My name is Alex Blair. I am the daughter of Scott Blair. I was 17 years old at the time of this incident in 2019. My father was 50 years old and in the end stages of a six-year battle with Stage IV Glioblastoma brain cancer. Due to progressive neurological decline, he had significant loss of motor function and was experiencing frequent falls at home. Our residence was no longer safe for him, which is why our family made the difficult decision to admit him to Casey’s Pond in Steamboat Springs for safety and supportive care. Upon admission, we made it explicitly clear to staff that my father had very limited use of his arms and hands and required assistance with eating, mobility, and transfers. The day after his admission, when we visited for lunch, he was served spaghetti despite his inability to feed himself. He was not assisted. When I respectfully requested alternative food or assistance, the nurse responded dismissively and did not address the concern. Later that afternoon, we left briefly while he was resting, with plans to return for dinner. Within approximately 30 minutes of arriving home, we received a call from his hospice nurse informing us that he had fallen and required stitches. When we returned to the facility, my father was back in his bed with a deep, open forehead wound, active bleeding, and rapidly forming bilateral black eyes. There was visible blood, and no bandaging or wound care had been performed. Staff informed us they could not provide stitches and that he would need to be transported to the emergency room if stitches were desired. Earlier that same day, I had observed what appeared to be a serious delay in the bed alarm monitoring system intended to prevent falls. While my father was in the restroom, I briefly sat on his bed. When I stood up, a nurse entered the room nearly 20 minutes later in a rush, asking if he was attempting to get up. This demonstrated a significant delay in the alarm response system. Based on this experience, I believe my father may have been on the floor for an extended period of time before assistance arrived. The primary reason for his admission was fall prevention and safety. Instead, within less than 48 hours, he experienced a severe fall resulting in a traumatic head injury. Following this injury, he rapidly declined neurologically. Within approximately 1–2 days, he lost the ability to speak, form sentences, and communicate with his family. We believe the head trauma significantly contributed to this decline. Our family ultimately had a physician friend come to the facility to clean and bandage his wound because immediate appropriate care was not provided. We believe Casey’s Pond failed to: - Provide adequate fall precautions and timely monitoring - Properly respond to bed alarm systems - Provide appropriate assistance with feeding - Provide timely wound care following a traumatic injury - Ensure basic dignity and safety during end-of-life care This experience caused preventable physical harm to my father and profound emotional trauma to our family during his final days. Personal Impact Statement: The events that occurred at Casey’s Pond during my father’s final days have had a lasting emotional impact on me and my family. My father was admitted to the facility for safety and protection during an already devastating end-of-life battle with brain cancer. Instead, he experienced a traumatic fall that we believe accelerated his neurological decline. Within days of the injury, he lost the ability to speak and communicate. I was deprived of the opportunity to hear my father say “I love you” one final time. That loss is something I will carry for the rest of my life. Knowing that he may have lain on the floor injured and unattended, and that he was left bleeding without immediate care, has caused ongoing distress, anger, and grief. This was a time when he deserved dignity, compassion, and careful supervision. Instead, our family was left with trauma layered on top of an already heartbreaking loss. I am submitting this complaint not only for accountability, but to prevent another family from experiencing similar harm during such a vulnerable time.

I have been a resident here at Casey's pond for over 9 years and couldn't be happier with the facility and staff. Despite my physical disability I try to remain as active as possible. I am grateful for the staff enabling and encouraging me to be independent as much as I can but when I do need help it is a comfort to know that there are professionals available to help me 24/7! ALL of the departments- admin, plant ops, culinary, and especially the nursing crew- are staffed by friendly, professional folks who make Casey's Pond a great place to live!

My wife and I stayed in one of the apartments for a few days just to "test drive" the facility. We were really very pleased with the whole experience, from the physical plant to the staff to the food, and especially the warm reception by residents, old and new. We have always been impressed with the Steamboat Springs community, especially since we are still avid outdoor folks. Now we have a place in that community that we can one day call home.

My mother was in the Doak Walker facility for a little less than a month recovering from a broken hip. The care and compassion were exceptional. From the caregivers to the administrators I cannot imagine a better place.

Our family has had an excellent experience! The staff at Casey’s Pond are gracious, skilled, down-to-earth and excellent at what they do. From our first introductions over e-mail, all the way through to our move-in day, the communication has been warm, receptive and intuitive. The building is beautiful and tended to with care by an upbeat and diverse set of care providers. We feel so fortunate to have found such a great match!

I’m witnessing parents with their children fishing and making wonderful memories. My hubby is taking a break from driving these alpine roads. Steamboat Springs is magical with its vistas and water walkways. I’m definitely visiting again.

I moved into Casey's Pond with my wife when it first opened and stayed there until my wife passed away. I have been living alone ever since. It has been getting to be a challenge since I don't drive a car. I might consider moving back if I were able to use Zoom to visit my young grandchildren whenever I could arrange it. I plan to have a Zoom meeting with my grandchildren who live in Indianapolis in a couple days. I also think Zoom should be made available to other residents so we could talk about our grandchildren.

Wonderful Caring Staff. Been very difficult for all residents due to the COVID restrictions, but the entire staff is going above and beyond to try and provide support to both residents and families. Wonderful facilities and great location.

My Mom and Dad are actively living at Caseys Pond. I have found that the staff is remarkable! Everyone is genuinely active with the residents. It seems as if there is an open door policy when needing information. Especially when it comes to my Mom. It isnt cheap the only thing missing at the this Retirement Resort is a dive shop and a beach. ITS THAT GOOD!
