Dimensions Living Sheboygan is an excellent Assisted Living Facility that creates a home-like experience. My Dad has been living there for over 3 years and we know he is being provided competent, loving care because we see him and observe the interactions with our Nurse, Caregivers and QOL personnel on a daily basis. In addition, the Executive Director is doing a great job by supporting the residents and staff to ensure the best quality of life possible. We recommend highly!

How I wish I could give this place 5 stars. Reading the reviews makes me ask if they’re under new management??,..hired new caregivers? What happened?!!! My 81 yo sister spent the last 6 months of her life there, now called Dimensions Living. January 14 family received a call stating my sister was sent to the hospital with 6 broken ribs. She passed away 1 week later. Her daughter shared images showing extensive bruising across several areas of her body. A terrible heartbreaking testament to suffering no one should endure. The circumstances surrounding this remain deeply troubling to those of us who loved her.

To whom it may concern. My father is Gus Tarnowski, a current resident of Azura Assisted Living in Sheboygan WI. He has been there for 1 year, 5 months. I am VERY pleased with the level of care he receives! The incredible staff and caregivers that take care of Dad, day in and day out, are the best in the business, hands down! I live in Great Falls, Mt, and I come home to visit all different times. Every time I visit my Dad, I am always treated with respect and courtesy. The staff is always professional and willing to address any of my concerns about my Dad. They are always eager to help. My brothers and sisters who live in Sheboygan also interact with them, more than I do, with the same results! This is no coincidence! They genuinely care about my Dad's well-being and quality of life. The caregivers even went the extra mile and visited my Dad in the hospital when he was recovering from several surgeries. Who does that anymore? Azura caregivers do! Thank you wholeheartedly for the great care you give my Dad, and your collaboration with input from our family! Very Respectfully John Tarnowski

Great Experience!!! At Azura Memory Care of Sheboygan, the staff was incredibly caring and attentive, going above and beyond to meet the residents' needs. The facilities were clean and comfortable, creating a warm and homely environment. The healthcare and quality of life services provided were top-notch. I highly recommend Azura for its exceptional care and compassion.

My uncle resides at Azura Sheboygan. The people are the heart of this facility. From the moment I spoke with management, I could tell they were going to really care about my uncle and go above and beyond to be sure he received great care. These workers support dementia patients day in and day out and they are patient and know just how to work with these very precious (but sometimes challenging) residents. It is amazing! The facility itself is excellent - small number of rooms (some with private bathrooms) and excellent spaces for meals and activities. The staff works hard to keep all areas clean and maintained and even decorates for all holidays. I highly recommend this facility to care for your loved one with dementia.

My Mom recently transferred to the Azura facility. She has adjusted well and seems happy. The staff has been great - warm, caring and respectful. Mom is active and engaged. It gives me peace of mind!

My mother in law was recently admitted to Azura. They welcomed her with open arms and treated her like family. The facility is beautiful, warm and like a home versus a nursing home. The Director is really dedicated to the care and happiness of each person in Azura. The staff to client ratio is much higher at Azura. They also have a fantastic chef who makes wonderful healthy food. The first day on arrival he made my mother in law pancakes which is one of her favorites. We believe she is much happier and best of all has excellent care. Thank you to the great staff!

Both facilities are so beautiful now that construction is complete. It has been nice watching the director Anne put together a great team of caregivers. There are so kind every time I visit.

The journey of caregiving for my Aunt started over 4 years ago. It has been a very long difficult journey. It is hard to watch and frustrating to watch a loved one decline, as well as for them to live the decline. My Aunt found her final peace at Azura Memory Care in Sheboygan. It was not only her peace but mine as well. I no longer had to worry about her care as I could see that she was loved by her Azura family. She passed away on June 21 and they knew how important that it was for me to be there, but I also found peace that if I was not there, she was with family that loved her. No words can express the appreciation for the care, compassion and love that they have for all their residents. God has opened many doors through my journey and the doors at Azura were truly a gift from God. This family made a difference in a long journey. I will always have them in my thoughts and prayers. They are where and doing what they are meant to do!
