Our experience with assisted living facilities has been horrendous. My father has been to 2 facilities prior to moving to Azura Memory Care of Stoughton. At Azura he has eyes on him at least 3 times during the day and every 2 hours at night. Staff are cheerful and attentive when talking and interacting with my Dad. There is palpable concern in compassion from staff. Dad's room and bathroom are always clean and well maintained. Meals are prepared by an in- house chef. The meals are hot well balanced, and delicious - thank you Ian - fabulous! There are always options for enrichment activities. It's very hard to finally come to conclusion that your loved one needs some additional care. After many visit and reviewing places we decided on Azura in Stoughton which I'm so glad we did. My father received great care and everyone working there were so friendly and kind. I looked forward to seeing them each time. I worked a lot with Tracy and she was exceptional, she always was updating me on how he was doing. They were always doing fun activities throughout the day which my dad seemed to like.

Azura Memory Care of Stoughton, Wisconsin exceeded our expectations as a physical facility on an initial visit to tour. My wife, who has a clinical background in this area of caregiving had joined me for the visit. We were received at the front door by the Executive Sr. Director, Tracy Stanelle who's greeting was "welcome to our home!" Her huge warm smile said to us "I am glad you are here." Our impression touring was continually elevated with each caregiver we were introduced to. Heather, Cindy, and Charity shared their smiles and interest in the prospect of my 93 year old mother with Alzheimer's may be joining their Azura dementia family. As we moved through the facility with Tracy we observed Baily, a high energy full of life activities coordinator engaging a room of residents in the common area with an interactive activity. We learned these and other caregivers here receive 100 hours of memory care training initially and get an additional 30 hours annually. This is unique! We were happily greeted in the halls by employees cleaning floors, delivering bedding, and by kitchen staff inviting us to stay for a home cooked lunch. Wow! We later learned that Azura Memory Care of Stoughton (under the umbrella of assisted living) is a well-kept secret. Azura is in the top 3% of Memory Care facilities in the U.S. and Canada rated by an industry independent agency. Azura was recently awarded for the 5th year in a row the "best place to work" award by an industry independent agency. This really got our attention. The fact that the Azura model has a small population of residents limited to around 25 allows for attention to detail. But, not without a proper focus on priorities and a mission in sight always. The MOSAIC model rooted in the Azura system is a holistic care philosophy well understood by the employees in my opinion. If you observe closely one realizes caregivers, employees and volunteers are fun loving but quite serious and professional about their work. Every employee knows my mother's name and my name, and has made an effort to develop a relationship with us. That is what makes Azura Memory Care of Stoughton, "a family" and Azura "a home." Tracy Stanelle is a model leader to her team and always has an open door policy. Although I am retired, my extensive experience in business building allows me to quickly identify the seldom recognized little things that truly make a business successful. No matter the business, the bottom line is always the people and the product. In this case the people are, the exceptional lead by example Director, Tracy Stanelle, & Team at Azura and the product is Azura Memory Care of Stoughton as professional, trained, positive, loving, compassionate caregivers, employees, and volunteers delivering care at the top of the memory care industry. My mother has been at Azura Memory Care of Stoughton for five months now. These were our impressions on our initial tour, and each visit day over the past five months. I could mention more positive things about what I have observed regarding the people at Azura Memory Care, but I want to spotlight something important about the facility maintenance and resident care. When we walked into Azura the first time it was what I did not smell! It was the absence of the smell of urine that spoke volumes to me about the management, facility maintenance, and resident care at Azura Memory Care of Stoughton! If you have been touring other facilities in search of a place for your loved one you know what I am talking about. Save yourself time, stop searching, schedule a tour at Azura Memory Care of Stoughton, Wisconsin and get on their wait list. Your search is over!

Azura is a wonderful place for your loved one! We moved our mother to Azura Stoughton a few years ago after she spent some time in a different facility. At Azura our mother is happy, eating and well taken care of. Azura is clean and comfortable. The staff is wonderful in their care, communication and working with us to meet the needs of our mother. Good care facilities are extremely hard to find and I'm so happy we found Azura.

I call my mom regularly at Azura in Stoughton, the staff has always been friendly and attentive and have gotten to know me over the phone. My mom moved to Azura from another facility and the difference was very noticeable as she was so much happier. I live in North Carolina and have not been able to visit until a few weeks ago as my mom turned 100 years old at the beginning of November. My wife and I were greeted warmly and when I said we were there to see my mom Cindy asked if I was Dan, she expressed joy to finally meet me in person as was I to meet her. This experience was repeated over and over as we met more of the staff. Without asking this staff decorated and celebrated a fantastic birthday party for mom delicious cake included. What a wonderful experience. My wife worked in nursing homes for years and this facility is the best we've ever been to. Thanks to all the staff at Azura you're the best.

Azura Memory Care here in Stoughton, Wis is the most home-like place we could find. It not only feels warm & welcoming, it actually is. All the residents here have dementia, so everyone who works here interacts with dementia patients all day every day. Positive sweet care givers carry on with their bright attitudes all shift long. Every business is experiencing short staffing issues since Covid and this place is as well. This staff covers for each other: some put in extra hours to cover; all are willing to go above & beyond to assist each other with needed duties. Tracy, the CEO, has helped in the kitchen! I was so impressed by Tracy when I learned she had personally clipped my Mom's toenails shortly after we arrived because the podiatrist was not due for another week! Yes Tracy picks up hours to cover short staff issues too. Does not matter when you call or knock on her office door. Tracy always has time to listen, she often answers the facility's phone. Tracy looks young & energetic, bouncing around here with her gorgeous smile & ponytail, but she has been here for years & consequently handles the ups & downs of this population with grace and confidence - just amazing how reassuring it is to walk in here unexpectedly & find everyone living here getting along & accepting each other's challenges with respect. Everyone fits in. Everyone loves the home cooked meals too! And the caregivers, activity directors, med passers, nurse, hospice staff all seem to not only appreciate each other but genuinely love the residents. I have not witnessed the cleaning staff, but would be willing to bet they love it here too!

