When Rehab is necessary, Aspen Trace has been a blessing. I am impressed with the team especially Eric (RN) and Gina (Admissions). PT, OT, and Speech are fantastic too. Really like that each Aspen leader is assigned a patient and they give the resident and family their personal cell number along with checking in with the resident daily.

My mom became a resident at Aspen Trace in mid-December 2025 after my dad passed away on December 1, 2025. It was a very difficult time for all of us, especially for my mom, who had just lost her husband of 74 years and was also having to adjust to assisted living. From the beginning, the entire assisted living staff have provided outstanding care and support. I am grateful that we found an assisted living community for my mom that’s staff is as professional, courteous, and caring as the Aspen Trace assisted living staff.

I will a 3.5 maybe almost 4.. so let me start from my dad is an 80 Year old man that fall broke his back in 2 places.. of course insurance is going to dictate your stay and not a physician!! Nice place it is clean except if you in a room for more then a week.. I got on my hands and knees an would clean up the floor especially on the weekends it's the worst! No janitorial on the weekends is what I was told!! I will just say that if you don't have family members and you're in this place you're going to get the littlest care..even my dad having family and someone being there every day for him he still got the minimalist care bath once a week like how unsanitary is that!! Even on his first night he had to get himself up an go to the bathroom because the night crew did not care to come help and my dad be dammed to put on no depend!! Just like any facility that you put your loved one in just make sure to always watch them because them facilities will not they're going to do their job get their check! End of story.. edit::update! : after bringing him finding sores on his back an butt . And where he had fell he hit his elbow that is now infected it was not probably tended to. So take the 3.5 away to now definitely at 3.

Aspen Trace was where mom went post-op with a broken femur. The staff welcomed her and us like we were family. All the nurses and aids are very caring and attentive. Special kudos to Estell, Star and Joey for all the kind words and support. PT nurse Neal and OT nurse Kim were both very kind and encouraging. Gina and Kasey were extremely helpful with navigating all the moving parts we face in the 3+ weeks of being there. Housekeeping does a great job with keeping the rooms tidy and the food is above average for a care facility. I would highly recommend Apen Trace for elderly care of a love one.

Having a family member move into assisted living is always an adjustment for the entire family, filled with all kinds of angst. It's been complicated for sure. But Aspen Trace has the best staff with very little turnover. We especially appreciate Ashley Austin because she takes such a special interest in the residents and goes the extra mile.

My brother was admitted to the rehab unit. He has a wonderful, roomy room & bathroom. The food is terrific & the care is beyond terrific. Had a meeting today with Social Services, a Nurse Navigator, Occupational Therapist & Physical Therapy. They were all very professional & provided information on the care plan & goals. Everyone here is so caring & attentive unlike the facility he was in before that was so neglectful & almost cost him his life. Would recommend Aspen Trace to anyone in need of care.

My husband and I have been at Aspen Trace for over 7 years. I fell down the stairs in our house and it was decided that we needed to be in a safer place with no stairs. We have been happy with the staff help in assisted living. Ashley Handy is easy to work with. The servers in the dining room are helpful and friendly. Another Ashley(I don't know her last name) is super in helping anyone or any place that needs help. All the nurses and aides are willing to help or get help if needed. A problem there is quite often workers leave but that is happening everywhere. I am not happy about the lack of appliances - stove, disposer and such. This is all for safety and expense but it is a long way from home. The residents are friendly and seem satisfied with the situation.

My mother recently stayed at Aspentrace while recovering from a hip fracture. We had a wonderful experience. Her physical therapist Anil Kanakadani was exceptional” professional, patient-focused, detail-oriented, and empathetic. The entire therapy department was great! The nursing staff at the facility were professional, friendly, and courteous. The facility was always clean and welcoming. We are very grateful for the care my mother received, I highly recommend Aspentrace.

