Kim and her staff go over and beyond to treat everyone in this facility like family. You can rest assure, your loved ones will be loved here. They have tons of activities that keep the residents busy. We are fortunate to have a place like this in Jacksonville.

I cannot express how amazing Kim Nolley and her staff are at Country Place. My father-n-law has been there for the past 2 years and they have been nothing short of exceptional. They are compassionate, supportive, attentive and have continuous activities for everyone to engage in. It's small enough so the residents feel like family and are loved unconditionally. The last several months have been extra challenging for my father-n-law and they have went over and beyond to care for him while he waits for the memory care facility to open. I am forever grateful to Kim and the entire staff for all they do everyday.

My mother-in-law was in Country Place for eighteen months and she absolutely enjoyed her stay. Kim Nolly the administrator and the staff are wonderful with the residents. If you are looking for a place for your loved one please check out Country Place Assisted Living.

When the time came that my aunt could no longer remain in her home, we moved her to Country Place in Jacksonville. She lived there comfortably for over a year. I don't have the words to communicate how grateful I am to the staff for the care my aunt received. She felt safe, cared for, and content. Her family felt very at ease knowing that she was getting the attention she needed. Kim Nolley went above and beyond to be available for us when necessary and keep us informed between visits, as the family resided outside of Jacksonville. Kim and Country Place have truly been an answered prayer; not only for my family, but for the entire senior community of Jacksonville. If finding yourself in need of a place to relocate a loved one, I highly recommend contacting Kim and exploring the option of Country Place in Jacksonville.

My mom lived at Country Place for about a year and a half before I had to move her to Memory Care earlier this week. I can't say enough good about the care she received from them. I never once worried that she would be well-fed and her medications would be admnistered properly. Her room and her clothes were always clean, as was the facility in general. Communication was excellent and the staff was friendly, professional, and courteous at all times.

My Mom is a current resident of County Place. The first thing she said about the residence is "this feels like home". Her room is spacious and roomy to allow us to make a home within this home. That WAS our #1 goal for Mom. We wanted her to feel like she is still in control, managing her own home. The entire staff is caring and attentive. There is plenty to do and each resident is encouraged to engage in all activities. So many activities provided, along with volunteers that come in helping the residents continue to be a part of the community by bringing the community to them. My Mom is not a "mixer". She prefers her room, her TV, etc.... the good news is, she is still not alone. The friendly staff checks on her frequently, visits in her room and engages with her, tailoring the residence to what makes her the happiest. I recommend County Place to all my friends and family and recently was fortunate to recommend 2 friends to bring their Moms to County Place. Both of my friends mother's have since moved in and are living their best lives tailored to meet their needs as well. My family highly recommends County Place Jacksonville to your family. You will be glad and RELIEVED you chose County Place for your loved one.

A staff member neglected to help take care of someones bedroom. Granted i understand they are in a assisted living environment and can clean their room, but when you pile dirty rags in the corner from their accident they had and not take them out of the enviorment once you put them in the bed, thats neglect. There is actual s*** on those towels. You wouldnt like it if you walk in your grandparents room and look in the corner and see piled of rags with poop on it. You understand how disgusting and unprofessional that looks? Theres been other times at well we have wa;led in there and had to clean because yall cant seem to assist them. Trained staff... Learn to help them out with whatever they are needing help with. Dirty diapers and shit rags just left on the floor. Help them clean. Yall get paid to help assist. Thank you have a blessed day.
