Recently we needed a safe location for my 88 year old Father who has been having too many falls ending up in the hospital. After spending many hours trying to find a location that allows him independence but care if needed by a professional caring staff I found out about Legacy House in Spanish Fork. The facility is well kept, clean and has the most absolutely amazing staff that enjoys their positions to care for my Dad. We have only been here for a short time but I'm very happy for my Dad that he gets to call Legacy House his home now. Thank you to all the staff for the great service you provide!

My mom lived in Legacy House of Spanish Fork for over 3 years until she recently passed. The staff was wonderful to work with and so kind to my mom. Our family really appreciated Jace, Shelby, Holly Walker NP, the activity leaders, all the Med Tech's, CNA's and food service helpers. The neighborhood volunteers that come and provide activities and support to the residents make Legacy House of Spanish Fork an even better place for your loved one. Thank you to all:)
November 2023
As a family, we love the tender care and love that everyone shows to our mother, her friends there, and to us as we visit. The activities that they provide for the residents are superb. I want to call out a few people - and I know I will miss many. As my father passed away and at other times with Mom, we have held family parties and dinners with food there and Mel has gone out of her way to make the event memorable for Mom. As a side note, Mel and her excellent staff provide great meals for the residents - Thank you. Tami and Shelby are always there to greet us as we enter the building, are available to answer questions, keep us informed, and ensure that the billing is managed perfectly. The management team of Jace and Tonya always make us feel at home as we visit. MaKell and all of the CNAs, etc. do an exceptional job of taking care of Mom with all of her needs. And a really big thanks to all of those aides and CNAs that personally take care of Mom. Over the last 18 months we have grown very close to those listed above as well as many others such as Nikki and so many others that take such tender care of Mom in the cottages. We have watched as all those listed and so many others may be employed at Legacy, yet we see them in their comforting, caring, loving, and true caregiving roles with not only Mom, but with all of the residents. They are so tender in everything they do - whether it is delivering medications or food, brushing out their hair, painting nails, helping them bathe and get dressed - the tenderness shines through above all else. Thank you Legacy House staff - you are great!!! At this time of year - I want to state that as a family we are so thankful for everyone of you.

Best care center around. People that work there really do care.
September 2023
The love of my life reached a point where I could no longer care for her at home. In November of 2021 we moved her to The Legacy House. She was able to stay until October 2022 when she required special geriatric psychiatric care and medication management. During the 11 1/2 months she was at the Legacy House the staff went above and beyond the capacity of the facility to meet her special needs. I am forever grateful for our time together with our Legacy House family. As I drive by to an from work I have fond memories of what I consider sacred ground.
August 2023
My 99 year old mother has been at Legacy House for about 2 months now. I felt extremely guilty about moving my mother from our house to a care facility. I was completely surprised at how well she has adjusted and how happy she seems to be there. The staff is just excellent. The CNAs in the Memory Care unit seem to have great relationships with the residents. I think that it takes a very special type of person to care for the elderly and disabled and I have nothing but compliments and appreciation for care they have provided for my mother.
April 2023
Our parents have been exceptionally pleased with the staff at Legacy House. They love feeling independent. We appreciate that they are in such a nice environment and safe.
April 2023
One of the best assisted living facilities i’ve ever worked for! All of the staff and management is amazing and so kind. The legacy house is like my second family. I would definitely move my loved ones in this amazing building❤️
March 2023
My mother and father in law moved in just over a week ago. This has been a hard adjustment leaving their home of over 50 years. The staff and friends at Legacy have taken such good care of them. Whenever we are there we are greeted with smiles from everyone. They have worked to customize meals where possible. We are so relieved that they are being so well cared for.

Legacy House is amazing!! It is the greatest place to be. The first time my grandchildren came they said "Grandma you live in a big hotel!". It is so nice. The staff are friendly and helpful, and the residents are awesome. We have such a sense of community. Everyone is so friendly and we have a good time together. The activities are fun and we have our own Branch of The Church of Jesus Christ of Latter Day Saints. It helps to bring us together. I would recommend this assisted living to everyone. I love living here and hope to be here for a long time!!

After nearly a decade of staying at the facility, my mother's retirement funds were exhausted and she was forced to go on Medicaid. After an "assisted fall", the facility did was is known in the industry as a hospital dump where they took her to the hospital and didn't let her back in. They did so rather then accept the reduced payment from Medicaid. She is much happier and doing great at different assisted living center, but know that once you are no longer a "self pay" resident, the facility will do what it can to kick you out. The Legacy House model is to take all of your retirement savings then toss you to the curb. You should know that the pharmacy they use is owned by the same parent company and they do what they can to get the residents to purchase as much medicine (including over the counter medication) as possible to further rack in the money. A review of the medication my mother was taking by her new facility resulted in a 75% reduction in the number prescriptions and a savings of hundreds of dollars each month.

