The grounds are pretty and if you’re independent then it’s a nice place. We put my MIL in the memory care unit there and it’s clear the higher ups don’t give a damn about their residents. We experienced problem with an aide at night who would check on mom once, if that. She would be wandering her room for hours. The director of Ohio living [name removed] made the determination that what we had on video was enough to fire her from the memory care unit. Well the next month or two went a lot better. The girls they brought in were doing 2 hr checks and helping guide her back to bed. A few nights ago we notice the horrible aide is back and again we are right back where we started. One check per night and no help or compassion for our mom. I would definitely not recommend this facility. The day shift aide are very nice and good and the activity lady is very nice but at night it’s like watching a nightmare and your just waiting to see your love one get hurt because of negligence.

Very simply put, I don’t think that I can say enough positive things about the Swan Creek to really articulate how I feel. Our dad was a resident here for about the past year. He was first on the independent living side before moving over to assisted and then eventually skilled care in his last days. The facility is great inside and out. He/we loved the nature-centric setting and really liked walks along the Swan Creek on the new boardwalk. The Swan Creek staff, they are the real hero’s in my mind. From the aides, nurses, and Hospice folks he saw every day (or nearly) all the way to the kitchen staff, facility crew, and administrators that he interacted with less often, they are all truly amazing people - giving, understanding, warm, and kind. Living 2 hours away, even with my sister living close to the facility, this loving staff that I felt made our dad feel like he was part of their larger “family” right there really helped me cope with an already very difficult situation. If the folks at Sawn Creek are reading this, my sister and I would like to thank you for all you did to support us and our dad. We couldn’t have had him in our lives as long as we did without you help and for that we are forever grateful.

This is absolutely the best place! My mom has a wonderful apartment, and she loves living there. They have in-house rehabilitation, which she needed after hip replacement. I will recommend this facility to everyone!

Thank you so much for making a very stressful situation for rehab placement completely smooth and wonderful. My mom has had such a great outcome I can hardly believe its the same person! Shes doing fantastic and said she felt like she was leaving her family today when she left rehab to go home. Your team treated her like a loved one and I cant thank you enough.

Living in our villa is like living in our previous neighborhood home with an active club attached. Great staff that help us enhance our desires and wishes to the best of our abilities. We have found this atmosphere has attracted an outstanding number of interesting people to live here.

The love and compassion you showed her and our family has helped us through a difficult couple of weeks. As a nurse practitioner, I know health care isnt always easy, but each one of you showed us patience and concern and made sure her heavenly journey was comfortable. God Bless you all.

The grounds her are almost vacation like, the grounds and maintenance people really do a wonderful job of making the outside look great. The actual nursing home portion lacks a little a very much needed update, the staff is decent, nothing over the top but not a bad place to be.

I love the residents and know everyone by name. I even have some of the residents family members request me just to give me hugs and say how much they appreciate me and my co workers here at OLSC! I LOVE my job! Wonderful place to be!

Ohio Living Swan Creek has a positive atmosphere that is noted by most guests. Everyone is friendly and whenever residents or employees pass one another in the hall, there is always a friendly greeting. Employees create such wonderful bonds with the residents here which is so wonderful to see. Employees are very hardworking and really care about each one of their patients. As for the management team, they are always open to listen to their employees needs and do whatever they can to help. Overall, Ohio Living Swan Creek has a great environment and is a great place to work.
