Staff was helpful. Do not trust Dr. [Name Removed]. He never signed off on death certificate for my mother. Irresponsible and negligent after a week of her passing. Hospice Dr. responded immediately. It took a week before she was cremated because of his lack of caring for his patients.

This place has been absolutely wonderful in their care for my husband. He was seriously injured at work and suffered multiple broken bones including his lower back, his hip, multiple breaks in his left leg, knee & ankle, a shattered left shoulder, broken collarbone & 7 ribs were broken off from his spinal cord and he suffered radial palsy on his right side. He spent 5 weeks in the hospital & was released but still needed round the clock care & physically therapy, as well as therapy when the time comes that he can bear weight on the mended bones & begin the process of walking again…… From the first contact with Maggie (in admissions), we have been treated with genuine compassion and a willingness to assist us through each hurdle in this difficult situation (getting equipment , trips to the doctor, etc..).I can’t say enough about the terrific care that he has received while in Lilac of Bayview. Many THANKS to Maggie, the therapists & the nurses & the entire staff there!!

My father has been a patient in your facility for a few weeks now. It was brought to my attention that his request for a copy of his medical record upon discharge tomorrow was declined by someone named ‘Robin’ from medical records, who came to speak with him. My father communicated to me that ‘Robin’ said his records “belong to Bayview” and it would “take weeks to get a copy” to his healthcare providers. However, numerous regulations, including HIPPA, provide my father with the legal right to a copy of his record upon request. I called tto speak with anyone in administration. The woman who called me back identified herself only as “Shelley” and sounded painfully aware that she was stepping into a patient’s family-member complaint. I asked how she was, and her response was a deeply sarcastic “fabulous”. I was immediately confused by her complete lack of professionalism and flippant response. The call went downhill from there. As a medical professional, licensed in two states, I am familiar with hostility from employees who are on-call and have to make actual calls, even though it is part of their job. Anyway, I was in no way prepared for a nasty tone coming from someone who is literally responsible for the welfare of my elderly father. I explained to Ms. Nutter the issue with my father’s request for a copy of his medical record and added that I had already contacted The Joint Commission regarding numerous other violations I’d been made aware of since my father’s stay began. Ms. Nutter responded by laughing, “I don’t answer to The Joint Commission”. I think you get the gist with Shelley Nutter, who’s last name I had to look up on your website because she is THAT professional.. I have no doubt her passive aggressive unprofessional behavior isn’t new on this day. Is that really the way you want your business represented? I will obviously file complaints with any of your accreditors in St. John’s County, the state of Florida and anyone else who will listen. I have no doubt that there are wonderful people who work for you. My dad apparently met a few. However, your facility is only as strong as your weakest link.

I highly recommend Bayview Healthcare. My mother has had several surgeries and this is the first rehab facility that she has NO complaints with. All of the staff are super friendly and eager to please the patient. I can rest assured that shes being well taken care of and shes happy there!!

My mother has moved into BayView. It is going very well. I'm very happy with the care that she is getting there. They have lots of activities for her. They have field trips, trips to Wal-Mart, music, and entertainment. I'm very happy with what they offer. I'm very happy with the staff members, too. They seem to care about her and take very good care of her. Nevertheless, my mother has to be encouraged to take part in activities, so they need to be more aggressive and more insistent that she takes part in the activities.

This is such a wonderful place. If you have a love one that need to go into a long term care setting search no more. The staff here are so friendly and kind. Food is awesome and if you are here for rehab , the therapy department is awesome as well. Oh did I mention the view? This facility is located on the water and you can see boast sailing and the lighthouse. .Come to Bayview and become part of our family

My friend is staying at the Pavilion. They are very caring, and they do everything they said that they would do. They take charge of cleaning her room and washing her clothes, they have games and stuff, and they have people come in and entertain them, so it is great, and she is very happy there. The grounds are beautiful. She can walk out near the river where it is very pretty and very quiet. They have exercise, and then they bake cookies, and they have little things that they do every day to keep them interested. They take them to Walmart once a week and supply someone to take them if they had to go to the doctor.

We like the location of the Pavilion because it is near the water, which is very important to my sister. I like the outdoor spaces because it is close to nature and has a natural environment. The people are very friendly. The room is adequate for her, and my sister says the food is satisfactory

The Pavilion at Bayview, adjacent to the Bayview Nursing Home, is a wonderful facility. The staff, in addition to being courteous and friendly (which should be in their job description) also seem to genuinely care for each and every resident. I've never been greeted with anything less than a warm smile and a friendly face. The apartment my grandparents share is a three-room affair with a small kitchenette/dining room, a sitting room, and a bedroom. The bathroom is quite spacious and easily accessible. Outside the facility is a pool - around which the staff sometimes organizes events for residents and families. There's a wonderful view of the Intracoastal and Anastasia Island. There's also a spacious lounge and an activities area (complete with varous classes and fitness opportunities). There is also easy access to the Nursing Home in the adjacent building. Meals are served in the dining room and, while I've never eaten lunch or dinner there, breakfasts are fantastic. Both my grandparents seem genuinely happy with the facilities at the Pavilion at Bayview; and it is always a treat to visit them.
