My mother has been at Pathways for almost 2 years. We researched and toured many facilities. Pathways was the best for her. She has her own room, social activities and kind caregivers. She is happy and well cared for. Our family is thankful!

Sending a special Thank you to George for taking the time to meet with me. He went above and beyond to be able to place my father-in-law in a very short notice. Pathways takes great pride in the services they provide. This is evident by the inviting and at home feeling you get the minute you walk in. If you are looking for a place for your love one, I would recommend checking out Pathways.

It is a smaller, behavioral-based place. It’s very clean and updated, and everything is brand new. The staff came up and introduced themselves. How they interacted with my father when he had gotten there was very good and very personable. They did very well in just a short time.

It was a beautiful facility. The lady who assisted me was very accommodating. The rooms looked good and included a shower in some; other rooms had community showers. The dining area and staff were really nice.

My husband was admitted to Parks Senior Villas, Memory Care, Goodyear, Az on 9/16/20. I had several conversations with the Director of Community Relations who advised that the unit my husband was going to was well staffed with a Med Tech and caretakers who had to take an Alzheimer's course to familiarize themselves with some of the behaviors of Alzheimer's patients. She said there were always several employees on the floor at any given, and that even she and other office personnel would visit the residents from time to time and were even on a first name basis with some of them. My husband and I were interviewed with the Licensed Practical Nurse. She did cognitive tests with my husband and I talked at length with her about his medical history and medications he was currently taking. I put this information in an email and sent it to the Director of Community Relations. In addition, copies of my husbands medical records were requested from his Internist, Neurologist and Urologist which I was told were received. We also had a conversation about Hospice Care and I was told by the Director that many of their residents used Sage Hospice and were very happy with them. She said hospice was covered by Medicare. I indicated I did not feel my husband needed hospice as he was not suffering from any life threatening illness other than the Alzheimer's. She said many of the patients on hospice lived for many years on hospice and that it didn't mean they were at the end of their life. I contacted Sage Hospice, and a nurse came to our home. I went over all of my husband's medical history, medications, etc. with her and ultimately we signed on with them. I was told that a registered nurse from hospice would initially visit him twice a week, and that she worked with the hospice doctor and that he would provide information to me in about 2 to 3 weeks. I was reassured by the Director of Community Relations, and the facility nurse that if anything at all changed with my husband that they would call me "immediately". Feeling comfortable that my husband was going to get the care and attention he needed, he was admitted to Park Senior Villas on 9/16/20. On admission my husband's medical condition was good; his blood pressure was normal; he did not have a UTI which is common with catheter users and in July 2020, his Urologist put him on a maintenance dose of antibiotics to avoid UTI's. In addition he was seen by his urologist several days prior to his admission at Park Senior Villas. The Director stated that they strongly advised that family members not visit for the first two weeks to give the resident time to acclimate. She also stated that because of Covid 19, that an appointment needed to be made prior to visiting. I made an appointment that day to visit him on 9/30/20 at 1pm. I was told I could call his unit any time which I did. I called different shifts and was told he was "OK"; he was "doing good"; "he ate his breakfast"; "he went to bed with an ice cream sandwich"; he ate his sandwich". I asked the Director to send me a picture of him which she did on 9/23 with him standing in the activity room. On 9/23, I spoke with the Hospice Nurse who said he was doing good. I spoke with the Hospice Nurse again on 9/29. She said he was sleepy so she didn't check his blood pressure; she said he had a "little" cough but no fever and that he was "OK". She said she was told he hadn't been very hungry the last couple of days. I asked her if he was ok, she said he was and that she'd see him later in the week. A few hours later someone from the facility called and asked if I had a Do Not Resusitate on him. When I asked what was wrong I was told his blood pressure was low. I told her to immediately call 911 and have him taken to the nearest emergency room. I met him at the hospital as they were bringing him into the emergency room. I spoke with the admitting doctor who said he was "very sick", extremely dehydrated and weak, and that they were in the process of ordering blood work and a chest x-ray. Later that evening when the tests were back, I spoke with the night doctor who advised he had pneumonia, kidney damage, his blood results were so high "they were off the charts from the UTI he had, he had sepsis, his urine was orange in color, and his blood pressure was very low. He had lost ten pounds from the day he was admitted to Park Senior Villas on 9/16, and he was totally disoriented. When I was able to visit him, he had the same shirt on that he worn on 9/16, he also had it on in the 9/23 picture. It was extemely dirty and stained. He had a huge bed sore on his bottom. He couldn't speak to me. His teeth were filthy, ane he hadn't been shaven in days. After two days of treating him aggressively with fluids and antibiotics the doctors advised that he wasn't going to get better. On 10/3/20, we brought him home with a different Hospice and he passed away on 10/4/20.