09 Sept 2023 - 5 Stars For the past week, my family has been moving my father into this facility. In Washington State, I have worked with 2 different assisted living facilities and 1 nursing home as my parents aged. I found the move in process very smooth and staff very friendly and helpful. My father reports he has enjoyed all his meals so far and is much happier being with others his own age and shared life experiences.

Legacy House is amazing. My sister Theresa VanKampen is a resident here and from day one she has been treated with respect and great care. The place is beautiful, clean and very inviting. The staff is always very respectful, kind and sincere. When we met with Tonya for the tour, she was very knowledgeable and took the time to let us know what they could offer. Theresa needed transportation for dialysis 3 days a week and Tonya made all the calls, put everything into place and made that happen. This was the deciding factor in making the move and we have been completely satisfied with everything and everyone since then. We have had so many great interactions with the staff and other residents. Theresa has developed a close relationship with Sidney; she is so attentive and patient. The girls in the dining room know exactly what Theresa needs when she gets there (a cup of coffee with 4 creamers). I have had lunch with Theresa a couple of times, the food is delicious, and the people are very attentive. Legacy always has something going on for the residents; keeping them busy and involved, making sure everyone is given the opportunity to participate and have fun. The staff keeps us informed when Theresa is not feeling well. The CNA's and Nursing Staff are quick to identify a problem and if they think she needs anything they will call to let me know. We love this place and feel so good about leaving Theresa here. We know she is loved and cared for, and everyone goes above and beyond to make her feel like family. Thank you from the bottom of our hearts for all you do for Theresa. Tom & Toni Lopez

