RE: Rehab Center at Bristol, The (No. 49-5245), Parent Company: Traditions Senior Management, Inc. Operator: 301 Village Holdings, LLC. The Rehab Center and Memory Care at Bristol has a memory care wing as well as a SNF or long-term care section. The Facebook page of this facility posts photos depicting many activities with staff engaging with residents; however, these activities appear to be associated with the care provided to memory care patients. I know nothing about the care provided to the residents/patients in the memory care section, but my family has had experience with The Rehab Center over the last five months as far as care delivered to a SNF patient, a patient no longer able to independently meet activities of daily living. In my opinion, if you are considering placement of a vulnerable patient in a skilled nursing facility, avoid the skilled nursing services of Rehab Center at Bristol, at all costs. I made this comment to their Facebook in the comment section of a photograph and was subsequently blocked from their Facebook page.

The Memory Care Section of this facility is separate from the Long-Term Care and Therapy areas. I have worked in the Memory Care facility and love the care that they provide their patients. They strive to make the facility feel more like a home setting. They provide restaurant communal dining, providing even the smallest details such as shatterproof goblets and nice tablecloths for their meals. They have a dedicated activity assistant to provide activities for their residents. The facility is kept clean, residents are able to bring in their individual furniture to provide a more familiar environment. The director is very involved with families and the care. Of the many memory care facilities that I have worked in, this one is one of my favorites.

The Memory Care facility here is awesome! The administrator, director, nurses, and aides are all wonderful to the residents there. The nurse practitioner is so sweet and she is easy to talk to about any medical concerns. They seem to genuinely care about the residents and take such good care of them. The therapy department is very dedicated with helping the residents. The director and staff are great at dealing with dementia and seem really knowledgeable about this terrible disease. This is such a good place for someone with dementia to be!! I've never seen a place that that makes you feel like family as much as they do!

I am so happy that I found this facility for my aunt. From the first day the staff made us feel so welcomed. Misty the director is very knowledgeable she is always there to answer my questions in person or on the phone. I feel that I have finally found the perfect place for my aunt. I just want to thank all the staff there for taking such good care of her which I know can be difficult at times.

Great place with great people to take care of your loved ones... Yeah they got a few hiccups but what place doesnt... Im not saying theyre the best or the greatest because they arent... Theres no place that is perfect and its always greener on the other side...

I had to put my father in a rehab earlier this year. With covid new and scary, I was so nervous about placing him anywhere. I am so happy with our experience at Bristol Rehab. The staff were sweet and always available to answer my questions. My dad really enjoyed Laken with therapy. She got him up and moving again. Joanna and Brittany were on top of his medical needs. And Rebecca helped keep his spirits up while being his CNA. They worked hard and did a great job.

My mom went to this facility for short term rehab. We had a couple hiccups in the beginning but after everything was explained and I understood how things are different from being in the hospital everything went really well. I was very pleased with the nursing care. The therapists were great too!
