We have had my grandma in the memory care portion since 9/2023. When we first placed her here, she was on hospice with roughly 3-6 months left. Since being here, she has thrived and is off hospice. Everyone absolutely loves her. She has gained weight, eats, gets tons of snacks, loves bingo and all of the activities. One person she is obsessed with is Tamara. Every time we come to hang out or get her for a few hours, she wants Tamara to come. They have parties for the holidays and always do fun activities like decorating gingerbread, coloring, making ornaments, etc. We are so thankful we found this place for her.

My grandma spent the last 3+ years at this community, until she went to heaven. I'm so grateful for all the staff. From the moment she walked in the door, to her last moments, they all made sure her life was filled with love and joy. To have an assisted living not just provide a safe place to live, but a loving environment as well, is all a family could ask for. The wonderful people at Farmington Square took care of my mom during the last two years of her life. The made her feel welcome and loved, and I can't thank them enough for that. The facility was always clean and tidy, and none of the residents ever looked neglected. Food service and housekeeping were also top-notch, as was the medical care she received from all of the nursing and med tech staff. She had lived in three other facilities before I finally found Farmington Square, and it was such a relief to know that she was in good hands at last, with friendly, caring people all around. This really is a gem of a facility, one that kept my mom safe, engaged, and happy until the end.

My father was at this facility for about 3 years. He was in the transitional memory care unit before moving into the full memory care unit. My father arrived at Farmington Square with fairly advanced cognitive impairment, but was still able to do for himself with dressing, bathing, etc. Like so many people in his situation, he had no insight into how impaired he was. The staff at this facility are so caring, understanding and knowledgeable - I'm talking about you, Megan, Cory, MariCris and Allen, and so many others - and they are very skilled at gently redirecting folks away from challenging behaviors and toward positive interactions. My father passed away about 10 months after moving over to the full memory care unit and the staff in that unit were also very caring. This review is being updated after about year from my dad's passing. In hindsight, and now that the issue of a surprise bill is in the past, I can say that this was a very good facility for my dad and they have excellent staff. They give 100% every day, took very good care of dad, and they never betrayed the trust I and my family placed in them.

A Memory Care Community is near the bottom of the list of where you'd like your loved one to retire. But when reality slaps you in the face and says "Pick one!", most of us are in new territory. In my case, all prospective communities had good and bad aspects. And all were self-acclaimed as "The Best!". But Farmington Square just "felt right". My wife has been with them for over four years now, and has gradually slipped away from me due to her Alzheimer's progression. But as sad as that makes me, I am confident that she is comfortable and well taken care of. Staff and Management are quite competent and personable, and have been very good to us. The physical facilities are more than adequate to the task, and while it's not the same as having her at our home, they are much better suited to her care than I could provide here. I highly recommend Farmington Square Beaverton to any and all in need of the peace of mind so important in these trying times.

Farmington Square provides excellent care for my sister. She has been there now for 2 years. She loves the food, the friendly care by all the staff, the activities, especially bingo, chair exercises, singing, and the monthly concert from some great Portland area musicians. They take her to all of her appointments and Rob, the medtech/driver and activities coordinator, has given her exceptional care, he goes out of his way, like stopping to get her a treat on the way back from doctor appointments. The staff truly cares. My sister is the happiest she has been in years because of the extraordinary care she is receiving from all the staff.

I would recommend this care facility to anyone that has lost some of their independence due to life changes. My family and I had a mixed experience during my mother's 2.5-year stay at Farmington Square Beaverton Assisted Living. The first two years was in their transitional memory care unit, then .5 year in the full memory care unit. Here’s a breakdown of our experience. I always like to start with the good: Care Staff: The care staff at Farmington Square were generally good. They showed genuine care and concern for my mother's well-being. However, there were some employees who appeared to be primarily motivated by their paycheck, which was a bit concerning. I would like to point out some staff members who I believe went above and beyond: Cory, Megan, Isabel, Michelle, Brianna, and the van driver Rob. Through their actions, they all reinforced my feeling that my mom was being well taken care of. Food: One standout aspect of Farmington Square was the quality of the food. My mother consistently enjoyed the meals, and they were well-prepared with a variety of options and good portions. She was given snacks throughout the day, and someone was always ready to provide her with a cup of coffee. Billing and Communication: Unfortunately, our experience with billing and communication from day one left much to be desired. I stopped my direct withdrawal within a month of starting because they were withdrawing the wrong amount. Billing issues were particularly problematic. I find it interesting that anytime there was an "honest mistake," it always favored the facility and not the resident. We were charged room rent for 15 days after my mother's passing. While this may be legal, it is certainly unconscionable and gives you a sense of their moral compass. There were multiple instances of poor communication between the staff and our family, which led to frustration and confusion. This was both unexpected and disappointing. Management: Our interactions with the management team were less than satisfactory. They seemed disorganized and unresponsive when it came to addressing our concerns or resolving billing issues. This lack of effective management was a significant source of frustration during our time at Farmington Square. In summary, Farmington Square Assisted Living provided decent care and excellent food for my mother. However, the issues with billing, communication, and management were significant drawbacks to our overall experience. If these areas were improved, it could be a much better place for residents and their families. I'm not sure if I could recommend Farmington Square Beaverton to anyone who is private-pay; it seems to me that Farmington Square's number one goal is to separate you from as much of your money as possible. **** Response to the "Response from the owner" no surprise here a generic cut and paste response from the owner no wonder the management side of this business need so much improvement.