My in-laws spent the last two years of their lives at The Legacy House, Spanish Fork. The care they received while there, was wonderful! Everyone from the front desk staff, to the housekeeping staff, were so warm and caring. It's easy to be loving and caring to the sweet people. My MIL had struggles with dementia. She was often combative and verbally abusive to the staff. They simply loved her through it. The CNA's that worked with her, are truly angels! I visited my mother in law a few days a week. I will miss everyone at the Legacy House, so much! They are a family! Make them apart of your family! You will be grateful you did!
January 2022
I can't begin to thank Tonya and the entire staff at Legacy enough. They worked so hard to help us get a loved one admitted in an emergency. Her improvement over the past 2 weeks has been significant and feel so fortunate to have been guided here. Thank you so much to all the kind staff who are taking such great care of their patients here.
December 2021
My mother was being placed on hospice. However, because she was living independently prior to this, Medicare would pay for a facility OR pay for hospice. But she would only get to stay in a facility for 6 days because Medicare only pays for 20 days total and she had already stayed at a (terrible) rehab facility for 14 days prior to her second hospital stay. We would have to pay $200 a day after that 6 days and we had no idea how long she would be on hospice. Of course, we were hoping that she might still recover. Legacy House of SF let us move my mother there for half the cost of a rehab/nursing facility. They provided around the clock care, along with the hospice team. Their staff was AMAZING. I could not have made it through that time without them. Seriously. They took such great care of her and were always asking if they could do anything for us. I will forever sing their praises because they went above and beyond what I expected. Thank you, thank you, for making this difficult time a little easier to bear.
December 2021
Do not put your loved one in this memory care facility!!! We had so many issues with Legacy from day 1. I moved my mom out as fast as I could. Today I am dealing with a $5800 bill for my mom being there for 9 days. It’s absolutely disgusting that they have charged my family this amount when they did not provide the care promised. These poor residents are not getting the care and love they deserve at Legacy House. They run a terrible business with a lack of proper care for the poor residents. We had an awful experience with my mom being there and had to move her to a different facility because she was put in danger and not cared for. We were promised that there would be 2 to 4 CNA’s at all times. I was there all times of day and I never saw more than one CNA caring for approximately 15 dementia patients. These are people who cannot care for themselves. They have to be taken to the bathroom and have diapers changed multiple times throughout the day. They have to be fed, watched, washed and cared for. One person cannot possibly even begin to care for that many people. After we moved my mom out, the administration admitted to my dad they do not have enough staff… yet they had NO PROBLEM taking my mom in to make some money when they knew they could not take care of her! We had to put my mom in a memory care facility because four of us could not care for her at home. So we put her in a memory care facility for her to receive the care that she needed. Legacy cannot provide that care. They had one person trying to take care of 15 people. If Legacy cannot provide a safe fully staffed safe environment for these memory care patients they should not be open. After my mom being there for nine days she already started to get sores from not having her diapers changed. She is not even bedridden so I cannot even imagine what the poor people who are bedridden and cannot speak up for themselves are dealing with. It is heartbreaking to me. We were not told that they were having a massive Covid outbreak when we went for the tour and asked specifically about Covid. They dodged the question. I later found out that they had 30 current positive cases of Covid the week that my mom was there! We would have never put her in a place with that many positive cases. Her well-being as well as the rest of my family and anybody visiting was put at risk by them not telling us about the outbreak when we asked. We were promised activities for my mom so her brain and body could stay healthy. They did not have any activities or any one on one interaction with any staff. So we paid a ridiculous amount of money to this awful company so that we could go in and change her, bath her, provide activities, and change diapers ourselves. I don’t know how they can live with how they are running this business and not taking proper care of these people they are responsible for. They charge such a high price and their care is no where near the level of other facilities in that price range. Please do not send your loved one to Legacy.
February 2021
I was involved with the care of a family member that lived here. when they needed to transition from more independent living to the memory care part of the facility, the transition went smoothly. All of the aids and staff were kind and helpful.
August 2020
We recently had my Mother at the Legacy House. Every CNA that assisted her there, was more then kind they were wonderful. Staff always checking in on her and my family seeing if anybody needed anything. Every single one of us were impressed with this facility. This facility had a feeling of being in a grand hotel. We were all impressed. Ive never seen CNAs that showed so much Love, concern and compassion towards our Mother. I would Highly recommend this facility to others looking to put their loved ones
July 2020
Friendly, caring staff members, many who will go the extra mile to meet an individual residents needs. Property is well maintained and clean. As a smaller assisted living residence than many in the area, it has a much more homey feeling about it - - - which is why my mother chose to live there!
July 2020
Worked at the Legacy House of Spanish Fork as a CNA while going to school for three years. It was a wonderful work environment and have wonderful staff members who are truly passionate and caring toward the residents. Not to mention, the food their was delicious. My favorite was the Zesty Tomato Crouton Salad they would make.
May 2020
I worked here for a few years before I left on my mission and I loved it. I go to the facility a few times a week now for my Home Health work and love being there. The environment is upbeat and the staff is friendly.
May 2020
I worked at the Legacy House of Spanish Fork for a little under 1 and a half years as a CNA. Although there were enjoyable things about working here, I wouldnt recommend it to anyone looking for a job. The residents were amazing and I can easily say I miss them everyday and this job has taught me a lot in my career as a CNA, it was my first CNA job after all. However, management is extremely bad at handing situations, there are a lot of false promises being made, for example, they promised me Id get a certain pay but failed to actually give me the pay they promised. After multiple times asking I still havent gotten the pay they promised me. They promised a $600 hire on bonus as well which I should have got and no matter how many times Ive asked, I havent seen a dollar of that bonus. There are a few amazing CNAs who go out of their way to show love and compassion towards the sweet residents, however from the time I spent working there most the CNAs were just there for the check and showed no care at all towards the residents. They would be lazy and neglect call lights and leave the residents to care for themselves. Honestly, if there was a change in management then this facility would have a lot more potential. The CNAs are extremely overworked and under payed and management makes so many promises they arent able to follow through with. I can go on and on about the things this facility can improve on but Im just going to leave it here and strongly suggest that if youre looking for a place of work as a CNA that you avoid this facility, I didnt realize how bad it was until I got a job elsewhere and was actually treated properly and shown how residents should be cared for.
April 2020
What a wonderful place! Clean, Organized, Smells Good, Grounds well kept, and above all, Very caring staff! With the Resident in mind virtually every day, there are fun activities, pampering in the salon, Entertainment at night. And the food is Amazing! Residents will feel right at home.

I have worked at Legacy House in Spanish Fork for over a year now. We have a great team of CNAS. We all get along with each other, we communicate with one another, we help each other out with covering shifts. Our residents love being here. They feel at home here. Our administration here is the BEST that I have worked with. They take care of any problems that they hear or see, immediately! They are always there to help wherever the help is needed. It is a beautiful, wonderful, and respectful place to be.
July 2019
I recently visited my grandma here. I was very impressed with the location and the staff. My grandma is a very tiny stubborn old lady and they handle her so well. She has dementia they have many systems in place to take care of her and keep her safe. It is a very pretty place. They take her on bus rides to place all the time. There is also bingo she will play. I know they offer a lot more activities that she doesnt attend. She had a very hard time when moving here but has now made friends which has made it easier for her. I highly recommend this place if you are looking for somewhere for a loved one to go and live.
December 2018
I received a tour of the building recently. I must say its definitely one of the nicest assisted living facilities Ive been through, with almost a high-end hotel feel to it. The management and staff were very friendly and really care about those living there. It had a nice dining hall and outdoor space and there were lots of activities going on. Id hghly recommend checking it out!
