Wellspring Memory Care facility is absolutely WONDERFUL! My step mom went into memory care at Wellspring 18 months ago. My siblings and I are out of state and it was so hard to leave her with strangers and unable to see or care for her ourselves. But she had a couple dear friends who visited regularly and would report to us. When she was still lucid enough to know, she liked the food just fine and liked the staff. There were a lot of activities with engagement and interaction. Whenever we visited, the residents were always clean and well cared for. Most facilities reek of piss when you walk in and only once in many many visits did I smell anything. You can see they care well for their residents. Not only do they take care of the residents, they truly seem to love and care for them. When my step mother passed more than one nurse/aide was quite sad. In fact as we stood in her room the nurse said oh there is ***'s red jacket! And got emotional and I found myself comforting her . I cannot say that every thing is 5 stars (e.g., communications and housekeeping) but the daily care of the residents is ten out of five stars and that was most important to me.

Wellspring has been a wonderful facility for my wife. I could not do what they are doing on a 24/7 basis. In particular, they have a nurse, Dana, that is constantly going above and beyond to take care of "everyone". She does it all. I complimented her after two weeks because she was so impressive. I am complimenting her now after four months. She is the pinnacle of what a nurse is AND the rest of the staff in the Memory Care unit is also very good. I looked at 12 facilities before choosing Wellspring in Cincinnati. Aside from great staff, the facility is clean, the food is restaurant grade, and the aide staff is very compassionate.

I highly recommend Wellspring Memory Care, where my father is currently a resident. Kellee Gerding, the Memory Care Director, took the initiative to transfer my father from a semi-private room to a private room as per our request. She ensured that the room was prepared for his arrival and made the necessary arrangements to transfer him from skilled nursing to the private room at Wellspring. Kellee played a pivotal role in assisting my father in adjusting to his new room and kept me informed of all developments. Her exceptional organizational skills were instrumental in facilitating the smooth transition.

Just barely adequate. The staff are very gruff with the elderly, and seem to resent having to do physical care like cleaning them up or getting them in and out of bed. You have to bring up the same thing multiple times to get any consistency --- it seems the people on one shift never communicate a thing to the other shift, such as that someone needs help cutting up meat or other foods. We felt like a family member had to go there daily and then the care improved a little but --- we can't always do that.

All, I wanted to take a few minutes to let everyone know how much my family and I appreciate the folks at Evergreen Wellspring. Since being diagnosed with Alzheimer’s in 2023, my dad has had to deal with likely the most challenging episode of his life. Moving a loved one into senior living for the first time was also a huge undertaking for our family and one we did not take lightly. We are extremely happy with the decision we made to move our dad to Evergreen and now into the Memory Care unit at Wellspring. In the six weeks he has been at Memory Care, his demeanor has changed significantly and is now in a much better place than he was those last few weeks in Independent. I cannot say enough about the staff at Evergreen Wellspring. Every interaction I have had with staff there has been positive and reassuring. I specifically want to recognize Courtney, all of the staff at the Memory Care unit and even Darrick up front who always greets us with a smile and asks about my dad whenever I see him. If anyone I know has a loved one looking for senior living, I will certainly recommend Evergreen Wellspring. Thank you, Marshall (son of Jack)

This is a review about the Wellspring Memory Care facility and team. They have been doing a fantastic job taking care of my Mom - including working through some serious medical situations that she had. During her time in fighting Alzheimer's, I have never seen her more engaged, relaxed, and happy. Lorine and her team make sure residents have plenty of social time, activities - even outings! My Mom's personal needs are well taken care of. The food is very good. Lorine and the caregivers are responsive to our questions and needs - very friendly to interact with. We are very glad my Mom is here.

My parent spent approximately a year in the facility. We paid of our pockets because we hoped she would not just receive physical care but actual memory care as they professed to offer. ( activities to help inhibit loneliness, provide comfort etc). I visited twice a week at random times of the day and I saw them provide an activity only once. She languished in her wheelchair alone in her room ( a prohibited, unsafe practice which a nurse said she would address) or in front of the tv. She lost ninety percent of her personal ( very nice) clothing and wore rather sloppy outfits. Same socks more than one day in a row. There was supposedly an activity director but I don’t know why there were no activities (that I saw anyway). I didn’t call often but if I called the nurses I either could not get through get through at all or had to leave a message. I NEVER received a return call, even once. We once could not find her at the hospital they said they sent her to late at night after she had a fall. We called a number of hospitals to no avail. We called the nurse urgently hoping to find her and they never called us back. Through our own detective work we finally found her. On the plus side she was clean though her hair was not always very fresh and she had oral bad breath a lot. She never had any ulcers and they did treat the ones she came in with (resulting from a stay in rehab after a broken hip). They also called promptly if she had a fall and. Injury. Food was good. The facility was clean. However, Staff were often seen lounging with their cell phones out when things were slow and once a young man was semi dozing, head resting on the counter in the dining room for quite a while. . I never saw any staff make eye contact with her when serving her a meal. They’d slap a plate down and keep moving. No @hello, here you go, enjoy your meal-. No interaction. It was overall rather sad. We removed her and she is now in a small group home where they care for her lovingly and constantly update us. They know her name, her interests, they even say a short prayer with her at bedtime. Mom is very religious, it’s a comfort to her.