The hard work of a caregiver often goes unnoticed, but today I’d like to tell you about my fabulous experience during my visits at Farmington Square. When I come into visit I always come across Tawnya (Director) and though she’s always busy she always takes the time to greet me with a smile and respect. I always notice how Tawnya is always willing to put on a pair of scrubs to help out her care partners. It show’s that she is very loved not only by her residents but also her staff. Every time I walk in the cottages I notice that the residents are always entertained with Bingo and other activities throughout the day. I always stop by and check in with Melissa (Wellness Director) she is always patient and compassionate with her residents and care partners. And last but not least Carlos ( Lead Med tech) I admire how caring he is with his residents. How well he communicates and is always assuring his residents have the medication and care they need. And I give my love and respect to all the care partners for all the hard work they do to make sure all the residents are taken care of day after day. I definitely recommend Farmington Square Beaverton. I can assure your loved one will be in loving caring hands.

I highly recommend Farmington Square Beaverton to anyone who wants the best possible care for their loved one. They doted over my husband and cared for him like he was their own father or grandfather. I visited almost every day, so I know what I'm talking about. We had a tight timeline and called last minute for a tour. Even though the staff had other commitments, they found a way to accommodate us with grace! The place was awesome! Moving a loved one is not easy, and the opportunity to see a place that is filled with loving, caring staff who understand the challenges of aging, yet find a way to keep the atmosphere light and fun, was a big relief. I can't wait for my mom to move into Farmington Square!

My father has lived at FSB for the last 6 months and I am extremely happy about the care he is receiving. It was a very difficult & painful decision to move my father into memory care and the staff at FSB made the process as smooth as possible. I did a lot of research and toured 8 different facilities before choosing FSB. This facility is clean and does not smell like urine like so many places do. They have lots of activities, which really help my father stay physically & mentally active. The staff has been amazing and treat their residents with care. Big shout-out to Maria, the Memory Care Wellness Director who always goes above and beyond; she is so patient and kind “ a real pleasure to work with. And to Eric, the Executive Director, who is more involved with the residents than most directors; he truly cares and stays involved “ he even plays the guitar for them! All the med techs and caregivers are great, but I want to give special recognition to Grace & Brianna, who are rockstars. I can’t say enough about how happy I am with FSB!

My mom was a resident at Farmington Sq memory care for 4 years until she passed 2 weeks ago of Alzheimer’s. Everytime something would happen to my mom a fall etc... they would always call me to inform me of any other emergencies. I was impressed with the workers as the residents hair started growing because of Covid the aides took the time to braid the women’s hair and often gave my mom & others a manicure and also with fingernail polish. I also noticed how the aides where caring with love for the residents. The place doesn’t smell of urine and is clean. While my mom was on hospice I was able to go into the building to meet with hospice nurses. They commented on how the atmosphere was bright and not dark. All the residents were out in the living room listening to music or watching TV not stuck in their rooms. They also have a place set up for Skyping with your loved ones while Covid prevents us from visiting. Every time I called they were always pleasant and told me to call as much as I needed to. The last week I was there saying goodbye to my mom some of the aides came into her room to say goodbye to her. I was really touched by their love for her. I’m sure you can probably find something to complain about but I have nothing bad to say about the employees at Farmington Sq. they are doing their best even management was always great & the residents nurse was great and called anytime she was concerned about my mom. If you are looking for a safe & loving place this is it! THANK YOU FARMINGTON SQ EMPLOYEES FOR LOVING AND TAKING SUCH GOOD CARE OF MY MOM!!!

