As of right now, I'm rating this facility a 3 based on proceed/procedures for admission, communication and follow up. The facility itself is fairly well maintained and the staff is friendly. My father was admitted on September 2nd and it is September 8th and I have still not been in contact with the Social Worker nor has there been any type of well care meeting with the family coordinated. While the staff is friendly, the lack of communication and follow up from all departments is extremely frustrating. There is no formal admission process which seems crazy to me. The facility has no knowledge of my father's history, current medical condition or back story. I would think all of that would be important to understand while a care plan is being created. As well, this facility should know who the family primary and secondary contacts are. I completely understand the challenges in this industry but we MUST do better and we CAN do better. Communication is key! Follow up is key! If an employee says, "I'll get back with you as I'm not sure how to handle your question or am not sure of a status", get back with the person. No one is happy that our loved ones are at this facility or any other facility for care but we would be much happier if the staff communicated.

Very clean facility, patients seem to be very well taken care of.The lady at the main door of the office was very friendly and helpful. Nurses wllling to discuss residents health condition and etc as long as you are listed as being on the residents HIPA form.

My experience as a family member of a Symphony Linden resident has been frustrating and demoralizing. In two years, I've interacted with many kind, caring, and professional aides, nurses, and physical therapists. However, the rotating cast of Administrators, Directors of Nursing and corporate contacts have repeatedly shown that they have little regard or respect for their patients and the family members. Some lowlights: - SL filed for conservatorship without notifying our family. The legal filing was full of demonstrable errors and lies. We notified SL and their lawyers, corrected the falsehoods, and asked for an amicable resolution. The matter dragged on for months, with minimal communication, explanation, or remorse from SL. - Record keeping is abysmal and not up-to-date. Our family has repeatedly resent communications (emails, meeting notes) regarding patient history and health plans. New staff members have been unaware of our family member's needs. - Missed and inaccurate medications, lax Covid precautions, and untrained staff using mobility equipment are serious matters to us. Our written concerns regarding these instances are usually ignored, though on rare occasions, we get a phone call to discuss their response - which cannot be documented. Perhaps this is due to staff turnover, poor record-keeping, or deliberately keeping these infractions under wraps (no written acknowledgement of patient endangerment?). We've requested that all interactions are documented in writing to no avail. These are just a few of the issues that my family has gone through while trying to be supportive of our loved one's care. "Callous," "incompetent," and "negligent" are words that we would use to describe the majority of our interactions with the Linden administration and Symphony corporate office. I hope my review gives a warning to those considering entering a relationship with this company. I highly recommend looking elsewhere for quality care for your loved ones, and partnership, compassion, and respect for your family.

Well here I go again. I am a personal caregiver for "guest" at symphony. I give her her bed bath, I lotion her down, I do her laundry because they cannot pretreatment and her clothes are extremely stained, I sweep and clean her room I go fetch whatever she needs. Now dont get me wrong, I don't mind doing these things....it's my job she private pays me to be there for her. Repeat, SHE pays me not Symphony or anyone else so for the administrator to think it is necessary to tattle that I am laying on her love seat or on my phone is neither yours or the person you are reporting to. If you were making sure your emoyees were doing their jobs instead of your employees hanging out talking laughing, fighting, arguing and threatening each other then you can tattle. I do my job for my client and your facility's as well. Like making sure she gets a serving of each meal she is paying for, making sure she actually takes her meds, changing her canula as needed and emptying her water trap so water doesn't go to her nose. I could go on but you get the pic. Yes, I may be on my phone, what you don't know is alot of times I have looked up things and we are discussing it. Yes I layed on her loves eat, I come to look out for her even if I don't feel well because some of your employees are incapable and you don't have the backbone to fire them. Instead of worrying about if I am on my phone or laying on her loveseat which she knows and she is my employer take care of your own employees. That's why this place is going downhill like it is..

My dad has been living at Symphony of Linden for over 2 years. The staff has been courteous and attentive to my dads changing needs even during the covid-19 pandemic. Before selecting Symphony of Linden, I did many on-site visit-tours of other facilities in the area. If I had to do it over again – would bring my family members back to Symphony of Linden. Thanks for keeping my dad covid19 free!

The convenience to being close to my home is one of the benefits of this facility. The Aids on the floor are very nice and caring, but the facility food is terrible, when an elderly patient complains about the food and small portion size, you know it is bad. With the pandemic I felt the staff was disrespectful and uninformative to family. They would not let you send anything into your loved ones, like fresh fruit ( which they lack) snacks or beverages, even a gift, without a fight. This pandemic is tough on the elderly who are use to seeing loved ones daily. My mom become depressed and confused, because she was just setting in her room with no social interaction. She knocked off tv and broke it, it was a nightmare trying to get her tv replaced. I ended up removing her to come home with me and she is doing much better but the fight still continues with getting a refund for time paid but she didn't stay.