The decorations were lovely as was the holiday music, and you all worked so hard to make sure everyone was fed a good meal. Nice touch with the Grinch and Santa Claus appearances. My mother had been a resident at Aspen Trace, a CarDon & Associates Skilled Nursing Facility, for nearly five years, and I couldn’t have been more grateful for the care she received. Before choosing a facility, I had researched several options through CMS, and Aspen had stood out with excellent inspection results. It had been clear that the state had no concerns about the quality of care ” and now, years later, I completely understood why. The staff at Aspen had been compassionate, attentive, and had genuinely treated my mother like family. They had gone out of their way to make her feel comfortable, safe, and loved ” whether it was surprising her with her favorite snacks or helping her FaceTime me during the day. Their kindness and commitment had given me incredible peace of mind, and I truly believed she couldn’t have been in a better place during the last few years of her life. Thank you again, Aspen Trace, for all you did for my mother.

My mom began her journey in rehab at Aspen Trace in late 2023, and unfortunately was not able to regain her capacity to stand/walk. The PT dept. was terrific btw and I highly commend all they did with my mom. She also had a wonderful CNA, Steve, who still spends time/visits her today. Mom’s lack of progress in rehab, however, meant she was required to move to the Ambassador wing for skilled care. We were apprehensive but it was a smooth transition. I disagree with some reviews I’ve seen re: food as I think AT does a great job with variety of food (my mom rarely complains about it) and I think they take a lot of care to keep things clean. I work full time so mostly get to see my Mom at night/weekendso I’m not exposed to a lot of the day staff. Which is why my connection to Ambassador’s charge nurse, Lindsey H is so critical. She is always responsive, never dismisses a concern and investigates any situation, medical or otherwise, and gets back with me promptly. She is caring and really has taken time to get to know my mom, and I trust her completely where my mom’s care is concerned. She is the gold standard in my eyes. I also have to shout out Kadesha, one of mom’s CNAs, for her love and care of my Mom. She knows her, her needs, and goes above and beyond on a daily basis. I also love mom’s nurses! Have to shout out Megan as my mom often refers to her as my angel, high praise indeed from my mom as she doesn’t even call me that! Mom also loves activities and often talks about the days events. (Bingo is a fav.) Is it a perfect place? No but I will say anytime I raise a concern, it is seriously evaluated and I receive follow up. The Executive Director (Emily) has facilitated several conferences on my behalf and it’s a lot like family (open transparent discussions), but we work through the issue. Staffing levels have been a concern I’ve raised in the past but I feel like that has improved in recent months, and I greatly appreciate seeing some of the same staff day after day as feel it gives them the chance to get to know their patients and their needs. Lastly I have to shout out the facilities director, Brandon, and his team. Anytime there has been an issue with my moms room (bed, shelves, sink, etc), he/his team is quick to address it with a smile.behind the scenes work that is so appreciated. So if you ask me if I would make the same decision again in terms of selecting Aspen Trace, I would say yes (and I’m a hard grader! We all need to be when it involves our loved ones!). It might cost more than other facilities but I think the care is worth it, and I feel no matter what facility you choose the will be positives and negatives. For us, this is the right place.

Our mother has lived at Aspen in Cherished Memories for 4 yrs. We want to thank Tracy Terrell, Crystal Simpson, Valerie, Annabelle and all the regular staff who are so kind and loving with our mom. If we have an issue Tracy or Crystal are easy to communicate with and the issue is corrected. Our mom loves the staff, is happy and comfortable with Aspen being her home. My dad was here for rehab and the staff was amazing . Nurses Stephanie Meek , Estelle M and all other nurses were amazing but these 2 nurses were tip top and CNAS Star Barns , Steve Richards. And little Angie was awesome as well always stopped in to make sure if he needed anything. Our family was blessed to know that he was taken care of and we had no worries when we left

My mother really enjoys living AT. Although slightly understaffed, the long-term staff is dedicated, kind and in-touch with the residents. Debby, Ashley, Fatima communicate well with the family and my mother really likes them and my mother is very critical and hard to please. (Bless them) Building is well kept. Cleaning of apartments is well done. Food service is a challenge, but that has been the case in the other two facilities my mother has lived in since 2015. The site-based medical and pharmacy service was not a good fit, so continue with our own Primary Care and pharmacy services. All the non-care giving staff is communicative, responds timely to questions and my needs and questions. Highly recommend.

We have been very happy with the care my mother Rose Grubbs has received over the last 11 months. Her head nurse Lyndsey has always been available to me and helpful to mom. From the nurses, aides, therapist, activities helpers and social Worker we couldn't be more pleased. I feel relieved that I can leave her here with piece of mind. All the staff are always helpful in anything we ask for.