I came here for rehab after I broke my hip. I loved my stay here. The therapy department is absolutely fabulous! They got me up and walking within the day. Then there was Michelle in Social Services, she is just as Amazing! Nursing staff was attentive to my needs. There was so many wonderful people in this facility, Christa in Admissions and Zack the Administrator and Stephanie my nurse. Thank you guys so much for everything!

Cincinnati Veterans Hospital sent me there for a month. The staff is very polite and very friendly. If I ever need rehab help again I will not hesitate and will gladly go here. This place also has no smell like some have. Skilled care, Physical Therapy and Occupational Therapy was provided for me and they all did a great job.

My parents have lived in all levels of the Evergreen Wellspring community since 2012. From the 'get go' our family has felt supported by the staff at Evergreen and Wellspring. Mom is now currently in Long Term Care at Wellspring. The nursing staff is wonderful to work with. The nurses work well with the Hospice Team to manage Mom's meds and care. They can schedule transportation should she need to be seen outside the facility, schedule her for the 'in house' dental visits, podiatrist, audiologist etc. They have helped us navigate the morass of Medicare, LTC insurance etc. The aides are caring and intuitive in their daily interactions with Mom who needs considerable care. They are respectful and kind to the family and know us by name. There are several who go above and beyond in their kindness to Mom and other residents on a daily basis. They are so creative in how to interact/calm/understand their residents, many who have some level of dementia. The daily Activities program is fun and creative with the team entertaining and engaging each resident according to the individual's abilities. The grounds are beautiful for a wheelchair stroll to get Mom outside a bit. Wellspring is the perfect place for our mother at this point in her life and we truly appreciate all they do for her, our family and the rest of their residents.

My mom lives at Wellspring on the long term care unit. The nurse manager on her floor, Chrissy, is always polite and welcoming. She calls with updates frequently which makes me feel at ease knowing someone is looking after her. I also wanted to mention Shannon, the dining room manager, who works hard to make sure we have a memorable experience at all of the holiday meals. I believe she’s new to the team but a great addition for sure.

What a disappointment. My legally-blind, 98 year old grandma landed here for rehab after falling and breaking her leg. She had a previous fall and subsequent stay that was just fine. But this time around, it seems Wellspring is under new management. On the surface everything seemed ok: she was taken to therapy, invited to various events, and got visits from the ice cream cart. But taking a closer look, the care leaves a lot to be desired. Her call button was always broken, so often times she had to shout for help if she needed to use the restroom or needed anything else. When I reported the call button was broken yet again during one of my visits, a staffer said "well we gave her a bell". They literally gave her a tiny cow bell. I can't imagine 1) that anyone could hear it ring all the way down at the nurses station and 2) that this is an acceptable long term solution for someone in her condition. I don't know if they are short staffed or just lazy. On another one of my visits, the nurse actually asked me to look for supplies to change my grandmother's wound bandage. Seriously! She asked me to check in her dresser. I asked if this was something the facility provided or if this was something we were expected to bring in for my grandmother. She said they provide it but refused to go to wherever it was kept and instead said she'd make do with what she had with her. On another occasion, while being bathed/showered my grandmother fell and scraped her shoulder. I wasnt there when this occurred. Apparently it was a bad cut; there was still blood on the bathroom grab bar later that week when I came to visit. I guess no one cared to clean it up. My grandmother complained to us that the people weren't always nice to her, that sometimes they would leave her in her wheel chair until late at night before helping her to bed, and that they would leave her on a bed pan so long that her bottom hurt. My grandmother can be tough to deal with sometimes and has a strong will, but I imagine there is some truth to her complaints. Eventually, we got tired of reporting her broken call button and requested she be moved to a new room. That Friday, before a holiday weekend, we received word that she was being kicked out of the facility and would need to be gone by the following Tuesday or be charged some crazy amount of money per day to stay. Your family deserves better than to be stuck here. Update: After a couple of conversations with the director of the facility (post notice my grandmother would need to leave), our family was able to reach a more acceptable transition plan for my grandmother. I definitely get the feeling leadership wants to hear when care falls short of expectations and they are working to raise their standards.

This was our first time having to go through having to put my dad in a place like this so you can imagine how nervous our family was. We could not have asked for a better place for him to go. The nursing staff was knowledgeable, patient and caring. Cathy the nurse was friendly and made us feel comfortable leaving him there after we arrived. Stephanie the manager made herself available to our family and answered all our questions with a smile. Upper management came around and introduced themselves personally. We would recommend this place time and time again. If we ever have family or friends that need a place we know where to send them.

As with all places, there are some better and some not as good people taking care of patients. Overall, they do a good job. You just need to be involved and let them know you are paying attention as well. Holidays could use better staffing and a watchful eye to be sure they are taking proper care of whoever you have there for care.

I spent 2 weeks at Wellspring following total knee replacement surgery. The room was very comfortable, the meals were good and the staff was great. The time passed by quickly as I worked daily with the therapist to regain strength and flexibility in my knee. It was a very good experience.