After unfortunately losing a leg to a virus, I was fortunate enough to spend 100 days being cared for and trained by all the staff here in Linden. From my first contact with Aimee explaining the facilities capabilities until the tremendous send off from my "New Family" with tears in my eyes I have nothing but respect and admiration for their help to start to get my life back together. The nursing staff has great diverse knowledge that made such a difference in my healing, the aids, and everyone from the staff kept me moving forward every challenge. I've made friends here that will make the balance of my being full of smiles. The physical therapy employees truly cared for my improvement and honored me with quite an ovation for my first step. All of you mean so much to me and I truly love the work and encouragement Jennifer, Patty and all of you gave to this old man in his time of need. Heidi, Ben, Scott, Jenny and all I look forward to visiting soon when I acquire my permanent leg. You folks are all truly caring individuals and I found the facility top notch!!

Caretel Inns of Linden treated my sister like a precious gem. They made every accommodation possible for her in addition to making sure that all family members and friends were welcome at all times. We were encouraged to participate in every aspect of her care and supported during such a challenging time. Her physical therapists, Troy and Jennie worked a miracle with my sister and were instrumental in building her confidence, addressing her fears, and making her look forward to each day's worth of physical recovery progress . Every single staff member contributed to my sister's progress including but not limited to the custodians, kitchen staff, director, social worker, nursing, CNAs, dietitian, and physical therapists. It was a team effort and they made good on their commitment to exceptional care regarding my sister. I can't communicate the gratitude that my family feels toward Caretel Inn of Linden for their part in bringing my sister back to us.

My father-in-law has been at Caretel for rehab 3 times. Each time he has received excellent care.The staff is so caring and attentive. They get to know their guests and what their particular likes and dislikes are. You can't beat the rehabilitation team and center. All my questions are answered and addressed. There are fun activities if you choose to join in. It's a beautiful facility but the most important thing to us is the care our loved one gets.

My mother in law spent 5 months here and was doing wonderfully in her rehabilitation after almost dying in the care of another facility and spending months in the ICU. She came here & was treated with kindness and compassion from all staff as well as the therapy department! After over 3 months here we found out she had stage 4 cancer & SHE chose to stay at Caretel to die with those caregivers that had always showed her so much love & compassion every day! Her therapist & nurses even came to her funeral! Thanks to my wonderful mother in law I found the best job working with those who cared for her! I am reminded every day why I love this job!!

This was my dad's happy place. He went there a few times for rehab. He made great progress, and loved it. The first time he left he shed a few tears. This place is THE BEST. Wonderful staff. I would highly recommend this to anyone. I've never been to such a great place. Wonderful layout. Took many pictures. It's like it's own little town. Can't say enough good things.

Facility has customer satisfaction surveys in place as well as a very detailed concern/ complain procedure to try to resolve any issues guests may have

My brother has been here off and on since nov of 2015 and a year later still going there. i absoluetely love this place. it is claen, well kept up and the food is good. the staff is always pleasant and goes above and beyond. i would highly recommend this to all.. i am in the Ems business of 26 years and the medical attention patients get here is awesome. they are on top of it,,,,,

Caretel Inns of Linden has three different types of care. It has assisted living, a memory care unit for people with dementia problems, and another area for skilled nursing. They’re different modules in the same location. The actual one I saw was the assisted living. The staff was very good and very competent. They knew what’s needed for senior citizens. The dining area was very good. They have specials every day or, if you just want the same thing every day, there's a set menu of things you could choose from. They have a movie theater, a little store where you could buy stuff, a hair salon and you could get your nails done there. The family members can get a pass to go directly into their parents’ or family member’s place. You can visit at any time and stay there with them. They’ll get you a rollaway bed, so you could stay with them. The parking was very close because it surrounds it. They have little courtyards and lots of flowers around, and the maintenance crew is good.

We Visited Caretel. It was clean, smelled nice, and was very organized. The staff was friendly, professional, and informed. It was good place that my mother could have been. The rooms and apartments were spacious. The food was appetizing. They had choices for dinner. I thought they had a chef on staff. The nurses were friendly. The nurse-patient ratio is probably four or five to one, and then they had CNAs too. They do different kinds of games. They had an arboretum and wild birds. They have best physical therapy and occupational therapy. They had board games night. I thought they had book readings and a book club. They had cards you had to have to access places in the facility to visit your loved ones.

Caretel Inns had everything we need. The place was only two years old, and that was a big factor. There were no bugs in the place and that was a good thing. The staff was pleasant and observed cleanliness. They treated their residents well. The apartments had one bedroom and a kitchen in, where the residents can cook. The residents can also go down to their dining room. They have 3 meals a day and snacks. My wife was a trained chef, and she said the food was good. During our visit, there were residents playing cards outside. They had a pool for residents to swim in. They had three vans that picked people up to go shopping. We walked all over, and there were a lot of activities going on. Their security was very good. They had cameras all over. They had slide keys like in hotel rooms. Their price was $2800 a month, and it was reasonable.

At Caretel Inns, everything had a time set to it. They were all very scheduled. There was rest time but there was also activity time. When there was activity to do, they wanted you up doing their activities. They also had people from a hair saloon come in two days a week and get their hair done. My sister participated in some of those activities. She also had great physical therapy. Personnel wise, they keep the residents in good spirit. If there are aggressive people, they would stop them right then and there. With regard to food, my sister raved that she really liked their food. Everyday they got soup at lunch and a different soup in the evening. She really looked forward to that.