The folks at Farmington Square treat my father like family. They think about his needs before I do. They take care of so many little things that I could never do such as keeping him stocked with his favorite candy and always making certain he is entertained. He tells me frequently how much he loves where he lives.

My Brother and I moved our mom here. She started in B building where she could do most of her daily ADL's herself. I loved it. It was very nice. Everyone that worked in the building were very caring. As she started to decline get a bit combative and exit seeking they wanted to move her to the Memory Care Building. It was very upsetting for me but once she got settled it was so much better for her as they stay busy, have all kinds of activities. I thought she would just decline as she was still really social but she blossomed with all the constant activities that I thought she would hate! The staff here are awesome! Always kind even when my mom was very agitated and not so nice™ I will forever be thankful we picked Farmington Square! Thank you to all of the staff for making her feel loved.

Due to the Covid-19 pandemic I have had very little personal contact with the staff that takes care of my wife. But the interactions I have had over the phone and on Skype and Zoom have been great. Both the care givers and administrators have been both professional and compassionate both to my wife and myself. They have left me with no doubt that I made the right decision placing her with Farmington Square. I look forward to the day I can visit my wife in her room, hold her hand and also get to know those taking care of her. Until then I take comfort in knowing she is being kept safe and is well cared for.

My mother has been calling Farmington Square Beaverton "Home" for just about a month now. I knew when I first met the entire team, that mom would be in caring and capable hands. What I didn't "expect" (but dreamed of) was a residency that mom would be so graciously welcomed into. They understood the delicacy of transitioning to a new environment and met this with warmth, kindness, flexibility and patience. Mom receives the respect and individual support that I would hope for myself. Whether mom is tending to flowers in the courtyard, quietly working a puzzle in her room, or visiting with other resident friends, she has a sense of feeling at home, safe and loved. And for this I am deeply grateful. During this time of COVID, it is all the more assuring knowing that while I cannot "hug" my mom, I know she feels embraced. I want to add that the hands on attention and personal communication with the Executive team post signing moms Admission Contract didn't stop once we committed. Adrienne and Ashley have continued to be fully engaged in and with mom. Each and every one of their skilled, thoughtful, caring and friendly care team is extraordinary! If I try to name everyone I fear I will miss someone, so to each of you, THANK YOU! You are AMAZING!

My mom has been here since 2013. We love this place and especially the caregivers and all the the staff. They are like family to her. Everyone is kind and they do everything they can to make each day interesting. She is very disabled but they make sure that all her needs are met. We moved our mother to Farmington Square after a terrible experience at another memory care facility. The difference was like night and day. Mom was at Farmington Square for about a year before she passed on and we feel the care she was given was excellent from the day she arrived to the day she passed. My father was at the Memory Care unit at Farmington Square for the last 7 months of his life. The staff were all wonderful, caring people who took excellent care of Dad. They not only took great care of Dad, but they were also wonderfully caring and supportive of my mother. We'd had Dad in another facility that looked fancier, but the staffing ratio was terrible and we got him out. We were so happy to find the loving care at Farmington Square!

My 82 year father was at Farmington Square Beaverton for almost 3 years and we never had a problem with the staff there. They were nothing but awesome. After my father had a stroke, he was diagnosed with Lewy Body dementia. He was having very aggressive behavior and was denied by every memory care facility that we had tried to get him into. On the day before he was to be released from the rehab facility he was in, after his stroke, we contacted Farmington Square. They came out to evaluate him and said they would take him in. From that point on, my father continued to improve. They were instrumental in helping even out his meds and make sure he was as comfortable as possible. He eventually began to call Farmington Square home. We all were very happy with the care he received there. They were kind and caring to all of the residents, not just my father. When my father was in the last stages of his life, to no fault of Farmington Square, all of the staff, even the administration, were very kind and understanding. Making sure that my father, as well as the rest of my family, were comfortable. They even invited my family to join in their staff Thanksgiving dinner. I would recommend Farmington Square Beaverton to anyone. Thanks to Teddi, Rachael, Jen, Carlos, Nancy, Steven, Alex and to the rest of the staff at Farmington Square Beaverton. I wish I could give you guys more than five stars!

My mom has been at Farmington Square for 7 months now. The staff is always polite and courteous to her. They provide many activities for her and the other residents. I like how they communicate with me whenever there is a need or any change in my mom's condition. They also communicate with my mom's doctor and are always on top of her medical issues. It is a very clean facility. The only thing I would not rate as excellent is the food, and it's okay.
