have been incredible.

I am a current employee at Blacksmith Fork and can honestly say it is my favorite job I've had. I love coming to work every morning and seeing all of the residents. They are all so kind and friendly and treat all the staff members like their own. I enjoy being able to have so many adopted grandparents, especially when I am not able to be with my own. All of my co-workers are also amazing. You can tell the staff genuinely cares for each individual resident. They work hard and strive to make Blacksmith Fork not just an assisted living but a home. I am lucky to be able to work at such an amazing facility and would defiantly recommend it!

The facility is clean, smells good, tidy and well taken care of. The food is so good! The activities are so much fun and I truly think the residents enjoy them! I have worked at Blacksmith for a little over a year between working as a Dietary Aide, CNA, and Med Tech. I love it here, at Blacksmith Fork Assisted Living. The main reason I love coming to work is the residents! Seeing them makes my day much better. There is no better job, because of the friendships you create and the love you feel for all the residents. And the food is quite amazing as well. The cooks do an awesome job!

My husband lives at Blacksmith Fork Assisted Living and loves it. The people that run the facility, the CNA's, the chefs, and the others that keep it running really do care for the residents. It is very evident after just spending a short time there. The reasons I like this particular facility are as follows: Blacksmith Fork is smaller with 36 roomy apartments so it is easier for the residents to get to know each other. Everyone loves the food. They have lots of activities, including outside activities and scenic drives. They take the residents to their doctor and dental appointments which is a great blessing for us, the caregivers. I feel so confident that my husband is getting the care and help that he needs.

Our parents live here and it's truly a blessing. We dont know what we would without these Angel's that care for those we love. It's a beautiful building, the room is comfortable and our parents are very happy there. The food is fabulous and in fact so good, our Dads wife complains that she has gained weight, lol. When your parents get older, have some dementia, dizziness, are a little more frail than they used to be and they need that extra help, good medical care, fun activities to help them stay busy and continue to be part of life, its God Send to know that the people at Blacksmith Fork have our backs and care about our parents and each and every resident there. We recently moved a family member to Black smith Fork . It is a beautiful facility, very open and light. The rooms are spacious with high ceiling and large windows. The staff has been so helpful and kind helping with the transition. They have a lot of activities available. Chris has been awesome with all of this process. Highly recommend it.

Tomorrow marks the one-year anniversary of my mother moving into Blacksmith Fork Assisted Living Center. It has been the best year in recent memory for Mom. Two of her children had been taking turns having her stay in our homes during the previous four years but that was becoming increasingly difficult as Mom's memory decreased and pain in her back increased. We worried Mom would demand to go home (as she always did at a certain point in her stay with us). But BSFAL has become Mom's home. She receives the kindest care from the nicest people. The nurses, CNAs, the chef, housekeeping, activities coordinator, and the administration all know her name and go out of their way to speak gentle, loving words and treat Mom with care and respect. It is such a comfort to know Mom is well cared for at Blacksmith Fork. The staff is top notch, her room is clean, she is served delicious meals, she enjoys fun trips and activities, the other residents are friendly, and the facility is beautiful. What more could I ask?

My Grandma has been living at the Blacksmith Fork Assisted Living for almost 18 months. I am so impressed with the entire staff . Chris, the director, is so kind, concerned and willing to drop everything to answer a question or offer help. That includes transportation. The aides and nurses who work there are all so wonderful! They show kindness and compassion to each and every one of the residents, which sometimes requires a lot of patience, The activity director, Dawna, is also so wonderful. She helps with transportation and plans a lot of really fun activities at the building and other places as well. She always tries to involve everyone. Sometimes there are family activities and they are always a lot of fun. Any questions or concerns we have as a family are quickly addressed. We are so happy and thankful that Grandma is able to reside at Blacksmith Fork Assisted Living! The care she gets there helps ease our worries about her safety and health.

When the time came to move our parents from a senior living center to an assisted living center, our greatest objective was to move them to a facility where both their physical and emotional needs would be met. We looked for an environment that was clean and beautiful, where our parents would feel safe and secure, where their dignity would be maintained, and where any concerns by either our parents or the family would be addressed immediately and professionally. We feel management at Blacksmith Fork Assisted Living has been very accommodating in addressing any of our concerns. They have worked with the family as a team in ascertaining and providing the best care. Blacksmith Fork Assisted Living offers activities both at the facility as well as on field trips to help each resident stay as active as possible with the security of assistance whenever needed. Knowing help is available 24/7 has brought us great peace of mind. With deep gratitude, as a family we commend and deeply thank both management and staff at Blacksmith Fork Assisted Living for their loving, devoted, and professional care of our parents.

Blacksmith Fork Assisted Living is a fantastic facility conveniently located in Hyrum. The building is located close to the grocery store, park, food options, and the gorgeous mountains. My father-in-law passed away and our concern for my mother-in-law and her falling episodes prompted us to move her to where she would be in an around the clock safe place. We wanted her close and searched out the Blacksmith Fork Assisted Living Center in Hyrum. We have been so pleased with the beautiful apartments and the sincerely kind and loving staff. She did not want to leave her home but now seems to be thriving. They are more than attentive and responsive to her needs and to our family. We couldn't be happier. I like to think of it now as the Blacksmith Fork Loving Center. Thanks to Chris and his wonderful staff for every kindness.