As a Registered Nurse for over 35 years, I truly know what it takes to advocate for individuals & their families. Azura did just that with their compassionate care for our mom. I was guardian for my sister-in-law for 9 years. She was in memory care units for 8 1/2 years with Alzheimer's. I was very unhappy with the first facility I picked. I could write pages about why I was unhappy. Luckily we found Azura for her last 30 months and she still had money so she could move. Jeannie was Korean so very tiny. Right away Tracy the director ordered a shorter toilet and lower bed, that would accommodate her better. All the staff including cooks and maintenance are trained in dementia care. It is so obvious that they understand the disease. The staff are treated well and like their jobs. The other place she lived in, it was a revolving door. They weren't trained and they didn't stay. People with dementia need consistency and daily routines. Azura is very homelike, warm and inviting. The meals feel home cooked I loved how they allowed Jeannie to have her stuffed dog, who she thought was real. Later she had a baby doll that she thought was real. The staff understood the importance of the dog and baby to her. I could go on and on. I saw terrible care, so I recognized the extra mile that Azura goes for their residents.

It is never easy to move your loved one out of their home and into a nursing facility. The folks at Azura understand the difficulty your family faces - both the patient and their caretakers. Mom has been at Azura for 11 months. When evaluating facilities, we like the smaller size of Azura as it felt more like home than a memory care facility. The staff is well trained and incredibly caring. I can't say how many times I've seen them hug a patient to bring a smile to their face. Keeping a positive approach to care is key to providing a safe and friendly environment. So thankful for the staff - can't say enough about them. The facility isn't fancy but it's clean and kept neat. Many of the staff members have been there since mom arrived, and they are so kind to mom. When we take mom out for dinner, upon our return the staff makes mom feel like a returning friend - asking how was dinner and letting her know they missed her while she was gone. They have cooks that work hard to make meals tasty and enjoyable. Always some kind of dessert - and everyone knows mom's partial to chocolate so they spoil her with that when they can. Tracy is the director and she's incredibly hands-on. I've seen her cleaning, cooking, grilling (yes - hot dog picnics), etc. Tracy has been a resource for our family giving us ideas on making mom more comfortable. And she calls us if there are any concerns with moms health. Highly recommend Azura care if you have a loved one on the dementia journey. You want a staff that has been trained and experienced to guide you.

It's hard to describe the level of care at.azura memory care Stoughton but let me put it this way. My Barbara had the most loving people there in her 6 years there and that level of care could not be found anywhere else. These people are wonderful, caring, loving people. I would recommend azura to anyone who would have their love one who Need special care, especially with Alzheimer's or any other special needs. A very wonderful place

I have been an in home caregiver prior to caring for my mother in home since January of 2020. When my mother fractured her back we found and placed her with Azura memory care Stoughton. The family was so impressed by the director Stacey as well as the family/community feel of the facility. I was impressed with Azura's mission that I became a team member giving help when and where needed. I am so fortunate to assist our awesome activity director, Bailey, with giving the residents opportunities to exercise body, brain and social opportunities. Azura gives your loved ones a purpose and a family to complete themselves. They deserve 5 stars and more. 

My dear wife who suffered from Alzheimer’s, was a resident of Stoughton Azura Memory Care until her passing in May of 2022. She received the greatest care possible while she was there. The staff was always courteous, professional, and compassionate. Her room and other areas in the facility were always kept clean, and updated. When the disease took its course and new health or emotional issues arose, both Tracy the GM, and Amiee the RN worked with me and the staff to address and resolve any issues. I would highly recommend this Memory Care facility to those who would need care of a loved one.

Amazing place, amazing people. As my mom's Alzheimer's has progressed we have had her in care at a few other places in the Madison area, and the care and respect she receives at Azura Stoughton is heads and shoulders above. The facility is light and bright and well maintained, and the meals always smell sooooo tasty (OK, I sampled, they are GOOD!). We never visit at the same time, and no matter when we drop in she is always clean, dressed (in HER OWN clothes! ..if you know, you know), and engaged, enjoying herself. Her room is neat and tidy and smelling fresh and her clothes are laundered. In our previous experiences, this wasn't always the case... The team is compassionate and treats all the residents with dignity and respect. They have a sense of fun and you can tell they love what they do and know it makes a difference. They plan fun activities on a regular basis and special events that are really a hoot (Elvis impersonator anyone? Cowboy sing-a-long? Just examples!). They keep us up to date on her changing needs and status. They are always available to answer a call and provide information just to ease my mind. But most importantly I think, as I grieve what my mother is losing and say goodbye to who she was, they take joy every day in who she is now, genuinely enjoy her spirit and sass, and help me to see the joy in that too. I sleep well at night knowing she is in the hands of folks that care for her, physically and emotionally. Tracey and her team are unmatched, and we couldn't be more pleased with their entire program.
