My mother, Nancy Blazer, lived in Independent living for 13 years and assisted for 2 months. She loved it there. The crew IN the assisted living, med techs, as well as kitchen crew were great. The food was very good. (Wish I could be there for the beef rib nights in the future!) Hailey , the director?, is an excellent communicator and is kind. I wish I could remember all the names of the staff who were so good to us!

I'm told this memory care is the best in town - so sad. Staff should be trained on how to place eating utensils on table (You pick up the spoon by the handle, not the "bowl.") Or, perhaps the little plastic glasses of koolaid could be given residents at the beginning of the meal, not half-way through. The thing about a resident (male or female) being allowed to wander in and out of private rooms seems invasive. And volume of the TV, in the common area, makes it difficult to carry on a conversation. - It's true that most places in Oregon are understaffed, but workers here seem pretty relaxed, so it must be OK. All the same, the folks I visit could appear better cared for.

My dad was a resident in both assisted living and for the last 6 months of his life in memory care. He lived at Flagstone for the last 3 years, so I have a good sense of things. While no place is perfect, I believe that my dad received good care while there. I am appreciative of all care givers from dining staff, support staff, and administrative staff who knew my dad, treated him kindly, and showed concern. In the memory care section I found many staff who were willing to go above and beyond in helping my dad. This was also true in the assisted living section as well. Recently, the facility is being remodeled with new carpet, furniture, and paint. There is a good number of activities for residents and my dad enjoyed those that he participated in and talked about them. Most importantly my dad did feel that he was treated well and often told he believed he was "well known" at Flagstone. This was important to him! When there were issues I found the staff to be helpful and willing to try and solve problems. They gave many helpful suggestions. Thanks to all who do this difficult work!

Every staff member I came into contact with was friendly, accomidating, and sincerely enjoying my family visiting! Flagstone was nicely maintained, such as fresh paint on the interior, and the exterior looked great. Grass areas cut and trimmed, and hard surfaces paved and / or concrete were swept or blown clean. The staff shares parking with visitors, so be patient when looking for a spot.

My mother was moved there after a hip replacement. The facility is understaffed, dilapidated. The sidewalk needs to be repaired to avoid trip hazards and resident falls. Med Tech and Caregivers are not properly trained an the turnover is ridiculous. Theft of resident property as well as visitor propery has occurred. The new director will not return calls from family members and cancels meetings after a letter was sent to family members to reach out to her to hear our concerns. There are many vacant rooms as well. This place needs help.

The facility has recently been purchased by another company. There are some hiccups with changing of hands and a lot of the services paid for are not currently being provided, ie. activities, housekeeping etc. They have also struggled with cashing rent checks in a timely fashion (a month late a handful of times) which have resulted in overdraft fees. Hoping this change is for the better.

We are thankful that we were able to move our Mother to assisted living . She has declined mentally in recent months and she needs the additional help. The receptionist, Carol seemed curt and suspicious toward us. She acted like we were not welcome there.

The memory care is understaffed as are most businesses. Most of staff is traveling so big influx of change which is not good for patients with dementia. There are hidden fees and many family obligations to keep your family member safe that Flagstone is not expected to provide. My dad's C collar neck brace was on upside down and could only be put on by licensed staff. Compression socks, if remembered, were put on wrong cutting into calves, instead of, above calves to knees. Hearing aids forgotten in ears or in charger. Observed staff member not taking blood pressure properly. It was being taken over medical bracelet and not accurate. My dad was fall risk when he entered Flagstone and lasted 2 weeks before fall that broke glasses, bruising of upper arm and fractured pelvic bone. Lens lost, no record of where fall was or check up on body, until, 2 days later when family member noticed great pain when moving or lifting leg. Family member waiting for staff, sat on couch in urine from resident. Staff knew about soaked couch cushion and that another person had sat in it, too. They had not removed cushion, placed a pad over it or stood it up to deter another person from sitting in urine. The bottom of a pair of my dad's tennis shoes were coated in dry human feces. They were discovered in packing up his room. Morning before, there was quite a bit of poop on hands, under nails and on back of hand at breakfast but discovered by family member afterwards. Bed was made over wet pad and sheets. I believe, Flagstone was not able to keep this patient, our family member, clean or safe. Roommate had sat in my dad's wheelchair with poop on his body transferring to my dad's daily transport. Residents can wander in and out of any room. There are laws that Flagstone is required to follow that include rooms not being locked but patient staff ratio should be included in that for residents AND staff safety. This was an expensive 2 week stay in a facility that is understaffed and not equipped for the memory care they advertise. Staff is friendly. Residential side of assisted care looks great with clients getting to keep their dogs and enjoy the beautiful grounds. I wish that is what my dad could have enjoyed.

When my mom needed respite care earlier this year we began calling different facilities, unfortunately, because of Covid, no one was able to do temporary respite care. Luckily we got a call from Flagstone saying that even while they stopped doing respite, they would make a consideration as this was a service they did provide. I should add that all of this occurred over Memorial Day holiday and they were still able to make arrangements for her to move in within a week. I can't say enough good things about the personalized care my mother received at Flagstone; from our first friendly encounter with Christine over the phone, to our interview and tour with Karees and Tiffany, from the moment she moved in she felt cared for and safe. From the med techs and nursing staff, to the maintenance crew and kitchen staff, everyone was knowledgeable, friendly, kind and caring. Their activities director Debbie keeps the residents engaged with bingo, crafts, shopping trips and other fun activities, and even welcomes suggestions for new ones. This is a beautiful facility located above the Columbia River, the common areas are kept clean, there are regular covid-19 checks, it's quiet & secure, run by an incomparable group of people who genuinely seem to care about every resident. If you have to make the tough decision to have someone else care for your loved one, Flagstone is a wonderful alternative.