Randi, the Social Services Director, was incredibly supportive and attentive to my mom’s needs, helping us navigate a very complex situation of relocating my mom from Indiana to Florida. Her guidance ensured that my mom felt welcomed and cared for throughout her stay. Niesha in social services was equally wonderful, providing valuable assistance and support that made the process seamless and reassuring. The nursing staff, particularly Jennifer, Estelle, and Stephanie went above and beyond to ensure my mom was comfortable and received the best medical care possible. I can’t say enough great things about Aspen Trace Assisted Living. They were wonderful. However, I wish my mom would have never gone to their Memory Care. It was not a good experience at all. They really need to take a look at their care for their residents and be more compassionate with the families. I would not recommend the Memory Care.

Although having a loved one in a healthcare facility can be challenging, I have found that many of the personnel at Aspen Trace care about their patients and families. Our family has been helped so much by Niesha in Social Services, Amber - head nurse in the Heritage wing, Star - a CNA who shines bright in the Rehab wing.. Jessica - a nurse in the Heritage wing.. These are just a few I am thankful for.

My Grandfather - in law is currently in the rehabilitation center of Aspen Trace. I have been in a lot of rehabilitation/ Nursing homes in my life visiting family and friends of family. I have never been impressed with the employees , the care provided or the cleanliness. When I walked into Aspen Trace this morning to visit I was greeted at the door by Erika Anderson LPN with the brightest, sweetest smile she said Good Morning how may I help you? I told her I was there to visit my Grandfather and to my surprise she knew exactly who I was talking about, she said I can take you to his room, with no hesitation or having to look in the computer for his room number she personally walked with me to his door. I could tell Erika Anderson is an extremely busy person. With so much kindness she was answering questions from many different people (staff/visitors and patients), but no matter how busy she was, she kept a big bright smile as she answered everyone’s questions. About half way through my visit she checked in to see if there was anything myself or my grandfather needed. Erika Anderson if you ever see this ma’am always keep the kindness you have you were made to do this work. You are very appreciated by this family . During this visit I also had the pleasure of meeting Zach Peden RN. Zach passed my grandfather and I in the hallway as I was taking him for a stroll in his wheelchair to the eating area, Zach stopped in his tracks to help my grandfather put his arm on the handle of the wheelchair so he didn’t get hurt. The cleanliness of this facility is wonderful. Normally the smell of facilities of this nature are terrible and musty, but Aspen Trace just has a refreshing smell of clean and well kept. We are truly blessed for my grandfather to be able to spend his rehabilitation time at Aspen.

This is a very caring facility. Whenever I've needed anything, the staff took the time to listen and then address it. The meals have been pretty good and plenty. Steve, Star, Jenny, are above helpful. The rehabilitation staff are great. I won't say it's perfect, but for a nursing home, I think they do there best to help folks get back home.

My brother was a patient there, they were constantly understaffed, he fell a total of 6 times there once while in door seven had layed on the floor hollering for help for a long time he said around an hour and when I visited if you put the call button on sometimes you would wait for 20-25 minutes. There were a few nurses aides that were good but mostly everything else was horrible when he was transfered to door 3 one Sunday when I went to visit there was only one nurse there frantically trying to answer the phones tend to the patients all by herself because no one else had shown up for work. She did get on the phone to call for help which she did get about 30-45 minutes later. I would never recommend this place to anyone who has a loved one that cannot walk or communicate. Having said all that there were a few good people there such as a few of the CNA'S.

Diaden Sergi from dietary always goes above & beyond n all my (residence) dietary concerns! Since I have an issue w/ eating certain foods she will check n to c if there’s anything she can do for or get me to make sure I’m good & eating! She’s great @ her job coz she cares bout people & their needs! Aspen Trace is fortunate to hav such a dedicated employee coz these days staff like herself is hard to find & hold on to!

Diaden and company have been so super-gracious to my mother-in-law ever since she moved into Aspen Trace. We have dined with her (lunch and dinner) probably 40 times already, and the food has always been exceptional. It is such a pleasure to be greeted by such gracious staff and taken care of, and how they remember everybody’s name, I haven’t a clue. When I grow up, I want to go to assisted living at Aspen Trace.
